Hypertonic/hyperoncotic solution in hypovolemic patients: experience in the emergency room.

Hypertonic solutions have been studied extensively in the treatment of hypovolemic shock, both in experimental and clinical models. Safety, efficacy, and long-term effects on animals and patients have been evaluated. The present article reviews indications, safety, mortality rates, and outcome in patients with hemorrhagic hypovolemic shock who were treated after admission with a hypertonic/hyperoncotic solution under strict observation in the emergency room.

Small-volume resuscitation with hypertonic saline solution in hypovolemic cats.

We evaluated the hemodynamic effects of IV and intraaortic (aortic root) administration of 7.5% NaCl solution on hemodynamics in anesthetized cats with severe hypovolemia. Hypovolemic shock was induced by exsanguinating cats to a mean arterial blood pressure of 50 mm of Hg, which was maintained for 30 minutes prior to treatment. Shed blood volume was 38.4 +/- 2.1 ml/kg of body weight. The cats were treated with a small volume (4 ml/kg) of 0.9% NaCl solution IV, 7.5% NaCl solution IV, or 7.5% NaCl solution administered into the aortic root. The IV administration of 0.9% NaCl solution did not im

Effect of hypertonic NaCl-starch-solution on oedema of different pathogenesis.

Small volumes of hypertonic NaCl-solutions have been proven to restore haemodynamics in hypovolemic shock patients. Topic of this study was to investigate whether bolus application of 7.5% NaCl-6.5% starch-solution (HSS) apart from its relevance in shock might be an effective therapy in oedema. Considering differential therapeutic aspects, the volume effects of 7.2 ml HSS were tested in three types of oedema: hydrostatic oedema induced by venous congestion (n = 6), oedema caused by bradykinin injection (n = 6), and proteinase-induced oedema (n = 6). The arterial, venous pressure and weight cha

[Mechanisms of the action of hypertonic solutions on the ionic permeability of human skin].

For human skin transcutaneous transport of I ions in vitro depends on sodium chloride-iodide concentration relations in mineral water, whereas such transport for Br ions in mainly determined by absolute concentration of sodium chloride. Optimal proportions of the salts have been specified for I and Br ions transcutaneous transport in definite concentration range. An unknown feature of the mechanism of action of hypertonic salt solutions on ionic skin permeability has been identified. The results are analyzed from the point of view of the ion pairs conception.

[Role of a hypertonic theophylline solution and blood transfusion in necrotizing colitis].

The aim of this study was to evaluate the influence of an hyperosmolar solution of theophylline and of blood transfusion in the occurrence of necrotizing enterocolitis (NEC). By the end of 1982, theophylline previously given in alcoholic solution (osmolality greater than 4000 mosm/kg) was given in a preparation of sodium benzoate isoosmotic to the plasma; in addition, blood was irradiated for 20 minutes by X-Rays (5000 rads) before every transfusion. In the same unit of premature infants, the incidence of NEC was 6.7% in 1981-82; it decreased to 2.7% in 1983-84 (p less than 0.02). In the same

Hypertonic saline and seawater solutions damage sinonasal epithelial cell air-liquid interface cultures.

BACKGROUND: Nasal irrigation (NI) is commonly used to treat several sinonasal diseases, including chronic rhinosinusitis with nasal polyps (CRSwNP); however, the effects of NI on the sinonasal epithelium are not fully known. The aim of this study was to investigate the effects of commonly used NI solutions on epithelial mucociliary and barrier functionality in primary cultured human nasal epithelial cells (HNECs). METHODS: HNECs from control subjects and patients with CRSwNP were established as air-liquid interface (ALI) cultures. Differentiated cultures were treated with different NI solutio

[New therapeutic approaches in the treatment of shock: hypertonic hyperoncotic solutions and vasopressin].

The extreme disturbance of hemodynamics in shock leads to a minimized oxygen delivery to several vital organs. If this state is not rapidly lifted, a multi-organ-failure can occur. In addition to the removal of the underlying causes, for example, bleeding or septic focus, measures must be started to stabilize hemodynamics. In most cases shock can be successfully treated with standard therapeutic interventions including the use of crystalloid or colloid solutions as well as the infusion of inotropes or vasopressors. Up to now, there is not enough evidence to show that hypertonic/hyperoncotic so

Prehospital Resuscitation of Traumatic Hemorrhagic Shock with Hypertonic Solutions Worsens Hypocoagulation and Hyperfibrinolysis.

Impaired hemostasis frequently occurs after traumatic shock and resuscitation. The prehospital fluid administered can exacerbate subsequent bleeding and coagulopathy. Hypertonic solutions are recommended as first-line treatment of traumatic shock; however, their effects on coagulation are unclear. This study explores the impact of resuscitation with various hypertonic solutions on early coagulopathy after trauma. We conducted a prospective observational subgroup analysis of large clinical trial on out-of-hospital single-bolus (250 mL) hypertonic fluid resuscitation of hemorrhagic shock trauma …

Effects of hypertonic solutions on two species of human intestinal parasites during fecal examination.

Aim: Ova and parasite examination by flotation requires hypertonic solutions, which can damage the egg and cyst membranes, leading to false negatives. The authors investigated the harmful effects of ZnSO4 and C12H22O11 solutions on the ova and parasite examination. Materials & methods: The authors processed samples using the Three Fecal Test technique. Aliquots were floated in different pH levels, temperatures and solution densities. Results: Densities above 1.12 g/ml led structures to collapse after 6-10 min. pH neutralization of the ZnSO4 solution did not prevent the parasites from changing.…

Intravenous high-dose furosemide and hypertonic saline solutions for refractory heart failure and ascites.

Several studies have shown the efficacy of hypertonic saline solution infusion in conditions in which regional organ blood flow is impaired. Our group has shown that treatment of patients with diuretic-resistant heart failure with high-dose furosemide plus hypertonic saline is effective and well tolerated, improving symptoms of congestion, reducing plasma levels of markers of neurohormonal and inflammatory activation, decreasing hospital readmission rates, and reducing long-term mortality. The same regimen was shown to be better than repeated paracentesis in patients with cirrhosis and refract
