A case of a patient with mitral valvular disease with a mitral valve prosthesis type Björk-Shilley of two year duration is reported. The patient was admitted to the intensive care cardiac unit in grave condition--shock and left cardiac failure. The patient died after 14 hours in spine of the efforts to over come the cardiogenic shock. The post mortem examination revealed a broken prosthesis and lack of one of the cusps which was found in the abdominal section of the aorta 10 cm above the bifurcation.

The article deals with the results of ultrastructural study of the myocardium at three stages of cardiosurgical operations: preischemic, ischemic, and during reoxygenation. A statistically significant difference was revealed in the condition of the cardiomyocyte organelles in each of these stages, and the degree of their intraoperative destruction was determined. Starting in February 1978 52 St. Jude Medical Prostheses have been implanted in 47 patients (age 13--73 years) in aortic, mitral and tricuspid positions. Perioperative death occurred in 3 cases. Post mortem examination of these patients showed unimpaired valvular function. There were no other complications in an accumulated total valve time of approximately 300 months. In order to be able to offer pregnant artificial heart valve patients a practical management approach enabling reduction of maternal and fetal risks, the authors evaluated 40 pregnancies in 24 women. Thirty one valves had been inserted prior to these pregnancies: 24 mechanical valves and 7 biological valves. OBJECTIVES: In patients with acquired valvar disease, morbidity and mortality rates after tricuspid valve replacement (TVR) are high. However, in adult patients with congenital heart disease, though data concerning outcome after TVR are scarce, even poorer results are suggested in patients with Ebstein anomaly. To investigate the applicability of these results to a broader array of congenital heart disease patients, we report the long-term follow-up of prosthesis-related complications, including re-replacement of patients with a tricuspid valve prosthesis and congenital heart disease.

42 patients with isolated mitral valve defect were divided into two groups: without cardiac complications -- and with acute myogenic heart failure, after implantation of a mitral valve prosthesis.
