A survey of 138 patients with systemic lupus erythematosus (SLE) (113 with nephritis) was carried out following the finding of a patient with membranous nephropathy and IgA deficiency who later developed SLE. Two hundred random hospital patients and 143 patients with idiopathic glomerulonephritis were also studied. One patient with minimal change nephrotic syndrome had IgA deficiency, but the other 342 patients had normal IgA concentrations. In the SLE group, in contrast, 4 patients had IgA deficiency and another patient prolonged IgA deficiency following phenytoin treatment. These and other p

Five patients with no detectable serum IgA (less than 20 mug/ml) and one patient with low serum IgA were compared to normal subjects. The number of circulating E-RFC was normal as was the lymphocyte DNA synthesis induced by PHA, Con A, and streptokinase-streptodornase. The patients had normal numbers of IgA-bearing lymphocytes and normal or increased numbers of B cells. Purified anti-immunoglobulin antibodies specific for IgG, IgA and IgM induced a normal lymphocyte DNA synthesis as did PWM. The patients' lymphocytes were able in vitro to transform into actively secreting IgA plasmocytes. This

In a prospective study of thirty-two children with seizures treated with phenytoin (diphenylhydantoin), five had low levels of serum-IgA before treatment. All of these were among the fifteen who had had febrile convulsions in infancy. IgA levels fell significantly during 6 months treatment in the fourteen patients studied sequentially. Treated children with low serum-IgA had normal numbers of lymphocytes with surface IgA. This suggests that phenytoin causes failure of terminal differentiation of B lymphocytes, and is the first known cause of this, the commonest mechanism of immunoglobulin defi

IgA deficiency (IgAD) is the most common immunodeficiency, characterized by an arrest in B cell differentiation. It has a sporadic occurrence or variable inheritance pattern, and is also linked to the HLA genes. IgA deficiency is sometimes associated with IgG subclass deficiency. In this study the Gm allotypes, as genetic characteristics of the IgG1, IgG2 and IgG3, were analysed in 83 Caucasian IgAD individuals. Half of the patients presented with IgG4 < 0.01 g/l compared with 5% (P < 0.001) in a healthy population. Three of the 83 had significantly low IgG2 and four had significantly low IgG3

BACKGROUND: Immunoglobulin A antibodies (anti-IgA) are rare but can cause transfusion-associated anaphylaxis. The detection of anti-IgA has traditionally been performed using a labor-intensive hemagglutination assay in a limited number of reference laboratories.
STUDY DESIGN AND METHODS: Two simple gel card assays are now available that can be used to screen for anti-IgA and IgA deficiency. A total of 24 serum samples that had been previously assayed for anti-IgA over a 3-year period were used to assess the DiaMed anti-IgA and IgA deficiency assays.
RESULTS: The DiaMed assays correctly ident

IgA production from B cells can be driven by three signals (anti-Ig antibody, B cell growth factor and helper T cell factor). It was thought that this three signal system might be a way other than the mitogen-stimulated system, to access B cell differentiation into Ig-producing cells. Seven children with IgA deficiency were investigated by this method. In all cases B cell differentiation was defective when using this three signal system. In one case, IgA producing cells were generated in normal number from B cells when stimulated with pokeweed mitogen in the presence of normal T cells. Definit

Selective IgA deficiency (SIgAD) is the most common primary immunodeficiency but does not always result in clinical disease. This may in part be due to the definition based on serum IgA, while most IgA is secreted at mucosal surfaces, not amenable to measurement. Clinical complications include increased risk of sinopulmonary infections with bacteria and viruses, gastrointestinal infections with a predilection for Giardia lamblia, a myriad of autoimmune diseases including systemic lupus erythematosus, hyper- and hypo-thyroidism, Type 1 diabetes, celiac disease, and rarely, malignancy. OBJECTIVE: Measuring serum tissue transglutaminase immunoglobulin A (tTG IgA) levels is the most widely used screening test for coeliac disease. However, given an increased prevalence of IgA deficiency among coeliac patients there is a risk of false negative results. Hence, in addition to specific serum tTG IgA, screening tests frequently include total IgA levels. The objective of this study was to determine whether tTG IgA antibody levels might be used to predict IgA deficiency and hence avoid unnecessary testing of total IgA levels in all individuals.
DESIGN: Retrospective analysis of 9429

Crusted scabies, an unusual clinical variant of human scabies mite infestation, is usually reported in cases of gross debility, mental deficiency, or immunosuppression. We report here the occurrence of crusted scabies in a 40-year-old man with acquired selective IgA deficiency suspected to be caused by long-term medication with phenytoin for epilepsy.

The authors report one case of Moya-Moya disease in a 3 month-old infant, who developed microcephaly, mental retardation and tetraparesis. The appearance in the following months of a Raynaud's phenomenon in the upper limbs and of IgA deficiency may be of interest for the understanding of the disease.

A case of an IgA deficient child who developed several autoimmune diseases namely, celiac disease, pernicious anemia, autoimmune thyroiditis and autoimmune thrombocytopenic purpura is reported; the possible connections and etiology of these phenomena is briefly discussed. Individuals with selective IgA deficiency should no longer be considered "normal".

Sera, intestinal secretions and intestinal biopsies were collected from twelve biopsy-proved lepromatous leprosy patients with the help of a capsule invented by Roy Choudhury. Sera from another twenty-five proved lepromatous cases were also included. Sera and intestinal aspirates from twenty-five normal subjects and twenty patients with intestinal tuberculosis were also taken as controls. Acid-fast organisms, morphologically resembling Mycobacterium leprae, were detected in the intestinal aspirates of only two leprosy patients. Immunoglobulin levels in their sera and elevation secretions were

24 cases of selective IgA deficiency were found among 2,650 children studied in an immuno-Allergy service. The population studied must therefore be considered highly selected. The Amman and Hong criteria were used as a basis for diagnosis (Table 1). Even though they are currently being revised, they are still on some practical value. Statistically, the incidence of selective IgA deficiency in this study is higher (0.905%) than in other publications. Respiratory symptoms were the most important. Only one case presented digestive symptoms (resembling coeliac disease), and no cases with autoimmun

This study included 36 children with IgA-deficiency, increased susceptibility to infections and/or other disorders. Recurrent, usually bacterial infections were noticed in 23 out of 26 patients (88%) with complete and in 7 out of 10 patients (70%) with partial IgA-deficiency. All patients with severe infections had complete IgA-deficiency. Complete IgA-deficiency was also present in the six children who had autoimmune disorders associated with recurrent infections. In 22 out of the 36 patients studied the serum could be analysed for concomitant IgG subclass deficiencies: one patient had marked

Immunoglobulin A (IgA) deficiency is the most common type of primary immunodeficiency. When a patient receives a blood product transfusion, anti-IgA antibodies are formed. Second transfusion may sometimes cause an anaphylactic reaction, thus caution is necessary. Reported here is a case of Stanford type A acute aortic dissection performed in the patient with IgA deficiency with a history of blood transfusion. Red blood cells and platelet were washed and prepared, and flesh frozen plasma from IgA deficient donors was obtained. Thereafter, the surgery was safely performed.

Background: Imunoglobulin A (IgA) deficiency (IgAD) is the most common form of primary immunodeficiency in Western countries. There have been several reports on IgAD complicated by glomerulonephritis in adults, but only very few cases of IgAD with nephropathy have been reported in children. We present two cases of IgAD with relapsing nephrotic syndrome in pediatric age. Case presentation: A 4-year-old boy and a 2-year-old boy presented with bilateral periorbital oedema and weight gain. The results of laboratory tests revealed IgAD (IgA < 7 mg/dL), normal creatinine, hypoprotidaemia, hypoalbumi
