PURPOSE: To report a surgical technique for aqueous misdirection refractory to medical treatment consisting of combined pars plana vitrectomy, hyaloido-zonulectomy, and peripheral iridectomy.
DESIGN: Noncomparative case series.
METHODS: The charts of 5 pseudophakic patients who sought treatment for aqueous humor misdirection refractory to medical treatment from May 2008 trough February 2009 were reviewed. All 5 patients underwent anterior vitrectomy, hyaloido-zonulectomy, and peripheral iridectomy with an anterior vitrector through a pars plana incision. Main outcome measures were preoperati…

In this retrospective study we evaluated 154 patients with primary angle-closure glaucoma operated on by peripheral iridectomy to determine the factors contributing to the high occurrence (57%) of cataracts. Cataract formation in an eye operated on by peripheral iridectomy was age-dependent and less related to the time after surgery. The acute attack enhanced the appearance of the lenticular changes but did not influence the overall occurrence of cataracts in older patients.

BACKGROUND: In this study, we aimed to report the short-term (6 months) effects on visual functionality and safety of femto-laser assisted s … on improvement, without serious postoperative complications. We recommend intraoperative mechanical iridectomy in order to easily manage post-operative IOP and to avoid sudden IOP rise with its possible consequences. These good results can be a hope to partially improve the quality of life of patients suffering from severe end stage macular atrophy.

Epithelial cysts run a high risk of recurrence and conversion to sheet-like ingrowth after surgical intervention. In this retrospective study, we introduced a modified iridectomy for treatment of secondary epithelial iris cysts (EICs) in the anterior chamber. Twenty-nine patients (29 eyes) aged 2-61 years received "open iridectomy" for EICs between April 1995 and July 2019. After viscodissection, most of the cyst wall was cut using a 20-gauge aspiration cutter via a 2.5-mm clear corneal incision. The residue closely adhering to the iris stroma was remained to avoid photophobia and diplopia. At…

A retrospective study of 107 cases of retinal detachment in aphakic and pseudophakic eyes showed different types of retinal break depending upon surgical method. Three techniques were studied: intracapsular extraction (ICCE), intracapsular extraction with anterior chamber lens implant (ICCE, AC-IOL), and extracapsular extraction with posterior chamber lens implant (ECCE, PC-IOL). Results were compared for each technique with and without peripheral iridectomy. Typical aphakic and pseudophakic retinal breaks were found only in eyes having undergone peripheral iridectomy: our conclusion is that p…

We present a case of acute angle closure that occurred after insertion of an implantable contact lens (ICL). The apparent papillary-block angle closure did not resolve after 2 patent iridotomies and a surgical iridectomy, but did respond to pupil dilation (not constriction). Ultrasound biomicroscopy revealed abnormally large and irregular ciliary processes that may have contributed to the unusual behavior of the ICL-iris complex. The condition resolved after the ICL was replaced by one with a smaller haptic diameter. Routine ultrasound biomicroscopic assessment of the ciliary body anatomy preo…

In 67 eyes of 46 patients with chronic primary angle closure glaucoma, the effect of peripheral iridectomy in lowering the intraocular pressure was investigated retrospectively. In the majority of cases the pressure was reduced. For the whole population the mean reduction was 3.9 mmHg (SD 6.7 mmHg). The presence or extent of peripheral anterior synechiae showed no significant correlation with the amount of pressure reduction. A peripheral iridectomy is still recommended as first step in the treatment of primary angle closure glaucoma.

In 69 of 107 patients with primary angle-closure glaucoma, it was possible to control intraocular pressure and visual fields by means of peripheral surgical iridectomy; in 17 of these patients topical glaucoma medication was required in addition. In 11 cases a second surgical operation other than iridectomy was necessary: 6 lensectomies for malignant glaucoma in 2 cases and lens subluxation or phacomorphic glaucoma in 4 and 5 filtering procedures. Among the 85 treated fellow eyes 68 were controlled by iridectomy alone and 17 eyes needed additional topical glaucoma medication. One case of persi…

In an attempt to compare surgical peripheral iridectomy and laser iridotomy with regards to the long-term control of the intraocular pressure (IOP), analyses were carried out of the clinical records of 195 eyes of 149 primary angle-closure glaucoma patients who had undergone either peripheral iridectomy or laser iridotomy and were followed up for at least one year. One hundred and ten eyes underwent peripheral iridectomy and 85 eyes had laser iridotomy. The postoperative IOP not exceeding 20 mmHg was taken as the criterion for success. The iridectomized eyes showed no significant difference fr…

Studies with a camera and a model eye showed that the image produced by peripheral light rays is inferior to the image produced by paraxial rays. In a retrospective clinical study, we determined the best corrected visual acuity in 33 patients (52 eyes) who had had iridectomy (group I) or aspiration (group II) or both for central cataracts. Vision was significantly better (p less than 0.01) in group II than in group I, and postoperative complications of aspiration were minimal. Some patients had unilateral amblyopia or pendular nystagmus. We conclude that if vision is to be improved with a sing…

PURPOSE: To assess the role of surgical peripheral iridectomy (PI) in preventing iris-related complications associated with glued intraocular lens (GIOL) surgery in children with bilateral ectopia lentis.
METHODS: Nonrandomized interventional case series of 34 eyes of 17 children (<15 years of age) who underwent pars plana lensectomy (PPL) and GIOL surgery between January 2013 and December 2016. Eyes with surgical PI (January 2013-June 2015) were compared with those without surgical PI (July 2015-December 2016). The primary outcome measure of the role of surgical PI in GIOL surgery was to acc…

PURPOSE: After Descemet Membrane Endothelial Keratoplasty (DMEK), the air-gas compound in the anterior chamber can lead to a postoperative increase in intraocular pressure (IOP) up to pupillary block. YAG laser iridotomy (IO) or surgical iridectomy (IE) is performed to prevent these painful and sight-threatening elevations. We aimed to compare the safety profiles of the two procedures.
METHODS: We included a total of n = 196 eyes of N = 178 patients (55.6% female and 44.4% male) who underwent DMEK. Of these, 124 eyes received intraoperatively an IE (63.3%, group IE) and 72 an IO one day befor…

Residual primary angle-closure glaucoma (PACG) after iridectomy is an important issue among Asians, especially Chinese. In this study, we tested the effectiveness of latanoprost as an intraocular pressure (IOP) lowering agent in cases of residual PACG. Twenty-six eyes of 26 PACG patients with persistently elevated IOP after iridectomy, despite treatment with conventional IOP lowering drugs (beta blockers and pilocarpine) were included. Latanoprost 0.005%, one drop daily, was added adjunctively to all eyes. Measurement of IOP at baseline and after the start of treatment with latanoprost indicat…

OBJECTIVE: To investigate if apraclonidine can effectively prevent the acute increase of intraocular pressure (IOP) in brown eyes with Nd:YAG laser iridectomy.
METHODS: 48 eyes with primary angle closure glaucoma were divided into treatment and control groups, which were matched by age and sex. One drop of apraclonidine was applied into the conjunctival sac at one hour before laser iridectomy and immediately after laser iridectomy in treatment group, whereas one drop of antibiotic as placebo was applied in control group at the same time. IOP, pupil diameter were measured at 0.5, 1, 1.5, 2, 3.…

A 25-year-old man had a penetrating injury to the left eye. Ocular examination revealed a full-thickness corneal laceration, total aniridia, anterior capsule rupture, and microscopic hyphema. Ten days after the immediate primary-repair surgery, aspiration of the lens secondary to traumatic cataract was performed. Four months later, a black diaphragm intraocular lens was implanted. Postoperatively, the best corrected visual acuity was 20/20 in both eyes. Multifocal spectacles were prescribed to provide the near and far vision required for the patient's job. Normal intraocular pressure and accep
