Sclerosis and hyperostosis of the manubrium sterni.

Seven females and one male with almost identical sclerotic and hyperostotic changes of the manubrium sterni are reported. The clinical course was prolonged and characterized by intermittent periods of exacerbation followed by improvement. Malignancies, bacterial inflammatory processes, and Paget disease, which were first suspected, could be excluded. Pustulosis-palmoplantaris was present in four patients and one had psoriasis vulgaris. None of the patients had sacroiliitis or peripheral joint changes, but the three youngest patients had sclerotic changes of other bones, including the lumbar sp

[Hyperostosis of the skull (author's transl)].

X-ray findings and the result of microradiographic examinations of hyperostotic processes of the skull (Osteomas, Meningioma, Fibrous Dysplasia, Morbus Paget) are described. The films permit a reliable diagnosis statement only in a few cases, because similar findings can be seen in different diseases. Microradiographic investigations demonstrate the microstructure of the tissue of hyperostotic processes. There are no specific findings typical for one disease alone, but there are qualitative and quantitative variations in the different diseases mentioned above. Microradiography gives informatio

Diffuse idiopathic skeletal hyperostosis: musculoskeletal manifestations.

Diffuse idiopathic skeletal hyperostosis (DISH) is a common disorder of unknown etiology that is characterized by back pain and spinal stiffness. There may be mild pain if ankylosis has occurred. The condition is recognized radiographically by the presence of "flowing" ossification along the anterolateral margins of at least four contiguous vertebrae and the absence of changes of spondyloarthropathy or degenerative spondylosis. Even in patients who present with either lumbar or cervical complaints, radiographic findings are almost universally seen on the right side of the thoracic spine. Thus,…

Hyperostosis frontalis interna, acromegaly and hyperprolactinaemia.

The association between hyperostosis frontalis interna (HFI), acromegaly and hyperprolactinaemia was investigated. Thirty six acromegalic patients, of whom 19 had hyperprolactinaemia, were compared with 36 randomly-selected, age-sex matched controls. There was a higher prevalence of HFI in the skull X-rays of the acromegalic cohort (P = 0.0002) when compared to the control group. This difference was apparent in both men (P = 0.01) and women (P = 0.01). Acromegalic patients with hyperprolactinaemia also expressed HFI in a higher proportion of individuals than the control group (P = 0.0001). Int…

Diffuse skeletal hyperostosis in idiopathic hypoparathyroidism.

A case of idiopathic hypoparathyroidism (IHP) is reported with extensive ligamentous and tendinous ossification and soft tissue calcification. The pertinent radiological features of IHP and the unusual findings in this case are reviewed together with similar previously reported cases. Whether IHP is a causative or aggravating factor in the aetiology of the skeletal changes is discussed with particular reference to their similarity to diffuse idiopathic skeletal hyperostosis (DISH). We conclude that, in patients with an ossifying diathesis, IHP acts as a stimulant resulting in exuberant skeleta…

Cervical ankylosing hyperostosis and airway obstruction.

Forestier's disease, or ankylosing hyperostosis of the cervical spine, has been described to cause dysphagia, foreign body sensation, and aspiration. We report two patients with ankylosing hyperostosis producing ulceration of the posterior plate of the cricoid cartilage, inflammatory edema, and secondary bilateral vocal cord paralysis with airway obstruction. This disease initially produces minimal dysphagia when the primary location of the osteophyte is just above and posterior to the cricoid, then, progressive airway obstruction. The pathogenesis is infection superimposed on ulceration of th…

[Sterno-costo-clavicular hyperostosis (author's transl)].

The authors report two cases of sterno costo clavicular hyperostosis. Six cases have been described in previous reports by Köhler (five cases) and CAMUS (one case). This disease begins with sterno costo clavicular pain that develops with exacerbations over several years. Biologically, there is a no specific inflammatory syndrome. Radiologically, clavicles sternum and first ribs are enlarged and increased in density. There is also an ossification of the sterno clavicular and sterno costal junctions. Phlebography sometimes show subclavian veins occlusion. Histological finding is hyperostosis wit…

Hyperostosis associated with orbital vascular malformation.

A previously healthy adult male presented with a slowly enlarging orbital mass associated with 5 mm of non-pulsatile proptosis. On imaging, a soft tissue lesion with avid contrast enhancement and associated bony hyperostosis was noted. The lesion and hyperostotic bone were surgically debulked, and significant arterial bleeding was noted intraoperatively consistent with an arteriovenous malformation. Histopathologic analysis revealed a vascular malformation with enhanced microvasculature infiltrating the periosteum. While vascular lesions elsewhere in the body can be associated with skeletal ch…
