Frontal sinus osteoma.

Osteoma is the most common benign tumor of the nose and paranasal sinuses, and the frontal sinus is its most frequent location. This tumor may be discovered incidentally on radiographs or may enlarge to produce symptoms and, rarely, complications referable to its location near the orbit and anterior cranial vault. A series of 22 cases of frontal sinus osteoma treated at the UCLA Center for the Health Sciences is reviewed. The presenting symptoms of this tumor, patient examination, radiographic evaluation, diagnosis, and indications for surgical and nonsurgical management are discussed. Various…

Surgery of the frontal recess and frontal sinus.

Surgery on the frontal recess and frontal sinus remains a challenge for endoscopic sinus surgeons. This paper examines the philosophy behind such surgery and presents a technqiue for 3-dimensional reconstruction of the anatomy of the frontal recess and frontal sinus. Utilizing this anatomical reconstruction the surgeon is encouraged to develop a pre-operative step-by-step surgical plan for each cell identified within the frontal recess and frontal sinus and thereby predict the anatomy of this region before surgery is undertaken. An increased understanding of the anatomy of this region should i…

Mini-endoscopy of the frontal sinus to guide endonasal frontal sinus surgery.

Endoscopic frontal sinus surgery requires special attention to the numerous anatomical variations. Additionally, in situations of revision surgery, endonasal frontal sinus identification may be challenging. Numerous strategies have been described to access the frontal sinus in endonasal endoscopic frontal sinus surgery including an additional external approach. A pilot study is presented using a newly designed modular mini-endoscope (1.1 mm, 10.000 pixel, 2 working channels and 120° lens) to assist the frontal sinus surgery through a transcutaneous puncture of the frontal sinus. We used this m…

Preservation of natural frontal sinus outflow in the management of frontal sinus osteomas.

INTRODUCTION: Management of frontal sinus tumors has traditionally been through open approaches with obliteration of the sinus. Recently, increased comfort with endoscopic techniques has made endoscopic resection an alternative to more morbid approaches. However, many skilled endoscopists still champion obliteration of the frontal sinus after the open treatment of large osteomas. METHODS: A retrospective review of the senior author's experience with frontal sinus osteomas was performed. RESULTS: Twelve frontal sinus osteomas were treated surgically without obliteration. All patients with gre…

Frontal sinus cells: identification, prevalence, and association with frontal sinus mucosal thickening.

BACKGROUND: Frontal sinus cells (FSCs) are thought to be a potential cause of narrowing of frontal recess outflow. It remains unclear if FSCs are associated with frontal sinus mucosal thickening and chronic rhinosinusitis. The goal of the current study is to determine the prevalence of FSCs and their association with frontal sinus mucosal thickening. METHODS: All adult patients undergoing computed tomography (CT) scans of the paranasal sinuses at our institution between February and October 2010 were reviewed. All CT scans were evaluated for the presence of FSC (types 1-4) and association wit…

Frontal bone defect with frontal sinus mucopyocele.

For this patient's treatment, all three consultants advise against the Lynch-type frontoethmoidectomy procedure, with or without mucoperiosteal flap reconstruction of the nasofrontal duct. Treatment plan: Culture and sensitivity of pus; 2-3 weeks of intravenous antibiotics followed by osteoplastic flap fat obliteration of frontal sinus; delayed defect repair with methyl methacrylate (Montgomery). Trephination followed by treatment with local and systemic antibiotics (Donald); removal of infected bone and soft tissue (sinus collapse) and delayed defect repair in 6-12 months (Donald, Calcaterra)…
