BACKGROUND: The first case of inguinal endometriosis was described by Cullen. Endometriosis in the round ligament could be in the pelvic or inguinal area and is a rare disease occurring in 0.6% of women. Women with inguinal endometriosis have a painful inguinal mass during menstrual cycles and they mostly have a history of surgery. The right side is more commonly involved in inguinal endometriosis than the left side (90-94%). A history of gynecologic or abdominal surgery is common in women with inguinal endometriosis.

Inguinal canal can have diverse range of pathologies due to its complex anatomy and multiple different contents in the males and females. Imaging plays a dominant role in the appropriate diagnosis and management as inguinal pathologies have fairly similar clinical presentation with pain and swelling. Various pathologies in the inguinal region include congenital, infectious/inflammatory, vascular, neoplastic and iatrogenic. This paper gives an overview of the role of various modalities like Ultrasound, CT and MRI in the evaluation of inguinal pathologies.

Thirty-five patients with clinically N0 cancers of the canal anal, 33 epidermoid carcinomas et 2 melanomas were histologically staged with inguinal sentinel lymph node biopsy (ISN). With the combined technique, blue dye and radiocolloid the ISN was identified in 100% of the cases. The ISN was invaded in 7 cases/33 for epidermoid tumors and 2/2 for melanomas. After 18 months of follow-up, no inguinal recurrence could be seen in ISN pN0 cases. In conclusion, ISN biopsy is a reliable procedure to stage anal canal cancers. We report a case of a paraplegic patient with an indwelling Foley catheter and epididymo-orchitis leading to a scrotal pyocele. This case is unique in that the patient's physical examination revealed both inguinal and scrotal abnormalities that mimicked a strangulated inguinal hernia. Imaging studies and surgical exploration helped elucidate the exact diagnosis. Although rare, this entity requires emergent surgical consultation. We will also discuss the etiology, pathophysiology, and treatment of scrotal abscesses.

Adenomyoma originating from the round ligament of the uterus represents a remarkably uncommon pathological entity. We report the case of a female adolescent who had an ectopic ovary in the inguinal canal without an associated hernia, a unicornuate uterus, and ipsilateral renal agenesis. The incidental discovery of the ectopic ovary and other Mullerian anomalies, as well as the surgical correction that followed, highlights important fertility considerations in children and available treatment algorithms for these rare cases.
