OBJECTIVE: To study the treatment and therapeutic effects in the shearing-type and comminuted talar body fractures.
METHODS: From October 1988 to September 2005,34 patients with shearing-type or comminuted talar body fractures were followed up. There were 19 males and 15 females ranged from 13 to 55 years (averaged 28.8 years). The disease course ranged from 3 to 14 days (averaged 6.0 days). Ten patients with a displacement of no more than 3 mm were treated with plastic cast. Eighteen patients were treated with open reduction and internal fixation, 6 patients were treated with joint fusion. T

Some comminuted or segmental humeral fractures have been well-treated by functional bracing, but some also have been reported to have a bad outcome. If surgery is required, unlocked nailing has the disadvantage of unstable fixation, and plating has the disadvantage of extensive soft tissue injury. In the current study, closed locked nailing was used to treat 23 Orthopaedic Trauma Association Type C humeral fractures (C1, five; C2, three; C3, 15). The patients were 12 men and 11 women with a mean age of 42.4 years. There were 16 closed fractures and four Type I, two Type II, and one Type IIIA o

In 42 patients, massive haemorrhage following comminuted pelvic fractures was treated by operation through a mid-line, Pfannenstiel, anterior iliac crest-femoral or posterolateral approach, or using these in combination. The superior and inferior gluteal, pudendal and obturator arteries were the usual source of bleeding. Bleeding from the pelvic bone tissue was of minor importance. Temporary compression of the abdominal aorta was sometimes required to help control bleeding before ligation of injured vessels could be accomplished. An average of nine units of blood was given to each patient befo

PURPOSE: In the setting of severely comminuted diaphyseal-metaphyseal fractures of the distal radius, arthroscopic reduction of the joint surface is difficult and often results in shortening and collapse. Yet, several authors have shown the benefits of arthroscopy in articular distal radius fractures. We present a method that safely allows a combination of arthroscopic reduction and rigid fixation and describe the outcomes in a small group of patients.
METHODS: Four consecutive patients with severely comminuted diaphyseal-metaphyseal articular fractures of the distal radius were treated using

Fracture healing in dogs is a complex process influenced by factors such as age, weight, fracture type, and underlying conditions. Among fractures, complex diaphyseal comminuted fractures stand out due to their susceptibility to complications like delayed union or nonunion. Despite the recognized complexities, veterinary-specific data on their incidence, complications, and effective treatment strategies remain surprisingly scarce. This retrospective study analyzed 99 comminuted fractures among 542 long bone fractures treated at the Small Animal Clinic of the Freie Universität Berlin (2007-2014

BACKGROUND: In recent years, the medical community has witnessed a notable increase in high-energy traumatic injuries, leading to a surge in complex fracture patterns that challenge existing treatment methodologies. Among these, the posterior approach to acetabular fractures stands out for offering direct visualization of the retro-acetabular surface, with current fixation methods relying on 3.5 mm low-profile reconstruction plates and various other implants. Despite the effectiveness of these methods, there is a burgeoning demand for a singular, adaptable implant that not only streamlines the

OBJECTIVE: Isolated and noncomminuted mandibular angle fractures are commonly fixed with a single miniplate on the superior border (Champy technique) through an intraoral approach. However, the use of this technique for comminuted mandibular angle fractures has not been reported yet. This article verified that the Champy technique can also be used to treat comminuted mandibular angle fractures.
METHODS: The authors compared the therapeutic effects of using the Champy technique for intraoral fixation of comminuted mandibular angle fractures with or without alveolar bone defect. The dislocated

Radial head arthroplasty is indicated for displaced comminuted radial head fractures that cannot be managed reliably with open reduction and internal fixation and that have an associated elbow dislocation. Replacement also is indicated in patients with comminuted radial head fractures that have or are likely to have a disruption of the medial col-lateral, lateral collateral, or interosseous ligaments. Biomechanical studies have demonstrated that metallic implants restore elbow stability similar to the native radial head. The early and midterm clinical experience with metallic radial head arthr

Four women aged 87, 77, 74 and 69 years with severely comminuted, displaced and intraarticular Colles' fractures of the dominant hand, where conservative treatment had failed, were treated by use of intramedullary methylmethacrylate stabilisation in order to retain optimal anatomical position of the fracture. The patients were followed by serial clinical, radiological and 99Technetium-scintimetrical examinations for at least 1 year. Normal mobility of the wrist and power of the grip was obtained within 6 weeks postsurgery in all patients. Radiological cortical bone healing occurred within norm

The comminuted fracture of a metatarsal shaft provides a greater challenge to clinicians in relationship to stabilization and and fixation then does the simple noncomminuted type of fracture. Comminuted fractures may result in gross displacement or fragmentation of the bony segments calling for more dramatic methods of fixation to insure proper union with optimal bone healing and minimal postfrature complications. Adequate approximation of fractured bone segments is essential to insure proper restoration of function and stability to fractured bones. Open reduction with circular wire fixation i

A nasogastric tube is commonly used as a method of enteral feeding or gastric decompression in clinical practice and its insertion is occasionally associated with local complications. In this case report, we present an extremely rare complication of a comminuted nasogastric tube fracture in a 54-year-old male patient receiving enteral feeding in hospital secondary to a diagnosis of haemophagocytic lymphohistiocytosis.

A 42-year-old male police officer sustained multiple injuries in a motorcycle accident, including a comminuted fracture of the scapula and several rib fractures. Four months after his injury, the patient was prescribed outpatient physical therapy, after 2 months of which the patient's cervical and lumbar spine function were restored but his shoulder mobility deficits persisted. A second physical therapy opinion was sought 6 months post injury for the left shoulder. Given his functional deficits, including an inability to return to work, he was referred to an orthopaedist for additional imaging

The most widely accepted treatment for comminuted fractures of the radial head is either the excision or open reduction and internal fixation. The purpose of the present study is to evaluate the value of an 'on-table' reconstruction technique in severely comminuted fractures of the radial head. In this study, two patients with a Mason type-III and four patients with a Mason type-IV radial-head fracture were treated with 'on-table' reconstruction and fixation using low-profile mini-plates. After a mean follow-up of 112 months (47-154 months), the mean elbow motion was 0-6-141 degrees extension

OBJECTIVE: To report and assess the efficacy of a coupled exofixator in the treatment of comminuted fracture of the humeral shaft.
METHODS: From June 1999 to September 2003, 24 patients with comminuted fracture of the humeral shaft were treated in our department, in whom 11 were involved in left humerus fractures and 13 in right humerus fractures. Closed reduction or open reduction with a small incision as well as a coupled exofixator was used to treat these patients.
RESULTS: All cases got anatomical reduction after 6-12 months follow-up. The time for fracture union averaged 5.8 months with

Treatment of comminuted avulsion fractures of the inferior pole of the patella is still an unsolved problem. Different methods of treatment have been introduced, some quite controversial, including immediate total patellectomy; a variety of partial patellectomies, and fixation by tension band wiring. Yet the problem of early development of arthrosis and a certain degree of limitation of motion due to the changed biomechanics, retropatellar incongruity and postoperative immobilization must still be resolved. We report two cases with comminuted avulsion fractures of the inferior pole of the pate

Surgical treatment is difficult in elderly patients who have suffered a comminuted fracture of the head of the humerus, often associated with dislocation. We performed follow-up examinations in our hospital in 11 patients after implantation of 12 endoprostheses since July 1990. Very good results were confirmed in respect of regained freedom of movement and overall high personal satisfaction on the part of the patients. Endoprosthesis appears to us as the treatment of choice in complex fractures of the head of the humerus in patients of advanced age.
