A prospective controlled study of outcome after trauma during pregnancy.

In a prospective cohort study, 85 women who suffered varying degrees of trauma during pregnancy (12 to 41 weeks) were compared with a control group of pregnant women matched for gestational age. Fetomaternal transfusion occurred significantly more frequently in the study group (30.6% vs 8.2%, p less than 0.05). Study subjects whose placentas were anteriorly placed were at increased risk for fetomaternal transfusion on comparison with other placental positions (47% vs 23.5%, p less than 0.05). Immediate adverse outcomes including abruptio placentae occurred frequently in the study group (9.4%) …

Prophylactic vs therapeutic blood transfusions impact on pregnant sickle cell patients.

OBJECTIVE: To observe the fetomaternal outcome of therapeutic versus prophylactic blood transfusions in patients with sickle cell disease (SCD) during pregnancy. METHOD: This single-center retrospective observational study was conducted on consecutive pregnant women with SCD between January 2018 and December 2020. All the pregnant women with SCD were included in this study. Sickler women carrying sickle cell traits (HbAS, HbAC), multiple pregnancy, or other medical diseases were excluded from the study. We used non-random, convenience sampling and collected their demographic details, medical

Spontaneous massive fetomaternal hemorrhage: two case reports and a literature review of placental pathology.

BACKGROUND: Massive fetomaternal hemorrhage (FMH) is a rare event during pregnancy that may cause severe fetal anemia or death. CASE PRESENTATION: This paper reports two cases of fetomaternal hemorrhage with unexplained reasons. Both cases required emergency caesarean sections for non-reassuring fetal status and were treated with neonatal blood transfusion. Fetomaternal hemorrhage was confirmed via maternal Kleihauer-Betke test. CONCLUSION: We found parenchymal pallor, increased nucleated red blood cells (nRBCs), and syncytial knots (SKs) in the placentas, which are compatible with fetal ane

In one case of two massive fetomaternal transfusion the child died. In both cases of fetomaternal transfusion the placenta was located to the anterior wall of the uterus. Therefore we think this location of the placenta is a contraindiction for external cephalic version.

Massive fetomaternal hemorrhage: A case report.

We present a case with an incidental finding of abnormal cardiotocography (CTG) pattern as well as elevated middle cerebral artery peak systolic velocity (MCA-PSV) in an otherwise inconspicuous pregnancy. Massive fetomaternal hemorrhage (FMH) was detected as the cause by flow cytometry (FC), resulting in multiple cycles of fetal blood sampling (FBS) showing severe anemia, intrauterine transfusions (IUTs), a preterm delivery, and a healthy infant in follow-up examinations.

Fetomaternal hemorrhage in monochorionic diamniotic twins at 37+ weeks' gestation: A rare complication.

Fetomaternal hemorrhage refers to a clinical syndrome in which fetal blood enters the maternal circulation through the placental villous spaces, typically triggered by specific stimuli, and may lead to fetal anemia or maternal hemolysis. The occurrence of fetomaternal hemorrhage is very rare in singleton pregnancies and even rarer in multiple gestations. In monochorionic diamniotic twin pregnancies, shared placental circulation can amplify the consequences of fetomaternal hemorrhage, potentially resulting in more severe outcomes than in singleton pregnancies or in twins with double chorions an

[Anti-D prophylaxis after abortions and interruptions].

In almost every pregnancy feto-maternal transfusions occur. From the 6th week of pregnancy, blood containing Rhesus antigens may be infused into the maternal circulation and cause sensitization. In case of miscarriage, abortion, ectopic pregnancy and cystic mole the chance of fetomaternal transfusion followed by sensitization of the mother is significantly increased. Therefore immunoprophylaxis with anti-D-immunoglobins should be performed in all these cases.

Severe neonatal anemia from fetomaternal hemorrhage: report from a multihospital health-care system.

OBJECTIVE: The incidence of fetomaternal hemorrhage that is severe enough to cause neonatal anemia is not known. Owing to its relative rarity, much of the literature describing this condition is in the form of case reports and small case series. We performed a large, muiticentered, sequential, case series to determine the incidence, antecedents and outcomes. STUDY DESIGN: From the multicentered databases of Intermountain Healthcare, we obtained records of all neonates with hematocrit (Hct) <30% or hemoglobin (Hgb) <10 g dl(-1) on the day of birth, who had Kleihauer-Betke staining or flow cyto

Massive fetomaternal hemorrhage and oxytocin contraction test: case report and review.

We present a case of a nulliparous woman who underwent oxytocin contraction test because of abnormal cardiotocograph. She delivered a severely anemic neonate due to severe fetomaternal hemorrhage. Fetal ultrasonography and Doppler studies of the umbilical arteries may not be helpful, while its worthy to perform flow cytometry for detection of fetal cells in maternal circulation when there is strong clinical suspicion. Management of massive fetomaternal hemorrhage requires immediate delivery by Caesarean section if the gestational age is suitable. Alternatively, for very premature fetuses could

Severe Hemolytic Disease of the Fetus Treated with Serial Intrauterine Transfusions: A Case Report and Review of Current Management.

Hemolytic disease of the fetus and newborn (HDFN) is a severe complication of pregnancy caused by maternal alloimmunization to fetal red blood cells, leading to significant perinatal morbidity and mortality. The prognosis is particularly poor in cases complicated by fetal hydrops. Prophylactic administration of anti-D immunoglobulin-during pregnancy, postpartum, and after events causing fetomaternal hemorrhage-has substantially reduced the incidence and severity of Rh-related HDFN. Nevertheless, the condition can still occur, either due to omitted prophylaxis or undetected fetomaternal hemorrh

Fetomaternal hemorrhage in threatened abortion.

In this study, we compared the incidence of fetomaternal hemorrhage between patients with threatened abortion and a control population of similar gestational age. The study population comprised pregnant patients at less than 20 weeks' gestation who presented to our emergency room with a history of vaginal bleeding without cervical dilatation or passage of tissue. The control population consisted of women presenting for elective pregnancy termination; they were excluded from the study if they gave a history of any antepartum bleeding. The amount of fetomaternal hemorrhage was evaluated using th

Fetomaternal hemorrhage: pathogenesis, diagnosis, and clinical management.

Clinically significant fetomaternal hemorrhage (FMH) can have devastating consequences on the newborn infant. Acute FMH close to childbirth can present with hypovolemia, shock, metabolic acidosis, and encephalopathy. Chronic FMH can be associated with congestive heart failure, pulmonary edema, hydrops and hepatomegaly. Early recognition and timely management of FMH are crucial in improving outcomes. This review article summarizes the epidemiology, pathogenesis, diagnosis, management and outcomes of clinically significant FMH. Current knowledge gaps in diagnosis and management of FMH are additi

Intrauterine blood transfusion: current indications and associated risks.

Fetal anemia is a serious complication in pregnancy and associated with perinatal mortality and morbidity. During 25 years of worldwide experience with intravascular intrauterine blood transfusion, a variety of indications have been described. Intrauterine transfusion (IUT) treatment is considered most successful for fetal anemia due to red cell alloimmunization. Moreover, the use of this procedure has also been reported in pregnancies with parvovirus B19 infection, fetomaternal hemorrhage and placental chorioangiomas, for example. This review focuses on the current indications of intrauterine
