Delta beta thalassemia and hereditary persistence of fetal hemoglobin.

Delta beta Thalassemia and hereditary persistence of fetal hemoglobin (HPFH) constitute a heterogeneous group of disorders characterized by absent or reduced synthesis of adult hemoglobin (Hb A) and increased synthesis of fetal hemoglobin (Hb F). Coinheritance of these disorders with other beta chain hemoglobinopathies, such as beta thalassemia and the sickle cell (beta s) gene, can result in attenuation of the clinical severity of these hemoglobinopathies owing to the increased Hb F levels. The molecular basis of these disorders is quite heterogeneous and consists of both deletion and nondele…

Prenatal diagnosis of hemoglobin Bart's hydrops fetalis by HPLC analysis of hemoglobin in fetal blood samples.

Since HbF and HbA are not found in fetuses with Hb Bart's hydrops fetalis the feasibility of prenatal diagnosis of homozygous alpha-thalassemia 1 by fetal hemoglobin typing was examined. Blood samples were obtained from fetuses at 18 to 22 weeks of gestation by cordocentesis in 32 pregnant women at risk of having a child with homozygous alpha-thalassemia 1 (alpha-thal-1). The samples were analyzed by a PCR-based method for the diagnosis of alpha-thal-1 (SEA type) and the proportion of hemoglobin fractions were determined by automated HPLC. DNA analysis showed that 8 of the 32 fetuses were homo…

Hemin-induced acceleration of hemoglobin production in immature cultured erythroid cells: preferential enhancement of fetal hemoglobin.

The effects of heme, when added as the ferric chloride salt, hemin, on human erythroid cells grown in a two-phase liquid culture system were studied. When added together with erythropoietin, on initiation of the second phase of the culture, hemin greatly accelerated hemoglobin (Hb) accumulation in these cells. The effect was greater during their early stages of maturation, suggesting that heme availability is then a rate-limiting step for Hb synthesis. Hemin increased preferentially the production of fetal Hb (HbF) compared with adult Hb; this was associated with a selective twofold elevation

Fetal hemoglobin fraction is correlated to the risk of prematurity complications.

BACKGROUND: Fetal hemoglobin (HbF) plays a critical role in the progressive adaptation to the postnatal oxygen-rich environment in preterm infants due to its peculiar properties and its postnatal decrease has been associated to the combined adverse effects of increasing tissue hyperoxia and decreasing antioxidant defenses in preterm infants. PURPOSE: We aimed to assess the association between HbF fractions and the risk of bronchopulmonary dysplasia (BPD) intraventricular hemorrhage (IVH), and retinopathy of prematurity (ROP). METHODS: We studied 166 preterm infants with a gestational age of …

Induction of fetal hemoglobin in the treatment of sickle cell disease.

Reactivation of fetal hemoglobin (HbF) expression is an important therapeutic option in patients with hemoglobin disorders. In sickle cell disease (SCD), an increase in HbF inhibits the polymerization of sickle hemoglobin and the resulting pathophysiology. Hydroxyurea, an inducer of HbF, has already been approved for the treatment of patients with moderate and/or severe SCD. Recent clinical trials with other pharmacological inducers of HbF, such as butyrate and decitabine, have shown considerable promise. In this chapter, we highlight the important clinical trials with pharmacological inducers…

Fetal hemoglobin reactivation in baboon and man: a short perspective.

Present concepts of the mechanism of reactivation of synthesis of fetal hemoglobin (HbF) in the adult under conditions of erythropoietic stress are briefly reviewed. Since HbF can be considered an effective natural antisickling agent, the reactivation of its synthesis in patients with sickle cell anemia as a desirable therapeutic goal has been extensively explored since the discovery in 1982 that 5-azacytidine increases HbF levels in the baboon. Hydroxyurea (HU) has become the most widely used agent, although its effectiveness in increasing HbF levels and the number of F cells is highly variab…

Fetal hemoglobin in women with normal and with hydatidiform molar pregnancy.

The quantity and the rate of synthesis of fetal hemoglobin (HbF); the level of HbF-containing erythrocytes (F cells); and the levels of progesterone, human chorionic gonadotropin (HCG), and human chorionic somatomammotropin (HCS) in serial blood samples from women at different stages of pregnancy were determined. An increase was observed in the synthesis and the quantity of maternal HbF, reaching a peak at about 9-12 weeks gestation. A major peak in F-cell level was also detected at about 9-12 weeks, with two minor peaks appearing at about 16-20 and 22-24 weeks. Levels of HCG, HCS, and progest…

Pregnancy outcomes in women with elevated levels of fetal hemoglobin.

OBJECTIVE: Elevated maternal levels of fetal hemoglobin (HbF) present a unique situation where both mother and fetus produce hemoglobin with equivalent oxygen affinities. We aimed to determine pregnancy outcomes in women with persistently elevated HbF. METHODS: In this retrospective cohort study, women with HbF levels exceeding 10% were identified by searching a provincial database. Maternal, obstetric, and neonatal outcomes were extracted from chart reviews performed at two hospitals. RESULTS: Twenty-two women with a total of 43 pregnancies and 33 live births were identified. Maternal level…

Fetal Cardiac Inflow Characteristics in Response to Fetal Anemia: Based on Fetal Hemoglobin Bart's Disease at Mid-Pregnancy.

OBJECTIVES: To identify the inflow (filling time fraction [FTF] and E/A ratio) characteristics of fetuses with anemia, and to evaluate the performance of the inflow markers in predicting the affected fetuses. METHODS: Fetuses at risk of hemoglobin (Hb) Bart's disease at 17-22 weeks were prospectively recruited to undergo echocardiography before diagnostic cordocentesis. Cardiac Doppler images were digitally stored for off-line blinded measurements of FTF and E/A ratio. RESULTS: A total of 428 fetuses at risk of Hb Bart's disease were analyzed, including 88 affected fetuses (20.6%). The mean

Detection of fetal-maternal hemorrhage in a mother with sickle trait and hereditary persistence of fetal hemoglobin.

An Rh-negative term primigravida with sickle trait and hereditary persistence of fetal hemoglobin and an Rh-positive infant were evaluated postpartum for fetal-maternal hemorrhage (FMH) by the acid-elution test for fetal hemoglobin (HbF) cells. Using the acid-elution test, 100 percent of the cells contained HbF. The patient received one vial of Rh immune globulin and was discharged. This report illustrates that lack of specificity is a major problem of the acid-elution test for detection of fetal-maternal hemorrhage, due to its failure to differentiate true fetal Rh-positive cells from materna…

Epigenetic strategies for fetal hemoglobin induction in sickle cell disease.

Sickle cell disease (SCD) is caused by pathogenic variants in the β-globin gene (HBB), most commonly the variant responsible for hemoglobin S, and affects an estimated 515,000 newborns each year, with the highest burden occurring in sub-Saharan Africa. Gene editing and hematopoietic stem cell transplantation have changed the therapeutic landscape, but their cost, technical complexity, and procedure-related risks still limit their wider use. For this reason, pharmacological induction of fetal hemoglobin (HbF) remains an important therapeutic strategy. HbF reduces HbS polymerization and is assoc…

Ultrasound Features of Fetal Anemia Lessons From Hemoglobin Bart Disease.

Insights gained from this review are as follows: (1) Ultrasound is highly effective in early detection of fetal hemoglobin (Hb) Bart disease. (2) The most sensitive parameters in predicting Hb Bart anemia appear to be the cardiac diameter-to-thoracic diameter ratio, middle cerebral artery peak systolic velocity, and placental thickness. (3) Several other ultrasound markers are helpful in increasing specificity, such as hepatosplenomegaly. (4) Hydrops fetalis is not a consequence of heart failure but rather of hypervolemia and high vascular permeability of fetuses, whereas heart failure is a ve…

Postnatal Prediction of Gestational Age Using Newborn Fetal Hemoglobin Levels.

INTRODUCTION: In many parts of the developing world procurement of antenatal gestational age estimates is not possible, challenging provision of appropriate perinatal care. This study aimed to develop a model for postnatal gestational age estimation utilizing measures of the newborn hemoglobin levels and other metabolic analyte data derived from newborn blood spot samples. METHODS: We conducted a retrospective cohort analysis of 159,215 infants born January 2012-December 2014 in Ontario, Canada. Multivariable linear and logistic regression analyses were used to evaluate the precision of devel

Individualizing fetal hemoglobin augmenting therapy for β-type hemoglobinopathies patients.

Individual genetic composition is an important cause of variations in the response and tolerance to drug treatment. Pharmacogenomics is a mo … ponse to hydroxyurea treatment in β-type hemoglobinopathies patients, deducted from the increase in fetal hemoglobin levels. This article aims to provide an update and to discuss future challenges on the application of pharmacogenomics for β-type hemoglobinopathies therapeutics in relation to the current pharmacological treatment modalities for those disorders.

Stimulation of fetal hemoglobin production by short chain fatty acids.

Butyrate, a four-carbon fatty acid, and its two-carbon metabolic product, acetate, are inducers of gamma-globin synthesis. To test whether other short-chain fatty acids share this property, we first examined whether propionic acid, a three-carbon fatty acid that is not catabolized to acetate, induces gamma-globin expression. Sodium propionate increased the frequency of fetal hemoglobin containing erythroblasts and the gamma/gamma + beta mRNA ratios in adult erythroid cell cultures and F reticulocyte production in a nonanemic juvenile baboon. Short-chain fatty acids containing five (pentanoic),…

Balanced globin chain synthesis in hereditary persistence of fetal hemoglobin.

In two black families with the hereditary persistence of fetal hemoglobin (HPFH) gene there are eight A-F heterozygotes and two double heterozygotes for sickle cell trait and HPFH. These patients are clinically asymptomatic and have homogeneous acid elution smears. Measurement of globin chain synthesis in peripheral blood demonstrates balanced production of a alpha and non-alpha (beta plus gamma) chains. In these patients, the balance is achieved by increased gamma globin production and increased activity of the remaining beta globin allele. In two patients, one A-F and the other S-F there is …

The histone methyltransferase MLL1 complex inhibits expression of fetal hemoglobin.

Increasing fetal-type hemoglobin expression in adult erythroid cells holds promise in the treatment of sickle cell disease (SCD) and β-thalassemia. We have identified the MLL1 complex as a critical regulator of fetal and embryonic hemoglobin repression. Knockdowns of MEN1 and KMT2A, encoding essential components of the complex, caused a significant downregulation of BCL11A expression and a substantial increase in γ- and ε-globin mRNA levels in HUDEP-2 cells. Significant binding of MEN1 and KMT2A was readily detected at the promoter and a critical enhancer of BCL11A in HUDEP-2 cells, suggesting

BCL11A deletions result in fetal hemoglobin persistence and neurodevelopmental alterations.

A transition from fetal hemoglobin (HbF) to adult hemoglobin (HbA) normally occurs within a few months after birth. Increased production of HbF after this period of infancy ameliorates clinical symptoms of the major disorders of adult β-hemoglobin: β-thalassemia and sickle cell disease. The transcription factor BCL11A silences HbF and has been an attractive therapeutic target for increasing HbF levels; however, it is not clear to what extent BCL11A inhibits HbF production or mediates other developmental functions in humans. Here, we identified and characterized 3 patients with rare microdeleti…
