Firefighter injuries are not just a fireground problem.

BACKGROUND: Linking firefighter injury reporting to general motion patterns may provide insight into potential injury mechanisms and the development of prevention strategies. OBJECTIVE: To characterize the injuries sustained by members of a large Canadian metropolitan fire department over a 5-year span. METHODS: Data were taken from injury reports filed by career firefighters between 2007 and 2011. Injuries were described by job duty, type, body part affected, and the general motion pattern employed at the time of injury (e.g. lifting). RESULTS: Of the 1311 injuries reported, 64% were categ…

[Health reason for firefighters to leave their job].

The objective of this study was to identify major pathologies among firefighters, being the cause of leaving the job. The study was based on the analysis of temporary work disability and disability certificates, taking account of age and duration of employment in the capacity of a firefighter. The study was performed on a representative sample of 1503 firefighters, employed between 1994 and 1997 in 29 fire stations. During that period, 214 firefighters left their job (40% of them got retired and 38% of persons were given the right to receive disability pension). The analysis revealed the avera…

Cardiovascular disease in US firefighters: a systematic review.

Cardiovascular disease (CVD) is the leading cause of on-duty death among firefighters (45% of on-duty fatalities) and a major cause of morbidity. CVD in the fire service also has adverse public safety implications as well as significant cost impacts on government agencies. Over the last decade, our understanding of CVD among firefighters has significantly improved and provides insight into potential preventive strategies. The physiology of cardiovascular arousal and other changes that occur in association with acute firefighting activities have been well-characterized. However, despite the str…

Hazardous alcohol use and cardiometabolic risk among firefighters.

BACKGROUND: Alcohol misuse is prevalent among firefighters, and associated adverse cardiometabolic health consequences could negatively impact readiness for duty. Mental health conditions may confer additional risk. Therefore, we aimed to determine whether alcohol misuse increases cardiometabolic risk among firefighters and whether mental health conditions modify these relationships. METHODS: Deidentified data from firefighters (N = 2405; 95.8% males, 38 ± 9 years, 29.6 ± 4.6 kg/m2) included demographics, Alcohol Use Disorders Identification Test (AUDIT) and AUDIT-C scores, mental health scre…

Adapting the Primary Care PTSD Screener for firefighters.

BACKGROUND: By the nature of their work, first responders are at risk for post-traumatic stress disorder (PTSD). Efficient screening instruments are useful to identify at-risk first responders and connect them to services. AIMS: The current study aimed to (i) evaluate the diagnostic properties of the Primary Care PTSD for DSM-5 (PC-PTSD-5) scale among firefighters, (ii) explore the use of an adapted PC-PTSD-5 on a five-point Likert-type scale and (iii) examine sensitivity and specificity of the adapted instrument in this population. METHODS: Pooled data were analysed among firefighters (N = …

Risk Factors for Cardiometabolic Disease in Professional Firefighters.

OBJECTIVE: Firefighters are plagued with cardiometabolic disease (CMD). Obesity, poor cardiorespiratory and muscular fitness, and blood lipids (low-density lipoprotein cholesterol, triglycerides, low high-density lipoprotein cholesterol) are risk factors for CMD. However, markers of oxidative stress, inflammation, and insulin resistance can provide further insight regarding CMD risk. METHODS: This study investigated the relationships between fitness metrics (cardiorespiratory and muscular fitness, percent body fat, waist circumference), blood lipids, blood pressure, and years of experience as…

Coccidioidomycosis outbreak among inmate wildland firefighters: California, 2017.

BACKGROUND: In California, state prison inmates are employed to fight wildfires, which involves performing soil-disrupting work. Wildfires have become more common, including areas where Coccidioides, the soil-dwelling fungus that causes coccidioidomycosis, proliferates. However, work practices that place wildland firefighters at risk for coccidioidomycosis have not been investigated. METHODS: On August 17, 2017, the California Department of Public Health was notified of a cluster of coccidioidomycosis cases among Wildfire A inmate wildland firefighters. We collected data through medical recor…

Prevalence and implications of gambling problems among firefighters.

Firefighting is a high-risk occupation that accounts for vulnerability to a range of mental health problems and addictive behaviours. However, no research has addressed whether this vulnerability extends to gambling problems, and the aim of this study was thus to provide new data on frequency and implications of such problems in this occupational context. The sample consisted of n = 566 career and retained firefighters who participated in a cross-sectional survey of an Australian metropolitan fire service. The Problem Gambling Severity Index (PGSI) was used to operationalise both clinically si…

Incident experience predicts freezing-like responses in firefighters.

Freezing is a defensive response to acute stress that is associated with coping and alterations in attentional processing. However, it remains unclear whether individuals in high risk professions, who are skilled at making rapid decisions in emergency situations, show altered threat-induced freezing. Here we investigated the effect of incident experience in a high risk profession on freezing. Additionally, we explored whether any effect of incident experience on freezing would be different for profession-related and -unrelated threat. Forty experienced and inexperienced firefighters were prese…

Long-term psychological distress among chemically exposed firefighters.

Long-term psychological effects of exposure to a dangerous chemical were evaluated in a sample of firefighters who fought a large polyvinyl chloride (PVC) fire. When compared with nonexposed firefighter controls (n = 22), the exposed firefighters (n = 64) had significantly higher levels of demoralization and specific emotional distress 22 months after the incident. Longitudinal analyses of a subsample of exposed firefighter subjects (n = 55) surveyed 5 to 6 weeks after the fire and again 22 months later revealed that there was no reduction in symptomatology over time. Some psychological distre…

Effect of heat on firefighters' work performance and physiology.

Wildland firefighters often perform their duties under both hot and mild ambient temperatures. However, the direct impact of different ambient temperatures on firefighters' work performance has not been quantified. This study compared firefighters' work performance and physiology during simulated wildland firefighting work in hot (HOT; 32°C, 43% RH) and temperate (CON; 19°C, 56% RH) conditions. Firefighters (n=38), matched and allocated to either the CON (n=18) or HOT (n=20) condition, performed simulated self-paced wildland fire suppression tasks (e.g., hose rolling/dragging, raking) in firef…

Exposure of firefighters to particulates and polycyclic aromatic hydrocarbons.

Firefighting continues to be among the most hazardous yet least studied occupations in terms of exposures and their relationship to occupational disease. Exposures are complex, involving mixtures of particles and chemicals such as polycyclic aromatic hydrocarbons (PAHs). Adverse health effects associated with these agents include elevated incidences of coronary heart disease and several cancers. PAHs have been detected at fire scenes, and in the firehouse rest area and kitchen, routinely adjoining the truck bay, and where firefighters spend a major part of each shift. An academic-community par…
