The third heart sound.

A unitary concept is proposed to explain the genesis of the third heart sound and associated "rapid filling wave" of the apexcardiogram in physiological and pathological states including constrictive pericarditis. This theory not only clarifies the hitherto unexplained phenomenon such as presence of S3 in significant mitral stenosis, but also places in proper perspective the hemodynamic significance of S3 in physiological and various pathological states.

The physiologic cause of swallowing sounds: answers from heart sounds and vocal tract acoustics.

A hypothetical discussion of the cause of swallowing sounds is presented. It is suggested that the pharynx contains a number of valves and pumps that produce reverberations within the pharynx to generate swallowing sounds. As heart sounds are propagated via vibration of muscles and valves, it is further suggested that an analogy exists between the generation of heart sounds and swallowing sounds. This new theory is known as the cardiac analogy hypothesis. The inability of the current literature to explain the cause of swallowing sounds is seen to limit the diagnostic potential of cervical ausc…

Heart rate study using the cardiac sounds.

The purpose of this paper is to present a straightforward framework for Heart Rate (HR) estimation from a Phonocardiogram (PCG) record and study the association between murmur severity levels and HR. The system focuses primarily on data processing procedure, which is based on signal preprocessing using Maximal Overlap Discrete Wavelet Transform (MODWT) to delineate murmurs from heart sounds. We exploit the characteristics of Logistic function to derive an enhanced PCG envelope that serves as prerequisite for HR algorithm detection. In fact, the PCG envelope presents a cyclostationarity that ca…

Mild cough · wheezing · loud heart sounds · Dx?

A 25-year-old man, who was an active duty US Navy sailor, went to his ship's medical department complaining of a mild cough that he'd had fo … within normal limits. Auscultation revealed mild wheezing throughout the upper lung fields and loud heart sounds throughout his chest that were audible even with gentle contact of the stethoscope diaphragm. He had no discernible murmurs, rubs, or gallops. In light of the unusually loud heart sounds heard on exam, we performed an electrocardiogram. The EKG reve …

An automatic segmentation method for heart sounds.

BACKGROUND: There are two major challenges in automated heart sound analysis: segmentation and classification. An efficient segmentation is … s of heart sound components are first located, and the components are then recognized. Finally, the heart sounds are divided into several segments on the basis of the results of boundary localization and component identification. RESULTS: In order to evaluate the performance of the proposed method, quantitative experiments are performed on an authoritative he …

Heart sounds: are you listening? Part 2.

The first of this two-part article on heart sounds was in the Spring 2013 issue of the Canadian Journal of Cardiovascular Nursing (Reimer-Kent, 2013). Part 1 emphasized the importance of all nurses having an understanding of heart sounds and being proficient in cardiac auscultation. The article also focused on an overview of the fundamentals of cardiac auscultation and basic heart sounds. This article provides an overview of the anatomy and pathophysiology related to valvular heart disease and describes the array of heart sounds associated with stenotic or regurgitant aortic and mitral valve c…

[Physical diagnostics--auscultation of fetal heart sounds].

Auscultation or recording of foetal heart tones is possible throughout pregnancy. In the first trimester, the presence of heart activity proves a vital pregnancy and reduces the probability of spontaneous abortion. From the end of the second and during the whole of the third trimester foetal heart activity can be used to asses the foetal condition. The introduction of cardiotocography failed to reduce serious perinatal morbidity or mortality as compared with intermittent auscultation, but did increase the number of obstetrical interventions.

Heart sound and lung sound separation algorithms: a review.

Breath and cardiac sounds are two major bio sound signals. In this, heart sounds are produced by movement of some body parts such as heart valve, leaflets and the blood flow through the vessels, whereas lung sounds generates due to the air in and out flow through airways during breathing cycle. These two signals are recorded from chest region. These two signals have very high clinical importance for the patient who is critically ill. The lung functions and the cardiac cycles are continuously monitored for such patients with the help of the bio sound signal captured using suitable sensing mecha…
