Dynamic procedures for claw-finger correction--dilemma of balancing tension in transferred tendon slips.

The critical step in dynamic claw-finger correction procedures is adjustment of tension on the tendon slips which are being sutured at the new insertion sites to correct finger-clawing. Several methods to balance and adjust the tension have been described ever since these procedures have been in use. Ultimately it is the experience of the operating surgeon that helps to decide as to the tension that is to be kept on each slip so that maximum deformity correction is obtained without compromising the functional capabilities of the hand. An attempt has been made to describe this "experience" in w

Rheumatoid hand deformities: pathophysiology and treatment.

Rheumatoid disease, as it affects the hand, is a disease of the synovium lining the joints and sheaths of the tendon. The proliferating synovium destroys the articular surfaces of the joint, interferes with the gliding mechanism of the tendons and weakens the supporting ligaments of the joints. The degree and variety of deformities is multifold. Treatment of the rheumatoid hand is aimed at conservation and restoration of hand function, as well as prevention of future deformities. Rheumatologists, physical therapists and hand surgeons carry out important functions in the well-planned, integrate

Swan-neck deformity: mechanism and surgical treatment.

There are several forms of swan-neck deformity which are amenable to surgical treatment with good results, if based on an adequate study of the mechanism. Selection of the most suitable surgical procedure must be based on the pathological type.

Intrinsic contractures of the hand.

Contractures of the intrinsic muscles of the fingers disrupt the delicate and complex balance of intrinsic and extrinsic muscles, which allows the hand to be so versatile and functional. The loss of muscle function primarily affects the interphalangeal joints but also may affect etacarpophalangeal joints. The resulting clinical picture is often termed, intrinsic contracture or intrinsic-plus hand. Disruption of the balance between intrinsic and extrinsic muscles has many causes and may be secondary to changes within the intrinsic musculature or the tendon unit. This article reviews diagnosis,

[Clinical aspects and surgical treatment of Dupuytren contracture].

The etiology of Dupuytren's contracture is still not clear. Many theories have been proposed, but no basic data are currently available that would make it possible to treat Dupuytren's contracture by other methods than surgical intervention. The surgical technique used in our clinic is described, and a postoperative treatment is also outlined. This paper is designed to propose an acceptable technique for practicing trauma and hand surgeons.

[Swan-neck deformity].

The factors contributing to stability in the proximal interphalangeal joints are described. As a result of damage to one or more links of the system, a typical abnormal position of the joints occurs, called swan-neck deformity. The possibilities for stabilization of the joints are discussed with emphasis on the technique of LITTLER which employs a distally based lateral band. This band prevents recurvatum deformity of the PIP joint and flexion deformity of the DIP joint when it is passed palmar to CLELAND's ligament, thereby simulating the oblique retinacular ligament of LANDSMEER.

Correction of ulnar claw hand and Wartenberg's sign.

Ulnar claw hand usually occurs when the ulnar nerve is damaged distally. Claw hand deformity is characterized by metacarpophalangeal hyperextension and interphalangeal flexion, making it impossible to oppose the fingers and thumb. Bouvier's test is used to guide the procedure. Palliative surgery requires prior preparation of paralytic hands. In case of a positive Bouvier's test, Zancolli's lasso technique is preferred because of its effectiveness. Capsuloplasty with anteroposterior transosseous fixation is used if the bone is strong enough and when flexor digitorum profondus muscle is inactive

Pollicization.

Congenital absence or traumatic amputation of the thumb is a major disability profoundly disturbing the function of the hand, as the thumb represents an estimated 50% of total hand function. The methods for thumb reconstruction utilizing the index finger were developed some twenty years ago, and progress has been made in the last several years with emphasis on intrinsic muscle reconstruction as emphasized by Buck-Gramcko. The method, technique and anatomy of index pollicization is discussed, and representative cases presented for congenital absence of the thumb and for traumatic amputation of

[General aspects of hand surgery in childhood (author's transl)].

Good regeneration of tissue, easy adaptation and rapid nerve regeneration are all favourable factors of hand surgery in infants and children. On the other hand, unfavourable factors include diagnostic difficulties, especially following early trauma and the absence of co-operation, as well as late damage due to growth retardation. Indications for surgery include malformation, accident, burns, infections, and rarely tumours, and treatment should as far as possible, be delayed until preschool age. Accurate operative technique and careful planning of surgery are essential. Post-operative fixation

Clarification of Extensor Tenotomy for Finger Deformities.

This article clarifies the terminology and different applications of proximal Fowler extensor tenotomy and distal Dolphin extensor tenotomy. Although Fowler is a popular eponym for all finger extensor tenotomies, proximal Fowler tenotomy is specifically indicated for swan neck deformities, whereas distal Dolphin tenotomy is indicated for boutonniere deformities. For swan neck deformities with a distal interphalangeal joint extensor lag of greater than 40°, central slip tenotomy can be combined with terminal tendon imbrication or reinsertion. More distal Dolphin tenotomy of the lateral conjoine

A modified Terrono classification for Type 1 thumb deformity in rheumatoid arthritis: a cross-sectional analysis.

The most common thumb deformity in rheumatoid arthritis is Nalebuff Type 1 deformity (boutonniere deformity). Type 1 deformity severely impairs hand function, and this impairment is evaluated by the Terrono classification. In some cases, the Terrono classification incorrectly categorizes advanced thumb deformity into earlier stages. We modified the Terrono classification by assessing the active range of motion of the interphalangeal joint prior to assessing the passive range of movement of the metacarpophalangeal joint. An active range of movement of the interphalangeal joint was strongly corr
