Ten-year results after connective tissue grafts and guided tissue regeneration for root coverage.

BACKGROUND: This study clinically evaluates the 10-year results of connective tissue graft (CTG) and guided tissue regeneration (GTR) therapies using bioabsorbable barriers for root coverage (i.e., the reduction of recession depth). METHODS: In 15 patients, 38 Miller Class I and II recessions were treated. Recession defects received a CTG or GTR by random assignment. At baseline (immediately prior to surgery) and 6 and 120 +/- 12 months after surgery, clinical parameters were obtained. RESULTS: Nine patients, who contributed 24 recession defects, were available for re-examination at 120 +/- …

Antibacterial resorbable barrier membranes with therapeutic activity for guided tissue regeneration.

The objective of this study was to develop a bioabsorbable membrane composed of poly(butylene adipate-co-terephthalate) (PBAT) with alkyl trimethyl ammonium bromide (ATAB) to provide antimicrobial properties. Membranes were manufactured via the solvent casting technique using chloroform solutions containing PBAT and varying concentrations of ATAB (1, 2.5, and 5% wt), with ATAB-free membranes as a control. The characterization of the membranes included assessments of contact angle, surface free energy, and degradation in distilled water over periods of one week, one month, and three months. Mec

Self-assembling peptide-laden electrospun scaffolds for guided mineralized tissue regeneration.

OBJECTIVES: Electrospun scaffolds are a versatile biomaterial platform to mimic fibrillar structure of native tissues extracellular matrix, and facilitate the incorporation of biomolecules for regenerative therapies. Self-assembling peptide P11-4 has emerged as a promising strategy to induce mineralization; however, P11-4 application has been mostly addressed for early caries lesions repair on dental enamel. Here, to investigate P11-4's efficacy on bone regeneration, polymeric electrospun scaffolds were developed, and then distinct concentrations of P11-4 were physically adsorbed on the scaffo

Flow cytometry analysis of guided tissue regeneration-associated human periodontal cells.

BACKGROUND: Expanded polytetrafluoroethylene (ePTFE) barrier membranes have been widely used for guided tissue regeneration (GTR) of the human periodontal ligament (PL). However, the precise cellular and molecular events involved in the re-growth of the new tissue are still unclear. METHODS: Retrieved membranes and the newly-regenerated soft tissue (RT) underlying the membranes were used to examine the cells associated with GTR compared with normal human PL and gingival cells. Flow cytometry (FCM) was used, for the first time, to analyze the spindle-shaped fibroblast-like cells which were adh…

Cervical root resorption associated with guided tissue regeneration: a case report.

Root surface resorption, ankylosis (replacement resorption) and alveolar bone resorption are not uncommon sequelae to periodontal healing in both animal and human trials whether the treatment objective is regenerative, preventive, or conservative. The present report describes a case with progressive cervical root resorption in a patient who received periodontal regenerative treatment with guided tissue regeneration (GTR). A 46-year-old woman was referred for treatment of severe periodontitis. Remaining radiographic attachment was less than 50%. Following a period of 18 months, during which non…

Guided Tissue Regeneration Involving Piercing-Induced Lingual Recession: A Case Report.

Recession on the lingual aspect of mandibular incisors may occur in patients with history of tongue piercing and other factitious traumatic habits. Treatment of these areas is challenging due to the site-specific anatomical features of the region. This case report presents a novel approach for a specific type of mandibular lingual defect caused by tongue piercing. A nonresorbable titanium-reinforced barrier membrane combined with an allograft and enamel matrix derivatives was used to promote regeneration of periodontal attachment. Reentry surgery for membrane removal was performed at 8 weeks.

A systematic review of guided tissue regeneration for periodontal infrabony defects.

OBJECTIVES: To systematically review the evidence for efficacy of guided tissue regeneration (GTR) for infrabony defects. BACKGROUND: The evidence for the efficacy of GTR has not yet been systematically appraised. METHODS: We searched for randomised controlled trials of at least 12 months' follow-up comparing GTR with open flap debridement (OFD). Data sources included electronic databases, hand-searched journals and contact with experts. Screening, data abstraction and quality assessment were conducted independently by multiple reviewers. The primary outcome measure was gain in clinical atta…

Periodontal guided tissue regeneration with a new resorbable polylactic acid membrane.

The aim of the present study was to evaluate the use of 3 types of biodegradable, high molecular weight DL-lactic acid (PLA) membranes containing respectively 0%, 10%, and 30% low molecular weight oligomers for obtaining guided tissue regeneration following treatment of experimental buccal periodontal defects created surgically in 6 young adult beagle dogs. On the buccal aspects of test and control teeth, a mucoperiostal flap was raised and the buccal alveolar bone removed. The exposed root surfaces were scaled and a notch was prepared in the root surface at the level of the reduced crest. PLA…

Guided tissue regeneration using biodegradable membranes of polylactic acid or polyurethane.

The purpose of the present investigation was to evaluate the use of biodegradable membranes of polylactic acid or polyurethane for obtaining regeneration following treatment of circumferential periodontal defects in monkeys with the "guided tissue regeneration" procedure. Orthodontic elastic bands were placed around selected test and control teeth in 4 monkeys in order to induce breakdown of the periodontal tissues. When the destruction of the supporting tissues had progressed to a level corresponding to approximately half the length of the roots, the elastic bands were removed. After 1 to 8 m…

Effect of guided tissue regeneration using bioresorbable membranes on periodontal regeneration in grade II furcation defects.

Grade II furcation defects pose a significant therapeutic challenge in periodontology due to their complex anatomy and limited accessibility for effective regeneration. Therefore, it is of interest to evaluate the efficacy of guided tissue regeneration (GTR) using a bioresorbable collagen membrane compared to open flap debridement (OFD) alone in 48 patients with mandibular molar Grade II furcation defects. Clinical parameters (probing depth, clinical attachment level, horizontal probing depth and gingival recession) and radiographic bone fill were assessed at baseline and 6 months postoperativ…

Bone regeneration adjacent to titanium dental implants using guided tissue regeneration: a report of two cases.

The biologic principle of guided tissue regeneration was applied to regenerate alveolar bone in conjunction with the placement of titanium dental implants. In one case, complete osseointegration of an implant was achieved by the placement of a Teflon membrane over an implant that had been inserted into an alveolus immediately following tooth extraction. In a second case, the same biologic principle was used to increase the volume (height and width) of a resorbed, edentulous alveolar ridge to provide adequate bone dimensions for implant installation. In both cases, the membranes appear to have …

The effect of photobiomodulation therapy on guided tissue regeneration in a canine model.

OBJECTIVES: Guided tissue regeneration (GTR) is a widely accepted approach for the treatment of periodontal defects. However, its regenerative outcomes remain suboptimal. This study aimed to evaluate the hypothesis that the adjunctive use of photobiomodulation therapy (PBMT) with GTR would enhance periodontal regeneration in a canine model. METHODS: A total of 20 standardized periodontal defects (2 × 4 × 5 mm) were surgically created in five healthy adult male dogs. The defects were randomly assigned to three groups: Control (C, n = 3), Resorbable Membrane (M, n = 8), and PBMT + Resorbable Me…

Evaluation of guided tissue regeneration in the treatment of paired periodontal defects.

The aim of the present study was to compare guided tissue regeneration with conventional surgery in matched periodontal defects within the same subject. Twenty pairs of sites in nine subjects were treated. Surgery was performed at both test and control sites on the same visit. Random allocation was not performed until all preparation and root instrumentation had been completed. Test defects were covered with e-PTFE membranes (Gore-Tex) and secured with sutures. Membranes were removed after 5-6 weeks with a second surgical procedure, whereas the control sites were not re-operated. At 12 months …

Clinical application of a new bioresorbable guided tissue regeneration device: case reports.

Bioresorbable barriers have been recently introduced in clinical practice for guided tissue regeneration therapy. One of these is the Guidor matrix barrier which is made of amorphous polylactic acid softened with a citric acid ester to increase malleability and facilitate clinical handling. The advantages of the bioresorbable barrier include: the elimination of second surgery; better handling and adaptation around the tooth and over the bone; and integration of the connective tissue of the flap with the barrier preventing epithelial migration, gingival recession, and pocket formation. In the c…

Clinical results with exposed polyglactin 910 resorbable membranes for guided tissue regeneration.

Three cases are presented where early exposure of a resorbable membrane occurred during guided tissue regeneration therapy. An antimicrobial regimen was selected to determine whether infection of exposed membranes could be controlled to the point of achieving acceptable clinical results without membrane removal. The results suggest that with the use of the antimicrobial regimen: (1) exposure is compatible with successful clinical results, and (2) removal is not necessary.
