Gout nodulosis: widespread subcutaneous deposits without gout.

Subcutaneous tophaceous deposits of monosodium urate, in the absence of arthritis, may occasionally occur as the initial manifestation of gout. In this report, we describe a 35-year-old man who presented with a 6-year history of multiple subcutaneous nodules and no history of previous articular complaints. Needle aspirations of the nodules proved them to be deposits of monosodium urate. A literature search revealed 28 other cases with a similar presentation. We propose the term "gout nodulosis" as a clinical entity at one end of the spectrum of gout to describe this group of patients.

[Gout, pseudo-gout or hyperlipoproteinemia? (author's transl)].

A number of patients with aseptic necroses of bone and hyperlipoproteinemia of types II and IV after Fredrickson are reported on. More than one half of them had hyperuricemia without symptoms of gout. The relationship between fat-, purine- and carbohydrate-metabolism are discussed. The clinical pictures of hyperuricemia, gout and hyperlipoproteinemia are shown to resemble each other. Differential diagnosis, particularly as related to aseptic necroses of bone, is discussed in detail.

Gout Knowledge: A Survey of Australian Outpatients with Gout.

BACKGROUND: The prevalence of gout has increased in the Western societies due to ageing and increasing BMI. Recently, lifestyle and dietary factors have been linked in epidemiological studies with an alteration of the risk of gout; however, there remains a lack of data on patient knowledge of these factors. The purpose of this survey-based study was to determine the knowledge of gout and its treatment both in the community and specialist care settings. METHODS: Participants were recruited from a hospital rheumatology outpatient department, consumer organization and a random sample of particip…

Gout. Novel therapies for treatment of gout and hyperuricemia.

In the past few decades, gout has increased not only in prevalence, but also in clinical complexity, the latter accentuated in part by a dearth of novel advances in treatments for hyperuricemia and gouty arthritis. Fortunately, recent research reviewed here, much of it founded on elegant translational studies of the past decade, highlights how gout can be better managed with cost-effective, well-established therapies. In addition, the advent of both new urate-lowering and anti-inflammatory drugs, also reviewed here, promises for improved management of refractory gout, including in subjects wit…

[Update Gout].

Update Gout Abstract. Gout, the most frequent arthritis worldwide, is seldomly considered a serious chronic disease. Cardiovascular morbidity and allover mortality are increased in gout patients. Prevalence of gout is very variable in different countries and is estimated to be 2.6% overall. Men are overrepresented (3:1 to 4:1). Environmental factors, such as diet and alcohol intake, are still important, but the genetic influence, e.g., in early-onset gout, gets more and more attention. Despite the broad range of therapeutic options outcomes are poor, and the disease often leads to disabling st…

Gout Storm.

BACKGROUND Gout is a chronic disease characterized by deposition of monosodium urate crystals, typically manifesting as arthritis. Clinical presentation of gout usually results from activation of local inflammatory response. Despite being one of the oldest diseases in the world, gout pathophysiology is incompletely understood and clinical features are still surprising. Recent reports describe unusual manifestations including atypical joints involvement, tenosynovitis, panniculitis, and multinodular inguinal swelling. Another atypical feature is the acute polyarticular gout with severe systemic…

[Pseudo-gout].

Pseudo-gout or calcium pyrophosphate crystal deposition disease (CPPD) is mainly a disease of the elderly. Diagnosis is obtained by identyfication of calcium pyrophosphate (CPP) crystals in the synovial fluid. Typical X-ray result is chondrocalcinosis (CC) mostly of the knee. CPPD is most frequently asymptomatic and not requires medical therapy. Treatment of acute form of desease is usualy joint aspiration and intra-articular injection.

[Tophaceous gout].

Identification of tophi indicates a definitive diagnosis of gout. However, recently they are rarely encountered. Tophi are most often seen in tissues that have a poor blood supply and low temperature, such as the ear helix and first MP joint. The nodules are yellowish-white, and non-tender, and range in size from 1 mm to 7 cm. Aspiration yields a chalky-like material that appears as needle-like crystals under light microscopy. In more advanced cases, tophi have a "punched-out" appearance with an "overhanging" margin on X-ray images. Differential diagnoses include rheumatoid nodules, xanthoma t…

Laryngeal gout.

A review of the literature reveals only 12 reported cases of laryngeal gout. We describe three further cases of laryngeal gout, the largest series ever published. The first case is previously undescribed acute gouty cricoarytenoiditis resulting in acute airway embarrassment requiring tracheotomy, and then we describe two cases of chronic tophaceous involvement of the thyroid lamina with computed tomography (CT) images of this process. The paucity of literature on this subject may not only be due to rarity but also lack of clinical awareness. Gouty involvement of the larynx must be considered i…
