Increased insulin responsiveness and insulin clearance in thyrotoxicosis.

The euglycemic insulin clamp technique was used to evaluate in vivo insulin action in thyrotoxicosis (TTX). Insulin was infused at 0.5, 3 and 10 mu/kg per min, and the steady-state glucose infusion rate was determined in five TTX patients with fasting hyperinsulinemia and five healthy subjects. TTX is accompanied by a significant increase in insulin responsiveness, clearance and basal insulin delivery rate. Therefore, insulin resistance in TTX does not reside at the postreceptor peripheral tissue level, but may be the consequence of increased hepatic glucose production.

The degradation of monoiodotyrosyl insulin isomers by insulin protease.

The four single-site monoiodotyrosyl insulin isomers were synthesized by lactoperoxidase-catalyzed iodination of porcine insulin and were separated from one another by high performance liquid chromatography. The susceptibility of the four isomers (A14-, A19-, B16-, and B26-monoiodotyrosyl insulin) to degradation by purified insulin protease was examined using several different assay methods, including trichloroacetic acid precipitation, immunoprecipitation, and Sephadex G-50 chromatograpy. Using trichloroacetic acid precipitation, isomer susceptibility, determined from the initial rate of hydr…

Cerebral insulin, insulin signaling pathway, and brain angiogenesis.

Insulin performs unique non-metabolic functions within the brain. Broadly speaking, two major areas of these functions are those related to brain endothelial cells and the blood-brain barrier (BBB) function, and those related to behavioral effects, like cognition in disease states (Alzheimer's disease, AD) and in health. Recent studies showed that both these functions are associated with brain angiogenesis. These findings raise interesting questions such as how they are linked to each other and whether modifying brain angiogenesis by targeting certain insulin signaling pathways could be an eff…

Insulin treatment in non-insulin dependent diabetes mellitus.

Non-insulin dependent diabetes mellitus (NIDDM) is an important cause of morbidity and mortality, both in developed as well as in developing countries. The eyes, kidneys, and cardiovascular and neurological systems are predominantly affected by this chronic disease, leading to loss of employment and sometimes life. The most common setting is uncontrolled glycaemia after dietary restriction, exercise and oral hypoglycaemic drug therapy. Insulin may be needed for treating NIDDM some time during the course of the disease. Insulin therapy results in improved beta-cell function, a decrease in the h…

Adequately adapted insulin secretion and decreased hepatic insulin extraction cause elevated insulin concentrations in insulin resistant non-diabetic adrenal incidentaloma patients.

BACKGROUND: Insulin-resistance is commonly found in adrenal incidentaloma (AI) patients. However, little is known about beta-cell secretion in AI, because comparisons are difficult, since beta-cell-function varies with altered insulin-sensitivity. OBJECTIVES: To retrospectively analyze beta-cell function in non-diabetic AI, compared to healthy controls (CON). METHODS: AI (n=217, 34%males, 57 ± 1 years, body-mass-index:27.7 ± 0.3 kg/m(2)) and CON [n = 25, 32%males, 56 ± 1 years, 26.7 ± 0.8 kg/m(2)] with comparable anthropometry (p ≥ 0.31) underwent oral-glucose-tolerance-tests (OGTTs) with gl…

Insulin secretion and insulin action in non-insulin-dependent diabetes mellitus: which defect is primary?

Defects in both insulin secretion and insulin action exist in patients with non-insulin-dependent diabetes mellitus (NIDDM). The loss of the acute plasma insulin response to intravenous glucose is seen in patients with relatively mild degrees of fasting hyperglycemia, but patients with severe fasting hyperglycemia also demonstrate absolute hypoinsulinemia in response to an oral glucose challenge. In contrast, day-long circulating insulin levels are within normal limits even in severely hyperglycemic patients with NIDDM. The relationship between NIDDM and insulin action in NIDDM is less complex…

Review of the Next Generation of Long-Acting Basal Insulins: Insulin Degludec and Insulin Glargine.

Older adults are at an increased risk of developing type 2 diabetes mellitus (T2DM). Although oral agents (i.e., metformin) are the preferred first-line therapy, older adults often eventually require the addition of insulin to control their blood glucose. Long-acting insulin analogues are the preferred insulin products for older adults with T2DM. Insulin degludec and insulin glargine U-300 are both new generation long-acting insulins. When compared with the standard of care, long-acting insulin product insulin glargine U-100, insulin degludec, and insulin glargine U-300 had similar glucose-low…

Insulin secretion, insulin sensitivity and hepatic insulin extraction in primary hyperparathyroidism before and after surgery.

OBJECTIVE: Primary hyperparathyroidism (pHPT) is associated with hypertension, hyperinsulinaemia, and insulin resistance. The present study investigated the causes of these metabolic disturbances by quantifying insulin sensitivity and glucose effectiveness, and by assessing the time course of beta-cell insulin secretion and hepatic insulin extraction, during a dynamic condition such as after an intravenous glucose load. In addition, we evaluated the possible link between metabolic disorders and high blood pressure. SUBJECTS: We studied 16 patients with pHPT, before and 12 weeks after parathyr…

Localized skin reactions to insulin: insulin lipodystrophies and skin reactions to pumped subcutaneous insulin therapy.

Patients treated with insulin can develop a variety of adverse cutaneous and subcutaneous reactions localized at sites of insulin injection. The reported frequency, severity, and type of skin reactions to insulin are varied, as are the theories relating to their etiology and recommendations for treatment. In this review, several types of localized skin reactions to insulin injection will be described. Particular attention will be devoted to insulin lipoatrophy and skin reactions in patients using mechanical devices for continuous subcutaneous insulin infusion. Recent advances in diabetes care …

Insulin dysregulation.

Abnormalities of insulin metabolism include hyperinsulinaemia and insulin resistance, and these problems are collectively referred to as insulin dysregulation in this review. Insulin dysregulation is a key component of equine metabolic syndrome: a collection of endocrine and metabolic abnormalities associated with the development of laminitis in horses, ponies and donkeys. Insulin dysregulation can also accompany prematurity and systemic illness in foals. Causes of insulin resistance are discussed, including pathological conditions of obesity, systemic inflammation and pituitary pars intermedi…

Insulin therapy.

Optimal glycemic control in type 1 diabetes mellitus (T1DM) requires Intensive Insulin Therapy. Implementation of intensive therapy should be early and prolonged as suggested by the results of Diabetes control and complications trial and Epidemiology of Diabetes Interventions and Complications (EDIC) study. Proper implementation of intensive therapy requires a course teaching flexible intensive insulin treatment combining dietary freedom and insulin adjustment as shown by the Dose adjustment for normal eating (DAFNE) randomized controlled trial. Pen injectors appear to be feasible for routine …

Insulin allergy.

While immunologic responses to insulin such as cutaneous reactivity and antibody against insulin are common significant clinical problems related to those immunologic reactions are uncommon. Immunologic responses that have been described included Gell and Coombs type I, II, and IV. The antigen may be the result of any of several factors: insulin as a heterologous protein, altered tertiary structure of insulin, presence of non-insulin protein contaminants, or pharmaceutical formulation additives. IgE mediated local reactions are the most common and almost always subside spontaneously. IgE media…
