HELLP syndrome: CT evaluation.

Patients with the HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count) require careful observation and expedient delivery. However, those who develop this syndrome peripartum do not always fulfill its diagnostic criteria before labor begins. We investigated the potential usefulness of liver and spleen CT evaluation in identifying such patients. Seven patients with this syndrome were studied by CT scanning on postpartum days 0-3 and again 3-4 weeks after delivery. Although 3 patients with severe thrombocytopenia had a reduced liver-spleen CT number ratio (< 1.1) and subseq…

[Experiences with HELLP syndrome].

Between 1988 and 1991 thirty-nine pregnant women suffering from HELLP-Syndrome were treated in our hospital. A retrospective analysis of clinical course and of the changes in the relevant laboratory values was performed. The incidence of the HELLP-Syndrome at Freiburg University Hospital was 0.85% for all pregnancies or 17% of the patients with toxaemia. 90% of the women were primipara, there was no prevalence of a certain group. The clinical course was characterized by the perseverance of symptoms for more than two weeks in 18% of the cases and a perinatal mortality of 25%, the latter mainly

A review of HELLP syndrome.

HELLP (hemolysis, elevated liver enzymes, and low platelets) syndrome is a variant of severe preeclampsia which is associated with substantial maternal and perinatal morbidity and mortality. As with preeclampsia, the etiology and pathogenesis of HELLP syndrome is not completely understood. An increase in vascular thrombosis and activation of the coagulation system may be important in the clinical presentation of this disorder. Laboratory criteria for the diagnosis of HELLP syndrome have been classically described but lack uniformity among different institutions. Aggressive management of HELLP …

Biochemistry of HELLP syndrome.

The HELLP syndrome is a serious complication of pregnancy characterized by hemolysis (H), elevated liver (EL) enzymes, and low platelet (LP) count that occurs in 0.2-0.6% of all pregnancies and in 10-20% of cases with severe preeclampsia and frequently leads to adverse maternal and perinatal outcome. The exact pathobiology of HELLP syndrome has not been clearly defined. As it is considered a form or a complication of severe preeclampsia, it likely has its origin in aberrant placental development and function resulting in ischemia-producing oxidative stress. However, there is still a debate on …

[HELLP syndrome. Case report].

We present a case of a 40-year-old multigravida with rare combination of HELLP syndrome followed by hemorrhagic complications, DIC, acute renal failure, ARDS. During her stay in intensive care unit total abdominal hysterectomy was performed. Peritoneal dialysis was conducted for two days because of transient acute renal failure. A gradual improvement of her condition was observed after drainage of extraperitoneal haematoma. After forty-two days she was extubated and discharged from intensive care unit.

[HELLP syndrome in postpartum].

A case is presented of HELLP syndrome which developed several hours after normal labour. The increasing disorders in the blood clotting system (DIC) were the cause of a massive intracerebral haemorrhage and death of the patient.

[Is there an intermittent HELLP syndrome?].

A case of an HELLP-syndrome in the 32nd week of gestation with complete remission of pathological biochemical data and clinical symptomatic within five days is reported. Twenty days later a Caesarean section because of severe fetal growth retardation was performed. Because of this case it has to be discussed, if an "intermittent" HELLP-syndrome does really exist?

HELLP syndrome and the anaesthetist.

Four pregnant patients are described who had varying signs of pre-eclampsia plus haemolysis, elevated liver enzymes and a low platelet count. Two of the patients presented without hypertension and all posed considerable diagnostic difficulties, with problems in clinical management. Pregnancy induced hypertension is only one manifestation of a much more diverse pathophysiological process. Anaesthetists need to be aware of these other pregnancy related disorders in order to avoid diagnostic pitfalls and to enable them to provide safely the appropriate general and regional anaesthetic techniques.

HELLP syndrome and HELLP-like syndrome in pregnancies with covid-19 - case reports.

OBJECTIVE: We present two case reports of severe course of covid-19 in pregnancy demonstrating similarity between covid-19 and HELLP syndrome. CASE REPORT: The first case report describes an asymptomatic course of covid-19 accompanied by elevation of liver enzymes and lactate dehydrogenase but low ratio of angiogenic bio-markers. No severe pregnancy complication occurred. All laboratory results had normalized after recovering from covid-19. The second case report describes a patient with elevated liver enzymes and lactate dehydrogenase which preceded a respiratory failure. Furthermore, one of…
