Mapping epicardial fat with multi-detector computed tomography to facilitate percutaneous transepicardial arrhythmia ablation.

A sizable portion of ventricular tachycardia circuits are epicardial, especially in patients with non-ischemic cardiomyopathy, e.g. Chagas disease. Thus there is a growing interest among the electrophysiologists in transepicardial mapping and myocardial ablation for treatment of arrhythmias. However, increased epicardial fat can be a significant hindrance in procedural success as it can mimic infarct during mapping and can also decrease the effectiveness of ablation. Quantitative knowledge of epicardial fat pre-procedure can potentially significantly facilitate the conduct and outcomes of thes…

Endocardial and epicardial activation maps for three-dimensional perimitral flutter using a three-dimensional mapping system: a case report.

BACKGROUND: Perimitral flutter (PMF) is a macro-reentrant tachycardia, and mitral isthmus (MI) linear ablation is considered to be the prefe … duction pathway through the intramural muscle/bundles that could not be mapped with the addition of epicardial mapping. The tachycardia was terminated by ablation at the mitral valve annulus in the 2 o'clock position, where the bundles might have been attached. DISCUSSION: Both endocardial and epicardial activation maps indicated 3D-PMF, whose circuit included the …

Endo-epicardial mapping of human sinus node in vivo: Novel electrophysiologic findings and anatomic correlations.

BACKGROUND: The human sinoatrial node (SAN) pacemaker is a complex structure located at the right atrium (RA)-superior vena cava (SVC) junction. OBJECTIVE: This study aimed to perform in vivo endocardial and epicardial electroanatomic mapping of human SAN in inappropriate sinus node tachycardia (IST) and to correlate electrical findings with anatomic observations from thoracoscopy during hybrid SAN-sparing IST ablation. METHODS: All consecutive patients with diagnosis of symptomatic IST, refractory to or intolerant of drugs, and endocardial and epicardial mapping of SAN during hybrid ablatio…

Demonstration of Arrhythmia Substrate-Associated Dispersion of Repolarization by Epicardial Unipolar Mapping in Brugada Syndrome.

BACKGROUND: Epicardial unipolar mapping has not been thoroughly investigated in Brugada syndrome (BrS). OBJECTIVES: This study aims to examine the characteristics of epicardial unipolar potentials in BrS and investigate the differences from overt cardiomyopathy. METHODS: Epicardial mapping was performed in 8 patients with BrS and 6 patients with cardiomyopathy. We investigated the J-wave amplitudes using unipolar recordings at delayed potential (DP) sites via bipolar recordings. The repolarization time (RT) at and around the DP recording sites was measured, and maximum dispersion of the RT d…

Biventricular pacing in patients with Tetralogy of Fallot: non-invasive epicardial mapping and clinical impact.

INTRODUCTION: Patients who have undergone repair of Tetralogy of Fallot (TOF) often present with right bundle branch block. Cardiac resynchronization therapy (CRT) with right ventricular (RV) or biventricular (BiV) stimulation has been proposed as a modality to correct electrical abnormalities and improve cardiac contractility in patients with repaired TOF. We aimed to 1) compare ventricular electrical activation in adults with repaired TOF during RV versus BiV stimulation, using a non-invasive epicardial mapping system, and 2) examine the clinical mid-term effects of BiV resynchronization. M…

Enhancing Geometric Fidelity of 3-Dimensional Electroanatomic Mapping During Open Chest Epicardial Radiofrequency Catheter Ablation.

Although electroanatomic mapping techniques have been previously applied to open chest epicardial ablation procedures, such efforts have often been limited by significant geometric distortions introduced by the need to use nonstandard mapping patch placements and by intrathoracic conductance changes introduced by having the pericardial space exposed. In this article, we present a case of a patient with recurrent hemodynamically unstable ventricular tachycardia who underwent a successful open chest epicardial ablation procedure with electroanatomic mapping in which geometric distortions were mi

Detection of Epicardial Late Potentials by Endocardial Wide-Bipolar Mapping in a Case of Cardiac Sarcoidosis.

BACKGROUND: Identifying substrates for ventricular tachycardia (VT) from the endocardium is often difficult when the arrhythmia originates f … h wide-bipolar configuration revealed low-frequency late potentials within the aneurysm. Subsequent epicardial mapping demonstrated extensive late potentials on the contralateral side, and ablation at this site was successful in eliminating VT. DISCUSSION: Wide-bipolar electrograms include more far-field components than close-bipolar or omnipolar signals, enabling …

Critical Isthmus Identification in Macro-Re-Entrant Atrial Tachycardia With Epicardial Conduction by Peak Frequency Mapping.

Macro-re-entrant atrial tachycardia (AT) can involve complex conduction pathways, including rare transient epicardial detours that complicate identification of the critical isthmus. We analyzed 5 patients with macro-re-entrant AT in whom activation transiently propagated from the endocardium to the epicardium and then returned to the endocardium. Peak frequency mapping using omnipolar technology was performed in conjunction with local activation and voltage mapping. A transient epicardial pathway was suggested when endocardial activation disappeared and subsequently reappeared along the re-ent

Use of three-dimensional electroanatomic mapping for epicardial access: needle tracking, electrographic characteristics, and clinical application.

AIMS: Pericardiocentesis is usually completed under fluoroscopy. The electroanatomic mapping (EAM) system allows visualizing puncture needle tip (NT) while displaying the electrogram recorded from NT, making it possible to obtain epicardial access (EA) independent of fluoroscopy. This study was designed to establish and validate a technique by which EA is obtained under guidance of three-dimensional (3D) EAM combined with NT electrogram. METHODS AND RESULTS: 3D shell of the heart was generated, and the NT was made trackable in the EAM system. Unipolar NT electrogram was continuously monitored
