[Current status of fibreoptic bronchoscopy in intensive care medicine].

Flexible bronchoscopy (FB) has been of great help in the management of critically ill patients. Its safety and usefulness in the hands of experienced professionals, with the required measures of caution, has resulted in the increasingly widespread use of the technique even in unstable critical patients subjected to mechanical ventilation and with high oxygen demands. The Spanish Society of Intensive and Critical Care Medicine and Coronary Units (SEMICYUC), through its Acute Respiratory Failure (GT-IRA) and Infectious Diseases (GT-EI) Work Groups, aims to promote knowledge and standards of qual

The endoscopic and histological features of peptic duodenitis.

This study attempts to compare the endoscopic features of duodenitis with the histological characteristics. During a 12 month period, 98 patients undergoing fibreoptic oesophago-gastro-duodenoscopy had the endoscopic appearances of the duodenum classified on a three point scale of increasing severity of duodenitis. Multiple duodenal biopsies were then taken and the histological features of the severity of duodenitis graded from 0 to 4 plus by an independent observer. The histological criteria included an assessment of neutrophil infiltration, villous bluting, gastric metaplasia and chronic inf

Fiber-optic intra-aortic balloon therapy and its role within cardiac surgery.

INTRODUCTION: The patient population has changed and the cardiothoracic team are now operating on patients with more co-morbidity. One of the significant aspects of this increased co-morbidity, which affects both short- and long-term outcomes, is compromised left ventricular function. Intra-aortic balloon pump (IABP) technology offers these patients and the cardiac team an easily accessible, cost-effective, mechanical assist device. Arterial pressure monitoring for IABP therapy: Fluid-filled transducers used to measure the aortic waveform can be unreliable and inconsistent. Fiber-optic manomet

Nucleic-Acid-Amplification-Free and Fluorescent-Label-Free RNA Detection with a Dual-Scale Gold Nanoparticle-Enhanced Fiber-Optic SPR Biosensor.

Highly sensitive RNA detection at ultralow concentrations remains challenging for conventional surface plasmon resonance (SPR) biosensors because low-molecularI biomolecules induce only weak effective refractive-index perturbations within the limited evanescent-field penetration depth. Here, we report a dual-scale gold nanoparticle (AuNP)-enhanced fiber-optic SPR biosensor that overcomes this intrinsic limitation through a mechanism-guided sandwich-type architecture. The sensor employs a multimode fiber-single-mode fiber-multimode fiber (MMF-SMF-MMF) configuration to enhance evanescent-field e

Fiber Bragg grating-based three-dimensional human-robot interaction force sensor with high precision and large range for lower limb rehabilitation exoskeleton.

This study presents a three-dimensional (3-D) fiber Bragg grating (FBG) force sensor with high precision and large range, designed to measure 3-D interaction force between human lower limb and lower limb rehabilitation exoskeleton during rehabilitation training. The sensor integrates four FBG-inscribed optical fibers with an elastomer. The FBGs are arranged at 120° intervals circumferentially around the elastomer to measure 3-D force. An additional fiber serves for temperature compensation. The sensitivity and resolution of the sensor are enhanced by introducing a simply supported beam structu

Multimodal Fiber Somatosensory: Optical Fiber 'Nerves' Empower Continuum Surgical Robots With Multimodal Feedback.

To achieve the force sensing at the sub-5 mm end effector of a continuum robot, optical fiber 'nerves' -incorporating the Fabry-Pérot (FP) cavity and step-etched fiber Bragg gratings (FBGs) was proposed to sense the multimodal forces (clamping force, 3-axis wrist force, and driving force) and shape awareness. The proposed laser-welded package solved the problem of clinical application adaptability, enabling the robot to withstand sterilization (15 cycles) and maintain an error of <5% over 60 days of operation. Combined with the in situ calibration method, the surgical robot can maintain high f

Rapid detection of Bacillus anthracis spores directly from powders with an evanescent wave fiber-optic biosensor.

There currently are no rapid, sensitive tests to directly and reliably detect Bacillus anthracis spores in common powders. Traditional culture is time consuming and molecular techniques cannot directly process powders. This study describes a biosensor assay that detects B. anthracis at concentrations of 3.2 x 10(5) spores/mg or higher in spiked powders in less than 1 h with minimal sample preparation.

Antibody immobilization using heterobifunctional crosslinkers.

Covalent attachment of functional proteins to a solid support is important for biosensors. One method employs thiol-terminal silanes and heterobifunctional crosslinkers such as N-succinimidyl 4-maleimidobutyrate (GMBS) to immobilize proteins through amino groups onto glass, silica, silicon or platinum surfaces. In this report, several heterobifunctional crosslinkers are compared to GMBS for their ability to immobilize active antibodies onto glass cover slips at a high density. Antibodies were immobilized at densities of 74-220 ng/cm2 with high levels of specific antigen binding. Carbohydrate-r

Fiberscopic methods for assessment of velopharyngeal closure during various activities.

Fiberscope NPF, developed to observe velopharyngeal movements easily, enables a direct visualization of the soft palate, lateral and posterior pharyngeal walls under the physiological conditions. It is most useful for the diagnosis of velopharyngeal inadequacy. It is very important that a patient himself can observe velopharyngeal movements through the eyepiece of the fiberscope in order to do self exercise.

An Evaluation Method for Partial Discharge in Generator Stator Bar Insulation Based on Fiber-Optic Acoustic Detection.

Partial-discharge (PD) monitoring is essential for assessing the insulation condition of generator stator bars. Conventional methods are susceptible to electromagnetic interference and are difficult to deploy in confined stator geometries. Fiber-optic acoustic detection technology offers strong immunity to electromagnetic interference and is suitable for the narrow and high-interference environment of stator bars, but it cannot directly provide discharge magnitude information. Therefore, in this study, fiber-optic acoustic detection technology was employed to acquire partial discharge acoustic

Dual-resolution megahertz optical coherence tomography prototype rectoscope for enhanced visualization of colorectal microstructures.

SIGNIFICANCE: Endoscopic optical coherence tomography (OCT) is a valuable tool for rectal imaging, enabling noninvasive visualization of transmural structures with near-histological resolution, critical for accurate tumor staging and diagnostic/therapeutic monitoring. However, anatomical variability of the rectum and the resulting changes in tissue-to-probe distance challenge optimal circumferential imaging. AIM: We aim to develop a dual-mode endoscopic OCT system, enabling real-time switching between high-detail and extended-range imaging for improved assessment of rectal wall morphology und

Passively scanned, single-fiber optical coherence tomography probes for gastrointestinal devices.

BACKGROUND/OBJECTIVES: Optical coherence tomography (OCT) uses low coherence interferometry to obtain depth-resolved tissue reflectivity profiles (M-mode) and transverse beam scanning to create images of two-dimensional tissue morphology (B-mode). Endoscopic OCT imaging probes typically employ proximal or distal mechanical beam scanning mechanisms that increase cost, complexity, and size. Here, we demonstrate in the gastrointestinal (GI) tracts of unsedated human patients, that a passive, single-fiber probe can be used to guide device placement, conduct device-tissue physical contact sensing,

Holographic diffusers for endoluminal-scale optical ultrasound imaging.

SIGNIFICANCE: Freehand optical ultrasound (OpUS) is an emerging imaging modality that utilizes arrays of fiber-optic, photoacoustical ultrasound sources, and a single fiber-optic detector to perform pulse-echo ultrasound imaging in real time and at video rate. The freehand OpUS imaging probes presented to date have utilized either flat-cleaved optical fibers to generate an array of small, circular sources, or featured an additional array of eccentric optical waveguides to generate elliptical sources that improve ultrasound directivity. However, the incorporation of waveguides imposed severe re
