OBJECTIVE: Hyperuricemia (HUA) is strongly associated with abnormal glucose metabolism and insulin resistance (IR). However, the precise molecular mechanism of HUA-induced IR is still unclear. Retinol binding protein 4 (RBP4) has been shown to induce IR in type 2 diabetes mellitus. This study was designed to clarify the relationship between RBP4 and HUA-induced IR and its potential mechanisms.
METHODS: Patients with HUA were collected to detect the levels of plasma RBP4 and clinical biochemical indicators. Rats were fed with 10% high yeast and oteracil potassium (300 mg/kg) via intraperitonea…

In 1996, the need for the clinical guidelines for the management of hyperuricemia and gout was proposed of the consensus conference held at the 29th annual meeting of the Japanese Society of Purine and Pyrimidine Metabolism (president Yuji Matsuzawa). At the consensus conference, the following announcement was made. 1. Because the majority of patients with hyperuricemia are in the condition of multiple risk factor clustering syndrome, hyperuricemia per se should be counted as one of the typical lifestyle related diseases. 2. Medical management should be directed independently for the treatment…

BACKGROUND: The increasing incidence rate of hyperuricemia (HUA), a common chronic metabolic disease, seriously threatens public health. Previous studies have shown that rosmarinic acid (RA) has a good effect in lowering uric acid (UA). However, the exact mechanism of RA in treating HUA is still not fully understood.
PURPOSE: To validate the therapeutic effect of RA on HUA and its potential pharmacological mechanisms through a comprehensive analysis of the gut microbiome, metabolomics and molecular interaction techniques.
METHODS: A rat model of HUA was replicated with 10 % fructose water. T…

Purine metabolism in the circulatory system yields uric acid as its final oxidation product, which is believed to be linked to the development of gout and kidney stones. Hyperuricemia is closely correlated with cardiovascular disease, metabolic syndrome, and chronic kidney disease, as attested by the epidemiological and empirical research. In this review, we summarize the recent knowledge about hyperuricemia, with a special focus on its physiology, epidemiology, and correlation with cardiovascular disease. This review also discusses the possible positive effects of treatment to reduce urate le…

Uric acid (UA), the metabolic mediator of gout and urate renal stones, is associated with increased cardiovascular risk burden. Hyperuricemia is a common metabolic disorder, and interaction among UA and cardiovascular diseases has been clearly described. Several illnesses, including hypertension, myocardial infarction, metabolic syndrome, and heart failure, are related to increases in UA levels. In this article, we discuss the pathophysiology of hyperuricemia and describe the biologic plausibility of this metabolite's participation in the pathogenesis of cardiovascular illness. We conclude by …

INTRODUCTION: The unmet need and the growing prevalence of hyperuricemia and its complications worldwide have pushed investigators to identify new agents to manage hyperuricemia.
AREAS COVERED: This review discusses the drugs in preclinical and early clinical trials for hyperuricemia, their mechanisms of action and available results. This article reviews a total of 10 novel agents: i) drugs in Phase II/III trials - arhalofenate (MBX201), AC201, RDEA group of drugs (including lesinurad), tranilast, ulodesine (BCX4208); and ii) drugs in Phase I trials, including levotofisopam, UR1102, KX1151, L…

PURPOSE OF REVIEW: Gout is a painful inflammatory arthritis associated with hyperuricemia, with a prevalence of almost 10 million in the USA. Reduced renal excretion of urate is the underlying hyperuricemic mechanism in the vast majority of gout patients; most of the genes that affect serum urate level (SUA) encode urate transporters or associated regulatory proteins. Acquired influences can also modulate SUA and renal urate excretion, sometimes precipitating acute gout. Coincidentally, the prevalence of renal comorbidities in gout - hypertension, chronic kidney disease (CKD), and nephrolithia…

The discovery of uromodulin mutations as a cause of FJHN and MCKD2 raises a new question; Why the mutant uromodulin causes uricemic underexcretion and hyperuricemia? Moreover, an old and still unsolved question is now highlighted; What is the physiological function of uromodulin? Recent experimental data on intracellular trafficking of uromodulin mutants and histopathological findings on renal biopsy specimens of patients are introduced. Hypotheses on mechanisms of FJHN/MCKD2 and supporting and contrary experimental findings are reviewed.

The objective of this prospective study was to determine the prevalence of hyperuricemia and gout in a sample of Saudi individuals and their relationship to certain risk factors, namely, obesity, serum glucose, triglycerides, cholesterol, age, and sex. A total of 487 Saudis (250 males and 237 females) from 14 primary care clinics were interviewed, examined, and investigated. The mean age for the males was 46.89 +/- 17.01 years (range 14-83) and for the females 45.08 +/- 13.67 years (range 21-80). Serum uric acid (SUA) values above 420 micromol/l for males and 360 micromol/l for females were co…

Hyperuricemia is a common manifestation in various hematological disorders such as hemolytic anemias, megaloblastic anemia malignant lymphomas and leukemias. Tumor lysis syndrome (TLS) is a serious complication consists of hyperuricemia, hyperkalemia, hyperphosphatemia and hypocalcemia due to massive cell lysis, which occurs shortly after the onset of chemotherapy or radiotherapy for acute leukemias and malignant lymphomas. Acute renal failure may develop due to uric acid nephropathy. The risk factors of TLS are heavy tumor cell burden, rapid cell turn over rate and renal involvement of neopla…

Many stress situations such as physical exertion, overheating, alcohol abuse, poisoning with lead, etc. are accompanied by a higher level of uric acid. The question up till what moment hyperuricemia is considered compensatory and under what effect it changes to a pathological state still remains unsolved. Proceeding from the literature data and the results of their own research the authors confirm the importance of regarding hyperuricemia as a risk factor since it is a biochemical marker not only of the disease of the joints but also of the cardio-vascular pathology and other states.

Also in the GDR during the last 10 years the frequency of the disturbances of the purine metabolism increased. With a rate of prevalence of about 20% this anomaly of metabolism reaches the highest percentage of the metabolic disturbances. The increasing accumulation of the disturbances of the purine metabolism on account of complex examinations depends above all on the changed eating and drinking habits of the population. Also influences on body-weight, blood pressure and bodily activities are to be taken into consideration. Sex, age and genetic loads belong to the non-correctable factors of i
