Domestic resources for surgical care financing: a descriptive analysis of global health expenditure trends and illustrative case studies on fiscal mechanisms in LMICs.

INTRODUCTION: Surgical care is essential to achieving universal health coverage (UHC) and addressing 30% of the global disease burden, including non-communicable diseases (NCDs), maternal health and injuries. However, low and middle-income countries (LMICs) face significant challenges providing equitable access to surgical services, with 5 billion people lacking access and US$20 trillion in associated global economic losses, emphasising the need for sustainable domestic financing strategies. METHODS: A multifaceted approach combined quantitative analysis of domestic health spending trends (20

Unlocking fiscal space for reproductive health in Nigeria: A rapid assessment.

Nigeria continues to face a persistent reproductive health (RH) financing gap despite decades of policy commitments. This rapid fiscal space analysis evaluates how the country can expand investment in RH services while maintaining macro-fiscal stability. Six domains were examined at the federal level: budget reprioritization, improvements in health sector efficiency, revenue enhancement, official development assistance, debt restructuring, and innovative financing mechanisms. The findings indicate that reallocating public expenditures, reducing inefficiencies, and introducing modest tax reform

The economics of hope: valuing the long-term fiscal returns from assisted reproduction.

In society, parents bear most costs of raising children, while the majority of economic benefits accrue to society and governments. From this perspective, parenting can be viewed as a public service from which society obtains financial benefits from the future productive output of children. Understanding where benefits accrue from future output attributed to children born today and in the future is important for establishing how to value live births attributable to assisted reproduction. Low fertility, driven by delayed childbearing, rising opportunity costs and reproductive ageing, currently

Forecasting Ontario Oncology Drug Expenditures: A Hybrid Approach to Improving Accuracy.

BACKGROUND: The Provincial Drug Reimbursement Program (PDRP) at Cancer Care Ontario (CCO) is responsible for monitoring actual and projected outpatient intravenous cancer drug spending in the province. We developed a hybrid forecasting approach combining automated time-series forecasting with expert-customizable input. OBJECTIVE: Our objectives were to provide a flexible tool in which to incorporate multiple forecasts and to improve the accuracy of the resulting forecast. METHODS: The approach employed linear and non-linear time-series techniques and a combined hybrid model incorporating bot

Menu Changes Under NYC's Revised Food Standards Were Associated With A Reduction In Greenhouse Gas Emissions.

Local policy makers increasingly have implemented nutrition standards for municipal programs to advance population health and climate change goals. Yet little is known about the impact of these policies. In 2008, New York City established nutrition standards for food purchased and served by city agencies, and in 2022, it revised the standards to limit meat and increase plant-based options. Using menu data from four agencies serving 77 percent of all city meals, we examined changes in their entrée offerings, as well as greenhouse gas and nutrition content associated with their total menu offeri

The Illusion of Inaction: Lessons from Medicare Physician Payment Reform.

In July 2026, the Centers for Medicare & Medicaid Services released its 2027 Medicare Physician Fee Schedule proposed rule, once again placing physician payment at the center of a familiar policy debate. Despite broad recognition that the current Medicare physician payment system is unstable, meaningful reform has repeatedly failed to materialize. Drawing upon my experience as a Robert Wood Johnson Foundation Health Policy Fellow serving on the U.S. Senate Finance Committee, this perspective examines the disconnect between widespread agreement about the need for reform and the political, fisca

Evaluation of the Fiscal Costs and Consequences of Alzheimer's Disease in Germany: Microsimulation of Patients' and Caregivers' Pathways.

BACKGROUND: Alzheimer's disease is a severe condition, impacting individual's wellbeing and independence in daily activities. Informal care provision is common and of great value to societies but is not without negative externalities to households and the broader economy. OBJECTIVES: Estimate the lifetime incremental fiscal consequences of Alzheimer's disease in community-based individuals and their informal caregivers. SETTING: The fiscal consequences of Alzheimer's disease was modeled using the German government and social security perspective. PARTICIPANTS: Synthetic cohort containing 1,

Long-term impact of a sugar-sweetened beverage tax and increased physical activity on obesity and diabetes burden in Central and Eastern European Countries: a microsimulation model.

AIMS: This study models the potential health impacts of combining fiscal and behavioral policies, specifically, sugar-sweetened beverage (SSB) taxation and increases in physical activity (PA), across six Central and Eastern European (CEE) countries. METHODS: We simulated the effects of a 20 % SSB tax combined with a 15 % increase in PA on obesity, type 2 diabetes mellitus (T2DM), and mortality over 30 years in Bulgaria, Czechia, Hungary, Poland, Romania, and Slovakia. Sensitivity analyses examined alternative scenarios, including SSB substitution with water, a 30 % fruit and vegetable subsidy

Lower taxes at all costs? Evidence from a survey experiment in four European countries.

It is commonly argued that citizens favour lower taxes, thereby exerting pressure for tax reductions that undermine the ability of governments to raise revenues. We argue that the ostensibly strong support for lower taxes is the result of survey measures that fail to account for fiscal trade-offs. An original survey experiment conducted in Germany, Italy, Spain, and the United Kingdom reveals that support for lower taxes declines significantly when this comes into conflict with other fiscal policy objectives, such as government spending, public debt, or other taxes. Overall, regressive changes…

The Fiscal Impact of the Medicare Secondary Payer Act for ESRD.

KEY POINTS: The Medicare Secondary Payer Act modestly reduced federal dialysis spending when accounting for the effect on forgone tax revenue. The policy saved money only when private prices were <3.05 times Medicare rates. Medicare assuming primary payer status for all dialysis care would have saved the government $1 billion annually. BACKGROUND: The Medicare Secondary Payer Act (MSPA) requires that employer group health plans serve as the primary payer for individuals with ESRD for 30 months, with Medicare serving as the secondary payer. After 30 months, Medicare becomes the primary payer.

Comparison of anti-infective drug use in elderly persons in Manitoba, Nova Scotia, and Saskatchewan, Canada: relationship to drug insurance reimbursement policies.

BACKGROUND: Antimicrobial drug resistance continues to be a concern. Inappropriate use of antimicrobial agents is a well-documented contributory factor in the development of resistance. Canadian publicly funded drug insurance (pharmacare) programs have various approaches to reimbursement for antimicrobial drugs and promoting the appropriate prescribing of these agents. OBJECTIVE: The objective of this study was to examine changes in antimicrobial use over a 3-year period in relation to the reimbursement policies of the public drug insurance programs for elderly persons in Manitoba, Nova Scoti

Best practices for implementing the WHO Model List of Essential Cancer Medicines: Chile, Kenya, and Thailand.

BACKGROUND: Cancer is a leading global cause of death, with the burden rising fastest in low- and middle-income countries (LMICs). The WHO Model List of Essential Medicines for Cancer (EML-C) provides an evidence-based benchmark for prioritizing safe and cost-effective therapies for reaching Universal Health Coverage (UHC). However, only 25% of countries have a national cancer EML-C, and fewer than 10% adequately fund it. OBJECTIVE: This study aims to identify factors determining successful implementation beyond income levels. METHODS: We used a comparative case-study design of Chile, Kenya,

An enabler or a barrier: implications of local government autonomy for effective primary health care (PHC) reforms in Nigeria.

INTRODUCTION: in July 2024, Nigeria´s Supreme Court granted autonomy, including financial, to local government, a decision with major implications for primary health care (PHC). Although local governments are constitutionally responsible for PHC, weak fiscal independence and administrative capacity have hindered performance. This study examines opportunities and challenges that LGA autonomy poses for PHC reforms, particularly within the Primary Health Care Under One Roof (PHCUOR) policy. METHODS: a qualitative exploratory policy analysis was conducted using key informant interviews (n=288) an

Reallocating fossil subsidies via social assistance reduces poverty and preserves emissions cuts.

Fossil fuel subsidies are widespread, regressive, and environmentally costly, yet abrupt removal can raise poverty. To identify a practical reform pathway, we evaluate the effectiveness of compensatory policies grounded in existing national social protection systems. We develop a high-resolution global social accounting matrix to quantify how reallocating subsidies through actual social assistance impacts poverty, inequality, and CO2 emissions. Without compensation, reform lowers emissions but increases poverty, especially for low-income households in emerging economies. Redirecting funds via

The evolving market structures of gambling: case studies modelling the socioeconomic assignment of gaming machines in Melbourne and Sydney, Australia.

The expansion of gambling industries worldwide is intertwined with the growing government dependence on gambling revenue for fiscal assignments. In Australia, electronic gaming machines (EGMs) have dominated recent gambling industry growth. As EGMs have proliferated, growing recognition has emerged that EGM distribution closely reflects levels of socioeconomic disadvantage. More machines are located in less advantaged regions. This paper analyses time-series socioeconomic distributions of EGMs in Melbourne, Australia, an immature EGM market, and then compares the findings with the mature marke

Subsidy Programs for Herpes Zoster Vaccination in Japanese Municipalities.

As of 2024, herpes zoster (HZ) vaccination for older adults was not included in Japan's routine vaccination program. Two types of HZ vaccines are currently available at the individual's expense. In this study, we surveyed program details for HZ vaccination in Japanese municipalities. We further explored the subsidy amount and the percentage of newly vaccinated individuals among target populations who were vaccinated by local governments under the subsidy. In June 2023, we mailed the questionnaire to 56 municipalities about HZ vaccine subsidy programs in fiscal year 2022. Information on the det

Differentiated regulation and health spending growth: European evidence from the new SCORE index.

INTRODUCTION: Rising healthcare expenditure poses a major fiscal challenge for European governments, driven in part by preventable diseases associated with tobacco use, alcohol consumption, and sugar-sweetened beverages (SSBs). Although regulatory interventions are widely used to address these risk factors, most evaluations focus on overall policy stringency and implicitly assume homogeneity within product categories. This overlooks the potential importance of risk-proportional regulatory designs that differentiate products according to their relative health risks. METHODS: This study investi

A network analysis of mental, somatic health, and perceived social supports among Chinese pregnant and postpartum women.

BACKGROUND: The mental burdens among pregnant and postpartum women were exacerbated by cultural expectations and policy shifts, which can be mitigated by social support. OBJECTIVE: To identify the networks of comorbid somatic and mental health, diverse sources of support (interpersonal and policy), and symptom-support interactions in pregnant and postpartum women. METHODS: Participants were recruited from seven Chinese tertiary hospitals. Health conditions, supports, and combined system networks were estimated. Core symptoms and support sources were identified via centrality indices. RESULT
