Acute normovolemic hemodilution is a most practicable method to avoid the transfusion of homologous blood in all those cases of surgical interventions which make expect an intraoperative loss of blood up to 30% of plasma volume. After introduction of anaesthesia 2-4 bags of blood are taken from the patient and simultaneously exchanged for an equal quantity of plasma substitutes. According to the amount of operative loss of blood the withdrawn blood will be retransfused still during operation or in the course of the same day. Under normovolemic hemodilution cardiac output. Coronary perfusion, a…

Homologous blood transfusion, while frequently life-saving, is attended by risks and complications. Autologous blood transfusions have become an increasingly common alternative. Volume expansion, which is simpler, also is used. This study was designed to construct computer models of hypervolemic hemodilution and normovolemic hemodilution to compare them with each other and with normal (neither hypervolemic nor normovolemic hemodilution). Each model started with blood volume (BV) equal to 5,000 mL. Initial hematocrits (HCTs) were varied from 25% to 50%. Following phlebotomy and hemodilution or …

The effects of intraoperative prebleeding and hemodilution with lactated Ringer's solution on hemodynamics, oxygen transport and lung water were studied in four patients undergoing extensive surgical procedures. The results were contrasted with those previously obtained from hemodilution of a group of patients with Plasmanate. The mean volumes bled were 1,950 milliliters in the lactated Ringer's solution group and 1,697 milliliters in the Plasmanate group. The posthemodilution hematocrit values were 26 per cent and 25 per cent, respectively. Cardiac output increased to only 115 per cent of the…

Hypervolemic hemodilution (HVH) is a blood-sparing technique increasingly used in pediatric neuroanesthesiology, particularly during high-risk procedures such as craniosynostosis repair. By expanding intravascular volume with crystalloids or colloids before surgery, HVH dilutes red blood cell concentration, thereby reducing their loss due to bleeding, while maintaining cerebral oxygenation and hemodynamic stability. This approach is especially valuable in pediatric patients, who are more susceptible to transfusion-related complications due to their limited blood volume and immature immune syst…

Shock was induced in sixty dogs by laparotomy, splenectomy and exteriorization of the small intestine for three hours. Resuscitation following different plasma substitutes was assessed by measuring hemodynamic parameters (mean arterial blood pressure, plasma volume, oxygen consumption, cardiac output and hematocrit). To justify a comparison, the colloids (albumin, ACD-plasma, dextran 40, dextran 70, gelatine) were given at the same concentration and dose (3.5 % solutions in saline at a dose of 1.5 g/kg or 43 ml/kg). Ringer's acetate was given in a three times larger volume (129 ml/kg). After r

Better understanding and advances in the management of neonates undergoing cardiac surgery have increased their survival. Twenty neonates were studied to evaluate the technique of acute hemodilution in open cardiac surgery. They were premedicated by rectal barbiturate, anesthetized and monitored for all vital signs and blood chemistry. Patients operated upon with hemodilution suffered less respiratory acidosis during bypass. They showed marked drop in pH which was rapidly corrected in the postoperative period. No significant change in blood pressure from the expected was observed. These neonat…

Clinical analysis of 168 cases of hemorrhagic shock of the 230 cases of hemorrhage and injuries admitted to the 2nd Surgical Clinic of Timis … f blood should not involve any risks. Even in the cases in which blood tranfusion is indispensable, hemodilution reduces to a great extent the amount of blood required.

Acute normovolemic hemodilution was performed in 46 pts. before major surgical procedures 1800 ml of pt's blood were withdrawn and hct. lowered to 24.8% mean. The hct. after retransfusion averaged 31.8% while homologous blood could widely be avoided. 78% of pts tolerated a.n.h. well or sufficiently. During a.n.h. circulatory parameters remained constant while CO rose from 4.4 to 6.1 1/min. Shortly after a.n.h., in 13 cases slight, in 10 cases, however, severe side reactions occurred: rise in BP, HR and PAP, peripheral vasoconstriction with decrease of peripheral blood flow, unproportional incr…
