Autoimmune hepatitis is a disease of the hepatic parenchyma that can present in acute or chronic forms. In common with many autoimmune diseases, autoimmune hepatitis is associated with non-organ-specific antibodies in the context of hepatic autoimmunity. This dichotomy has made definition of a unifying hypothesis in the pathophysiology of the disease difficult, although data from the past 8 years have drawn attention to the role of regulatory T cells. Several triggers have been identified, and the disease arises in genetically susceptible individuals. Clinical and biochemical remission is achi

Autoimmune hepatitis (AIH) was the first chronic liver disease in which remission was achieved by immunosuppression. Prognosis is poor when left untreated. Since the original description in 1950 by Waldenström, the initially reported treatment option has remained until today and is the core of the basic therapeutic strategy of inducing remission with steroids and azathioprine. Immunosuppression as a treatment concept spans different situations including the induction and maintenance of remission, treatment of nonresponders, avoidance of side effects, perioperative treatment of liver transplant

Autoimmune hepatitis (AIH) is an inflammatory liver disease affecting mainly females and characterised histologically by interface hepatitis, biochemically by elevated transaminase levels and serologically by the presence of autoantibodies and increased levels of immunoglobulin G. AIH responds to immunosuppressive treatment, which should be instituted as soon as diagnosis is made. Seropositivity for smooth muscle and/or anti-nuclear antibody defines type 1 AIH, while positivity for liver kidney microsomal type 1 antibody defines type 2 AIH. The aetiology of AIH is unknown, though both genetic

Autoimmune hepatitis (AIH) is a chronic and progressive disease. The etiology and epidemiologic characterizations of AIH are not fully understood. It has a prolonged course and can affect all age groups. Symptoms and signs may appear years before diagnosis, and may be manifested for extended periods only as minor liver enzymes abnormalities. Clinical manifestations vary from lethargy and jaundice, to extremely severe deterioration in quality of life, hepatic failure and liver transplantation or death. In most cases there is a good response to therapy, making it important to diagnose the diseas

Autoimmune hepatitis is characterized by inflammatory liver histology, circulating nonorgan-specific autoantibodies, and increased levels of immunoglobulin G, in the absence of a known etiology. Two types of juvenile autoimmune hepatitis (AIH) are recognized according to seropositivity for smooth muscle and/or anti-nuclear antibody (AIH type 1) or liver kidney microsomal antibody (AIH type 2). There is a female predominance in both. AIH type 2 presents more acutely, at a younger age and commonly with immunoglobulin A deficiency, whereas duration of symptoms before diagnosis, clinical signs, fa

Autoimmune hepatitis (AIH) is a progressive inflammatory hepatitis of unknown etiology that is responsive to immunosuppressive therapy. The diagnosis of AIH should be guided by the descriptive criteria and scoring system set forth by the International Autoimmune Hepatitis Group. Standard therapy is prednisone with or without azathioprine (AZA). Combination therapy with prednisone and AZA is preferred, as it allows treatment with lower individual doses of each drug and is thus associated with fewer side effects. Treatment goals include complete biochemical, clinical, and histologic remission. T

Autoimmune hepatitis (AIH) is an intermittently progressive liver disease characterized by hypergammaglobulinemia, autoantibodies, predominately periportal hepatitis, and a favorable response to corticosteroid therapy in most cases. There are no pathognomonic markers of autoimmune liver disease; rather, the diagnosis must be based on the presence of a constellation of characteristic clinical, laboratory, and histologic features in patients without other causes of such liver pathology. Although prednisone or prednisolone are the mainstays in initial therapy of AIH, azathioprine or other drugs w

Autoimmune hepatitis affects all ages with a peak incidence in preadolescent girls. The pathogenesis of autoimmune hepatitis has not been defined. Susceptibility, clinical manifestations, and treatment outcomes are affected by environmental factors, individual immunoregulatory responses, genetic factors, age and gender. An international panel has developed diagnostic criteria for autoimmune hepatitis, and a scoring system to assess the strength of the diagnosis has also been proposed. This article discusses the diagnosis and treatment of the three types of autoimmune hepatitis proposed based o

First the short history of the disease, then its etiopathogenesis, the role of genetic, environmental and immunologic factors are described. In the second part, the questions of diagnosis, differential diagnosis and the immunosuppressive treatment, with new therapeutic modalities and liver transplantation are discussed.

Autoimmune hepatitis (AIH) is characterised histologically by interface hepatitis, and serologically by the presence of non-organ and liver specific autoantibodies and increased levels of immunoglobulin G. Its onset is often ill-defined, frequently mimicing acute hepatitis. AIH usually responds to immunosuppressive treatment, which should be instituted as soon as diagnosis is made. Autoimmune hepatitis is the archetype of liver autoimmune disease. It is divided into two types, according to the autoantibody profile. It should be suspected and diagnosed early since treatment is highly effective in preventing liver failure and transplant. The possible pathogenic scenario is described in the present review.
