[Successful treatment of diabetic gustatory hyperhidrosis with topical glycopyrrolate].

Gustatory hyperhidrosis is a condition characterised by excessive craniofacial sweating in relation to food intake and is associated with diabetic neuropathy. The existing guidelines for treatment of this condition include antiperspirants, oral anticholinergic treatment, ionophoresis, botulinum toxin injections and endothoracic surgery. In this case a patient with diabetes suffering from gustatory hyperhidrosis was treated successfully with topical glycopyrrolate, an anticholinergic agent applied directly on the affected area.

Glycopyrrolate methobromide: 2. comparison with atropine sulphate in anaesthesia.

A double blind study of 98 patients was undertaken to compare the clinical usefulness of atropine and glycopyrrolate for pre-anaesthetic medication and their relative effectiveness in antagonizing the muscarinic effects of neostigmine methylsulphate given to reverse neuromuscular blockade. Intramuscular administration of atropine 0.4 mg or of glycopyrrolate 0.2 mg produced the same degree of dryness of the pharynx. However, glycopyrrolate was found to be a more potent antagonizer of the increased salivation induced by neostigmine. By intramuscular administration, both drugs produced the same d…

Glycopyrrolate and Post-Operative Urinary Retention: A Narrative Review.

The use of non-depolarizing neuromuscular blockade (NDNMB) necessitates the use of reversal agents. Glycopyrrolate, an anticholinergic agent, is commonly used in combination with neostigmine, an anticholinesterase, for the reversal of neuromuscular blockade medications. Glycopyrrolate is known to effect on the genitourinary system adversely with an inhibitory effect on bladder contraction, bladder hypotonia, and increase in the frequency of urinary retention. Many studies analyzing the association between glycopyrrolate and urinary retention are outdated and published over a decade ago. The de…

[The effect of glycopyrrolate on intestinal spasm and hemodynamics in painless colonoscopy].

Objective: To investigate the effects of glycopyrrolate on intestinal spasm and hemodynamics in painless colonoscopy. Methods: A total of 100 patients who were scheduled to undergo painless colonoscopy were selected as the study subjects and randomly divided into two groups by a computerized number method. Ten patients in both groups dropped out because of disruption of the study protocol, and 45 patients from each group were included in the final analysis. Before anesthesia induction, patients in group glycopyrrolate (group G) were injected with 0.2 mg glycopyrrolate, while those in congtrol …

Glycopyrrolate and the Management of "Death Rattle" in Patients with Myasthenia Gravis.

Death rattle commonly occurs at the end of life and is typically managed with anticholinergic agents. Myasthenia gravis is an autoimmune disorder characterized by fatigability of skeletal muscle, resulting from autoimmune destruction of acetylcholine receptors at the motor endplate. The condition is treated with acetylcholinesterase inhibitors, which potentiate the action of acetylcholine. Agents that antagonize acetylcholine activity (e.g., anticholinergic agents, such as glycopyrrolate) can, therefore, exacerbate myasthenia gravis. We discuss the case of a patient dying with myasthenia gravi…

Oral glycopyrrolate as second-line treatment for primary pediatric hyperhidrosis.

BACKGROUND: Primary focal hyperhidrosis not uncommonly begins during the first two decades of life, and can have a profound effect on quality of life. Few treatment options have been studied in children. OBJECTIVE: We sought to evaluate the response to oral glycopyrrolate in pediatric patients. METHODS: Records of pediatric patients with hyperhidrosis seen at a pediatric hospital in a 10-year period were reviewed retrospectively and, if possible, parents and patients were also interviewed. The efficacy and adverse effects of oral glycopyrrolate were assessed. RESULTS: In all, 31 children to…

Modification of cardiopulmonary and intestinal motility effects of xylazine with glycopyrrolate in horses.

Xylazine (XYL) administration in horses is accompanied by significant cardiovascular depression characterized by a 25-35% decrease in cardiac output (CO) which is likely to compromise tissue oxygen delivery (DO2), and usually vagally mediated bradycardia is an important cause of this reduced cardiovascular performance. To examine the possible benefit of preventing the bradycardiac response, 6 healthy horses were treated with intravenous (IV) saline (SAL) or 2.5 micrograms/kg glycopyrrolate (GLY) in a blinded, randomized, crossover trial. Fifteen minutes later, 1 mg/kg XYL was administered IV a…

Atropine and glycopyrrolate: hemodynamic effects and placental transfer in the pregnant ewe.

Atropine administration in pregnant women may cause fetal tachycardia and a decrease in beat-to-beat variability of the fetal heart rate. In the present study, placental transfer and maternal and fetal hemodynamic effects of atropine and glycopyrrolate were compared in chronically and in acutely instrumented, unanesthetized pregnant ewes. Administration of either atropine (0.05 mg/kg) or glycopyrrolate (0.025 mg/kg) increased maternal heart rate by 25% without changing maternal arterial pressure, fetal arterial pressure, fetal heart rate, or beat-to-beat variability. Maternal and fetal blood g…

Comparison of atropine and glycopyrrolate in a mixture with pyridostigmine for the antagonism of neuromuscular block.

Neuromuscular block was antagonized using pyridostigmine 250 micrograms kg-1 in two groups of 50 patients; one group received atropine 20 micrograms kg-1 and the other glycopyrrolate 10 micrograms kg-1 with the anticholinesterase drug. Atropine was associated with a greater initial tachycardia than was glycopyrrolate. The subsequent bradycardia was also greater in this group, although the decreases in heart rate were smaller than those generally observed following mixtures of atropine and neostigmine. Arrhythmias were transient and required no treatment in either group. Better control of secre…

Combination of Intravenous Ondansetron and Intravenous Glycopyrrolate Versus Intravenous Ondansetron and Intramuscular Glycopyrrolate for Reduction of Post-spinal Hypotension in Cesaraen Section: A Randomized Double-Blinded Study.

BACKGROUND: In parturients, post-spinal hypotension is common due to loss of sympathetic tone. Compression of the inferior vena cava by the gravid uterus further aggravates it. Various pharmacologic and non-pharmacologic techniques are used to reduce the severity of hypotension. Ondansetron, a selective 5-hydroxytryptamine receptor antagonist, nullifies the Bezold Jarisch reflex response to spinal anaesthesia and glycopyrrolate, an anti-cholinergic, has been used to prevent post-spinal hypotension. We estimated the efficacy of intravenous (IV) glycopyrrolate plus IV ondansetron versus intramus…

Glycopyrrolate and theophylline for the treatment of severe pallid breath-holding spells.

Severe pallid breath-holding spells (BHSs) are based on parasympathetic hyperactivity, leading to cardiac asystole, pallor, brain ischemia, loss of consciousness, and reflex anoxic seizures. In recent years, an increasing number of patients with severe pallid BHSs have been successfully treated with pacemaker implantation. We present the case of a 13-month-old girl suffering from repeated severe pallid BHSs, causing asystole, loss of consciousness, and generalized anoxic seizures. She underwent treatment with oral glycopyrrolate, an anticholinergic drug, and an oral retard preparation of theop…

Adjusting oral glycopyrrolate medication for hyperhidrosis to reflect seasonal temperature variations.

The condition of most patients with hyperhidrosis (HH) is known to worsen with increased temperature. However, most prior studies of oral glycopyrrolate (OGly) for the treatment of HH have assumed a stable treatment protocol, without taking into account seasonal variations in temperature. The main aim of this study is to evaluate the outcomes derived from performing a seasonal adjustment of the dose of OGly for patients with HH. A prospective study of patients who began OGly for HH, and maintained treatment for at least 1 year, was performed. All patients had experienced treatment failure with…
