[Case report of a primary pericollagen amyloidosis (author's transl)].

The authors report about a female patient, 59 years old, who was suffering from a stenosis of the cardia. A malignant process was suspected and an operative treatment was planed. During the preoperative internal examinations a low voltage in the ECG was found so that an amyloidosis was suspected. This was proved by biopsy of the mucosa of the rectum. A chronic suppurative disease could not be found, it was then assumed that the patient is suffering from a "primary pericollagen amyloidosis". Because a splitting of the stenosis through the esophagoscope the biopsy showed no malignancy, but congo

Chest Tube Placement of Secondary Tracheoesophageal Voice Prosthesis: Overcoming Challenging Anatomy in the Laryngectomy Patient.

Introduction: Total laryngectomy is used to cure advanced larynx cancer in many patients. The removal of the larynx requires the rehabilitation of the patient's ability to communicate, and one common method is to place a tracheoesophageal voice prosthesis (TEP) as a secondary procedure after the patient has completed cancer treatment. The traditional technique utilizes a rigid esophagoscope for access, but this can prove difficult in many patients who have kyphosis, scarring of the neck, or trismus. We describe a technique to allow TEP placement in these challenging patients that does not util

A new endoscopic technique for suspension of esophageal prosthesis for refractory caustic esophageal strictures.

There is no clear consensus concerning the best endoscopic treatment of benign refractory esophageal strictures due to caustic ingestion. Different procedures are currently used: frequent multiple dilations, retrievable self-expanding stent, nasogastric intubation and surgery. We describe a new technique to fix a suspended esophageal silicone prosthesis to the neck in benign esophageal strictures; this permits us to avoid the frequent risk of migration of the expandable metallic or plastic stents. Under general anesthesia a rigid esophagoscope was placed in the patient's hypopharynx. Using tra

[Measurement of the length of the adult esophagus using a fiberogastroscope: 104 cases].

From May 1979 to July 1985, the length of the esophagus in 104 patients with gastric disease was measured by a WX-C3 type fiberogastroscope. The result of measurement showed that the average length from the upper incisors to the cardia was 44.4 cm, and that from the upper end of the esophagus to the cardia was 28.0 cm. These measurement were respectively 4.4 cm and 3.0 cm longer than the traditional data (P less than 0.01). The difference in the length of the esophagus may be explained on the basis that measurement by rigid esophagoscope is in a straight line, while that by flexible fiberogast

Endoscopic examination of corrosive injuries of the upper gastrointestinal tract.

The authors present the case histories of five patients who have sustained corrosive injuries of the upper gastrointestinal tract. Flexible endoscopic examination was successfully carried out in each patient to assess the extent of the injury and anticipate further therapeutic needs. Three patients sustained major gastric lesions, i.e., strictures or bleeding following the ingestion of alkali, acids, or phenol, and one instance of esophageal stricture was recognized. The endoscopic observation of antral gangrene was inexorably followed by antral stricture requiring surgical management. One fat

Applications of intravascular scanning and transesophageal echocardiography in congenital heart disease: tradeoffs and the merging of technologies.

This chapter will review the evolving role of intravascular ultrasound imaging and transesophageal echo in the care of children, infants and … theters, the requirements for transesophageal echocardiography require higher frequency and smaller esophagoscopes. Applications of intravascular imaging including sizing of congenital stenoses, dilation of coarctation and valvular stenoses, imaging of intrapulmonary thrombi and monitoring of placement of ASD 'button' devices in the heart will be reviewed. The i …

Pneumopericardium due to ingestion of button battery.

Mostly ingested button batteries passed through the gastrointestinal tract without any adverse effects. But button battery can lead to hazardous complications including tracheoesophageal fistula (TEF), especially when the battery is impacted in the esophagus. Urgent esophagoscopic removal of the battery is essential in all cases. Once the TEF is identified, conservative management is the initial treatment of choice. Delayed primary repair can be tried if spontaneous closure does not occur. Here in we want to report a rare case of air leak syndrome, pneumo-pericardium secondary to the corrosive

Intrathoracic use of tubed latissimus dorsi musculocutaneous flap for reconstruction of the cervical and thoracic esophagus in a child.

A case report of an intrathoracic application of tubed latissimus dorsi musculocutaneous flap as an island flap for one-stage reconstruction of an extensive and impassable stricture, following corrosive burns of the oropharynx and the cervical and upper half of the thoracic esophagus in a 1-year-old child, with follow-up results of 9 years is presented. Esophagoscopic and cineradiographic studies were carried out at regular intervals to monitor the growth and the patency of the neoesophagus. This was probably the youngest patient for whom the latissimus dorsi musculocutaneous flap has been use

Diagnostic and therapeutic use of the flexible nasendoscope in the management of an impacted upper oesophageal food bolus.

The flexible nasendoscope is now an integral tool in most otorhinolaryngology departments for visualizing the hypopharyngolarynx. A technique has been described using the modified Valsalva manoeuvre to improve visualization of the hypopharynx during flexible nasopharyngoscopy. The authors report an alternative technique for visualizing the upper oesophagus using the flexible nasendoscope, and highlight three cases where there was accurate visualization of an impacted oesophageal food bolus. Subsequently, these patients underwent flexible oesophagoscopy to dislodge the foreign body. The authors

Modified Technique for Difficult Secondary Tracheoesophageal Puncture.

Tracheoesophageal puncture (TEP) with voice prosthesis (VP) placement is commonly used to restore voice in laryngectomy patients. The conventional procedure utilizes a rigid esophagoscope to open and visualize the pharyngeal inlet. However, this approach is challenging in patients with postradiation changes, reduced neck extension, or trismus. Here, we demonstrate a modified technique involving flexible endoscopy and endotracheal tube placement to reestablish the TEP tract in a patient with a challenging anatomic profile. This step-by-step video demonstrates retrieval of a dislodged prosthesis

Simultaneous manometry and electromyography in the pharyngoesophageal segment.

Whether the occlusion between hypopharynx and esophagus is established by anatomical relations and tissue elasticity or by a functional resting tone of the muscle fibers is not quite clear. This study describes simultaneous electromyography (EMG) and electromanometry in the pharyngoesophageal segment to solve this problem and investigate the complex mechanism of deglutition. To register the EMG from the hypopharyngeal constrictor muscle and the upper esophageal sphincter (UES), two copper wire electrode pairs with hooked bare ends were used. These electrodes were introduced into the muscles th

[Esophageal rupture].

Instrumental injuries of the healthy esophageal wall occur frequently due to esophagoscope are observed in the upper parts of the esophagus, have a lineal form and large sizes. Instrumental, frequently by bougie, injuries of the esophagus altered by scars are of a smaller size, round shape and localize in the thoracic part of the esophagus, frequently between two strictures. Foreign bodies in the esophagus produce defects of small sizes, as a rule, in the upper part of the esophagus, and numerous abrasions and hemorrhages in the mucous membrane. Wall ruptures under the influence of the hydraul

The use of a detachable mini-loop for the treatment of esophageal varices.

BACKGROUND: Endoscopic variceal ligation is facilitated by multiband ligating devices, but these have limitations including a fixed number of bands, occasional failure to firmly ligate a variceal column, and relatively high cost. We report the use of a mini-loop for treatment of esophageal varices. METHODS: A detachable nylon ring (mini-loop), maximum diameter 11 mm, passed through the accessory channel of a standard endoscope is opened at the rim of a transparent ligation chamber attached to the instrument. By suction, a varix is brought into the chamber, the mini-loop is maneuvered over the

[Esophageal obstruction in tube feeding: a result of protein precipitation caused by antacids?].

A complication of tube feeding, i.e. thickening of nutrient solution in the presence of an antacid leading to obstruction of the esophagus, is reported. Three patients on artificial breathing where fed by tube with Fresubin liquid. At intervals, Alucol liquid was given for stress ulcer prevention through the tube. After 2, 4, and 50 days respectively, reflux of nutrient solution into the mouth occurred. On removal of the tube a compact, yellowish-white substance was extracted. Endoscopically, residual particles were found in the upper part of the esophagus and in the hypopharynx which obstruct

A swallowed denture leading to misdiagnosis with esophageal neoplasm: a case report.

INTRODUCTION: Foreign body ingestion can lead to esophageal complications, including perforation and impaction, in up to 20% of cases, making it a critical situation. Misdiagnosis or delayed diagnosis can cause severe complications. CASE PRESENTATION: We present the case of a 78-year-old female who swallowed an acrylic partial denture leading to progressive dysphagia and a vegetative ulcerative lesion on endoscopy. The lesion was initially misdiagnosed as a neoplasm of the esophagus. CT scan and a repeat endoscopy revealed the presence of a denture in the esophagus. The denture was successful

[Prevention and treatment of intraoperative complications of thoracic surgery].

In order to achieve a minimal complication rate there is a need for a comprehensive strategy. This means in the first line preventive steps which include patient positioning, suitable approaches and access, an appropriately qualified surgical team as well as a carefully planned dissection and preparation. Furthermore, a supply of additional instrumentation, such as thrombectomy catheters, special vascular clamps and even extracorporeal membrane oxygenation (ECMO) and a heart-lung machine (HLM) in cases of centrally located lesions should be on stand-by. Control instruments, such as a bronchosc
