Invasive methods of active removal, i.e., hemodialysis and hemoperfusion, are indicated for severely poisoned patients with substances causing tissue damage. Both methods will only be efficacious if the relative contribution of the extracorporal clearance to the overall elimination for a given substance is high and if the toxin has a relatively low apparent volume of distribution and a low tissue binding. Six patients (two men and four women) aged between 15 and 60 years in hepatic coma stage IV and with very severe EEG changes were treated with hemoperfusion. Hemoperfusion was performed on 58 occasions to treat 48 patients with severe intoxication, of which 23 had taken on overdose of barbiturate or meprobamate. With one exception they all showed remarkable improvement during the treatment. In addition in many other intoxications patients greatly benefited from hemoperfusion, as with salicylate, paracetamol, quinidine, propranolol and mushroom intoxications. Five patients with severe barbiturate intoxication were treated with charcoal hemoperfusion. The perfusions were carried out for 3.5 hr, two of the patients requiring an additional treatment. A marked clinical improvement was observed in all of the patients. The serum concentration decreased by 17 to 42% and the amount removed corresponded to 4 to 13% of the ingested dose. A 44-year-old female was found comatose after attempting suicide. Toxicological screening showed phenobarbital intoxication. The patient was treated symptomatically. After ten days her serum level of phenobarbital still had not decreased and she was not clinically recovered. The patient was transferred to another hospital for hemoperfusion to decrease the level of phenobarbital.
