Epidemiology of Primary Immunodeficiency in Iceland.

PURPOSE: Primary immunodeficiencies (PID) are rare heterogeneous diseases. Little is known about the prevalence of PID in Iceland and no national registry exists. The aim of the study was to describe the epidemiology of PID in Iceland. METHODS: Using The European Society's for Immunodeficiencies (ESID) criteria for PID, information about individuals with a known PID between 1990 and 2010 in Iceland were collected from inpatient registries of the National University Hospital of Iceland, the Department of Immunology and from clinical immunologists. Selective IgA deficiency, mannan binding lecti…

[Melioidosis, first four cases in Iceland].

We report the first four cases of Melioidosis treated in Iceland and review the literature. Melioidosis is caused by the saprophytic Gram negative bacteria Burkholderia pseudomallei. Most disease occurs in residents of Southeast-Asia and North-Australia. The most common presentation of Melioidosis is pneumonia but as these cases demonstrate the infection has protean manifestations and B. pseudomallei can infect nearly every organ. It is important to notify the laboratory of the possibilty of Melioidosis as the bacteria can be difficult to identify and poses an infection risk to laboratory staf…

Listeria monocytogenes in horses in Iceland.

Twenty isolates of Listeria monocytogenes associated with five confirmed and four suspected incidents of listeriosis in horses in Iceland were characterised by serotyping, pulsed-field gel electrophoresis and ribotyping. Semiquantitative estimates of the numbers of L monocytogenes were made on faeces from horses with clinical signs of listeriosis and on grass silage fed to them. Large numbers of L monocytogenes were often found in the faeces of horses with severe signs of disease. The 20 isolates could be divided into six genotypes, each incident involving only one genotype. One serovar 1/2a g…

Spinal muscular atrophy. Incidence in Iceland.

Spinal muscular atrophy (SMA) is among the commonest degenerative disorders of the nervous system in childhood. This is an inherited autosomal recessive disease which results in premature death of anterior horn cells of the spinal cord and is manifested by progressive weakness and atrophy of skeletal muscles. Few studies have looked at the frequency of the disease in a defined population. We identified all patients diagnosed with SMA in Iceland during a 15-year period. The diagnosis is based on typical symptoms and supported by results of electromyography/nerve conduction studies and muscle bi…

[Animal and human bites in Iceland.].

Information on incidence, epidemiology and complications of human and animal bites in Nordic countries is limited, but these injuries can carry serious infectious complications. We conducted a prospective study at the Borgarspitalinn Emergency Department (ED) from August 1991 to July 1992. During this time 145 individuals entered the ED due to bites (accounting for ~0.4% of all admissions to the ED); 66 subjects (46%) had been bitten by dogs (41 provoked), 29 (20%) by cats, 34 (23%) by humans and 16 (11%) by other animals (horse, rabbit, hamster, rat, parrot, piranha fish, guinea pig, mink). [Congenital hypothyroidism in Iceland 1979-1993.].

A nationwide screening for congenital hypothyroidism (CHT) was initiated in Iceland on January 1st 1979. From the beginning the screening consisted of TSH measurements from blood samples obtained on the fifth day of life on filter paper. TSH was initially assayed by RIA but since 1990 DELFIA fluoroimmunoassay has been used. During the 15 year period 1979-1993, a total of 65,892 children were born in Iceland. Participation in the screening program was 99.5%. The diagnosis of CHT was established in 21 cases, eight boys and 13 girls or 1/3138. Elevated TSH was found in 33 infants in the initial s…

Studies on herediatary spherocytosis in Iceland.

Thirty members with typical hereditary spherocytosis (HS) and over 90 apparently unaffected members belonging to 12 families have been studied. Splenectomy has been performed on 22 HS patients. Of nine HS individuals, who had not undergone surgical treatment in 1957, four suffered from temporary severe anaemia, presumably due to aplastic crisis associated with influenza. One of them died, a male 18 years of age. Pedigree studies on one of the families indicate that the HS gene or genes have been transmitted through six generations over the past 200 years. Marked deficiency in the number of aff

[The use of endobronchial stents in Iceland].

Diseases that cause progressive obstruction of the larger airways are commonly encountered in pulmonary medicine. Even slight narrowing of the larger airways can cause intense symptoms and sometimes be life-threatening. In recent years progress has been made in endobronchial intervention procedures. A range of technologies has been developed to obtain and maintain patency of the bronchial lumen in disease. This article briefly describes the current use of these methods in Iceland and in some detail the use of stents in airways. The placement of a bronchial stent is a fairly simple procedure th…

Cerebellar abiotrophy in an Icelandic horse.

BACKGROUND: Cerebellar abiotrophy (CA) is an uncommon hereditary neurodegenerative disorder affecting the cerebellar Purkinje cells. Equine CA has been reported in several breeds, but a genetic etiology has only been confirmed in the Arabian breed, where CA is caused by an autosomal recessive mutation. CASE PRESENTATION: Clinical and histological findings consistent with CA are reported in an 8.5-month-old Icelandic filly. The filly showed a perceived sudden onset of marked head tremor, incoordination, ataxia, lack of menace response and a broad-based stance. Cerebrospinal fluid, hematologica…

[Scombroid poisoning at an Icelandic restaurant].

We report four cases of scombroid poisoning. During a lunch meeting three males had the same dish: a club sandwich with raw tuna. All developed erythema over the face and neck within two hours of eating the tuna. The severity of symptoms varied. Other symptoms were profuse sweating, a feeling of intense thirst and palpitations. The symptoms disappeared after few hours. Samples obtained from the tuna revealed histamine levels above the recommended FDA levels. We also report a case with similar symptoms after eating canned tuna in a mixed salad. Scombroid poisoning has been associated with the c

[The use of ulcer medication in Iceland].

In Iceland, the consumption of pharmaceutics for lesions of the gastrointestinal tract is 20 DDD/1,000 inhabitant days, which is 2-3 times more than that of any of the other Nordic countries. A nationwide investigation showed 1.54 per cent of the Icelandic population to be on continuous ulcer medication, consumption being greatest in the 70-79-year-old age group. H2-blockers are used in 79 per cent of cases, omeprazole in 17 per cent, and other drugs in 4 per cent. Of the prescriptions, 65 per cent were issued by GPs, 15 per cent by gastro-enterologists and 14 per cent by other categories of p…
