PURPOSE: To report a case of bitemporal hemianopsia due to ischemic chiasmopathy after mechanical thrombectomy of a right distal internal carotid artery (ICA) occlusion.
OBSERVATIONS: A 60-year-old female presented with left sided weakness and difficulty speaking and was found to have suffered a right internal carotid artery occlusion 10 days after tricuspid valve replacement for severe symptomatic tricuspid valve disease. She underwent mechanical thrombectomy and in hospital and at further follow ups was noted to have a bitemporal hemianopsia, consistent with an ischemic optic chiasmopathy.

BACKGROUND: Alzheimer's disease is a chronic neurodegenerative condition, mostly affecting the medial temporal lobe and associated neocortical structures. In this report, we present a rare clinical manifestation of this disease.
CASE REPORT: A 61-year-old female with word finding difficulty and memory disturbances was diagnosed with Alzheimer's disease. Two years later, she complained of right homonymous hemianopia without optic ataxia, ocular apraxia, and simultagnosia. No findings other than parenchymal disease were apparent in magnetic resonance imaging and laboratory tests.

We report a case of a prolonged postictal hemianopsia occurring after a focal extraoccipital seizure. A 55-year-old man with a history of neurosyphilis, treated with penicillin, presented to our epilepsy monitoring unit (EMU) for diagnostic evaluation of his spells occurring for 2 years. CLINICAL CASE: We describe the case of a man who, after a gunshot wound to the right facial region, exhibited chorioretinitis sclopetaria of the right eye and a contralateral homonymous hemianopsia.
DISCUSSION: Chorioretinitis sclopetaria is a rare entity resulting from an ocular injury caused by a bullet passing through the orbit. It is not common to find the condition associated with a damaged visual field caused by the subsequent path traversed and lodging of that bullet in the brain.

Cross sections of the orbital portion of the optic nerve, the decussating fibers of which underwent atrophy, have a distinctive histologic pattern. Distally, from the eyeball to the midportion of the optic nerve, this pattern of atrophy consists of a broad-based nasal and a narrow-based temporal sector that form a horizontal band. Proximally, near the optic canal, the pattern changes. The atrophic bundles occupy the nasal and central portions of the optic nerve only.

In cases of spontaneous haemorrhage without severe impairment of consciousness, indications for surgical treatment remain controversial and the effect of surgery on focal deficits is unclear. We report the case of a young man operated for left occipital spontaneous haemorrhage (28 cm(3)) who had presented with right lateral hemianopsia without impaired consciousness. The visual field defect improved soon after the operation and resolved over the following months. The rationale for this approach is discussed.

Giant or large anterior communicating artery (ACOA) aneurysms producing bitemporal hemianopsia are extremely rare. A case is presented of an ACOA aneurysm measuring 2 X 1.4 cm with the patient having had a post-traumatic amaurosis of the left eye for many years. Then he had a subarachnoid hemorrhage and subsequently developed impaired vision in the right eye and temporal hemianopsia. The aneurysm was visible on C.T. but disappeared on post-operative scans. Visual acuity improved radically after treatment with neck ligation and opening of the sac.
