Anatomical Variants of the Jejunal Veins and Their Technical Implications in Pancreaticoduodenectomy: A Systematic Review and Meta-Analysis.

INTRODUCTION: One of the most common causes of bleeding during pancreaticoduodenectomy is a dissection of the pancreatic head from the superior mesenteric vein and superior mesenteric artery. Knowledge of the anatomical variants of the veins draining the proximal jejunum may allow better control of bleeding during detachment of the uncinate process and pancreatic head from the mesenteric pedicle and division of the mesopancreas. The aim of this systematic review and meta-analysis was to evaluate the anatomical variations of the first jejunal vein (FJV) and jejunal trunk (FJT). METHODS: Fourte

Ruptured jejunal gastrointestinal stromal tumor (GIST) presenting with acute hemoperitoneum in a young adult with autosomal dominant polycystic kidney disease (ADPKD).

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms of the gastrointestinal tract, most commonly arising in the stomach and less frequently in the small intestine. Jejunal GISTs often present with vague symptoms, leading to delayed diagnosis, while acute presentations with hemodynamic instability are uncommon and usually result from tumor necrosis or rupture. We report a 37-year-old man with autosomal dominant polycystic kidney disease who presented with acute abdominal pain and shock requiring immediate resuscitation and intensive care admission. Ultrasonography revealed fr

Jejunal adenocarcinoma with isolated mesorectal metastasis: a case report of a rare entity.

Small bowel (SB) neoplasms are rare and often diagnosed at advanced stages, commonly presenting with obstruction or obscure gastrointestinal bleeding (OGB). An 83-year-old man presented with anemia and hematochezia. Imaging revealed an 8 cm SB mass and a 5 cm rectal lesion initially suggestive of GIST. Biopsy confirmed adenocarcinoma. Due to recurrent bleeding, the patient underwent segmental enterectomy and Hartmann's procedure. Histology showed jejunal mucinous adenocarcinoma with mesorectal metastasis (pT4N0M1). Despite adjuvant chemotherapy and hemostatic radiotherapy for pelvic recurrence

Jejunal intussusception due to malignant phyllodes tumor of the breast.

Phyllodes tumors are rare fibroepithelial neoplasms of the breast; classified as benign, borderline, or malignant based on the mitotic activity, cellular atypia, and stromal overgrowth. Wide surgical excision is the treatment of choice. The most common locations for metastasis are lung, bone, and liver; small intestinal metastasis is extremely rare. Here we present the first patient with jejunal metastases and intussusception due to malignant phyllodes tumor of the breast. Adjuvant treatment of malignant phyllodes tumor needs to be investigated.

[A rare cause of bleeding intestinal intussusception in adult: jejunal lipoma].

Small bowel neoplasms are usually diagnosed when patients are referred with complications such as hemorrhage or obstruction. Intestinal lipomas are the third most frequent benign tumors among all intestinal neoplasms and their definite diagnosis is usually obtained after histopathological examination of the resected specimen. A 76 year-old male patient with partial intestinal obstruction and rectal bleeding at admittance was found to have an abdominal mass on ultrasound. The density pattern of the mass at abdominal CT was supporting a jejunal lipoma leading an intussusception. Then an enterocl

Metastatic osteosarcoma causing intussusception.

Osteosarcoma is the most common malignant bone neoplasm. It has a very high metastatic potential, and lungs are the most common site of metastasis followed by bones. Multiagent chemotherapy has been reported to alter the biological behavior of the disease so that the tumor metastasizes to more rare sites such as brain, bowel, and soft tissues. The authors report a case of jejuno-jejunal intussusception caused by metastases from osteosarcoma.

Safety and function of isolated Roux loop pancreaticojejunostomy after Whipple's pancreaticoduodenectomy.

A novel method of pancreatic anastomosis after proximal Whipple-type resection: classical pancreaticoduodenectomy (PD) or pylorus-preserving pancreaticoduodenectomy (PPPD), has been evaluated over a 5-year period from 1987 to 1992 in 52 patients. Indications for resection included chronic pancreatitis (n = 9) and neoplasms (n = 43). Reconstruction involved a cephalad end-to-end duodeno-/gastro-jejunal anastomosis with a biliary anastomosis 6-8 cm downstream. A separate isolated defunctioned Roux loop was used to construct a duct-to-mucosa (Wirsung-jejunal) pancreaticojejunostomy. Median postop

A jejunal gastrointestinal stromal tumor with massive gastrointestinal hemorrhage treated by emergency surgery: A case report.

BACKGROUND: A jejunal gastrointestinal stromal tumor (GIST) is a rare neoplasm of the gastrointestinal (GI) tract. Massive bleeding due to a jejunal GIST is a diagnostic and therapeutic challenge. It may be a life-threatening GIST complication that requires urgent intervention. Acute GI bleeding, which requires urgent surgical intervention, is a very rare clinical manifestation of GIST. A jejunal GIST with massive hemorrhage with coronavirus disease 2019 in a male patient in older age with many comorbidities has been not reported in the worldwide literature. METHODS: In this case report, we p

Small bowel adenocarcinoma as first presentation of coeliac disease.

Coeliac disease is a chronic inflammatory disease of the gut with increased risk of gastrointestinal malignancy. Although enteropathy T-cell lymphoma is the most common neoplasm in coeliacs, an increased frequency of small bowel carcinoma has been described. A case is described of jejunal carcinoma as first presentation of coeliac disease, in which gastrointestinal and extraintestinal symptoms of disease developed only after surgical resection and disappeared after gluten withdrawal.

Perforated Jejunal Gastrointestinal Stromal Tumor in a Young Male Patient With Acute Peritonitis: A Case Report and Literature Review.

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms of the gastrointestinal (GI) tract, typically diagnosed in older adults. Jejunal GISTs with spontaneous perforation leading to acute peritonitis are uncommon, particularly in young individuals. We report the case of a 32-year-old male who presented with acute abdominal pain, vomiting, and peritoneal signs. Contrast-enhanced computed tomography (CT) revealed pneumoperitoneum and a heterogeneously enhancing mass at the jejunoileal junction, raising suspicion for a perforated GIST. Emergency exploratory laparotomy confirmed a

Optimal Lymphadenectomy of the Mesopancreas Based on Fluorescence Imaging During Pancreaticoduodenectomy.

BACKGROUND: Excision of the mesopancreas with lymphadenectomy is an important component of pancreatoduodenectomy. However, the optimal extent of lymphadenectomy remains unclear. Furthermore, accurate description of the mesopancreatic lymphatic pathways is difficult, probably because of the complex anatomy. Intestinal derotation simplifies the anatomy and facilitates both examination of lymphatic flow and the surgical procedure. The aim of this study was to evaluate lymphatic flow in the mesopancreas using indocyanine green fluorescence imaging with an intestinal derotation technique, and to cl

Gangliocytic paraganglioma leading to duodeno-jejunal intussusception: A case report.

The intussusception of the small bowel is rarely encountered in adult patients and is frequently associated with a lead point that is often malignant. In a 69-year-old female patient with an episode of gastrointestinal (GI) bleeding, computed tomography (CT) showed a duodenal-jejunal intussusception caused by an intraluminal mass. Open polypectomy and reduction of intussusception were performed and the diagnosis of gangliocytic paraganglioma was made at pathological evaluation. It would be important to consider neoplasms like gangliocytic paraganglioma in the setting of adult small bowel intus

A rare presentation of retroperitoneal liposarcoma presented with jejunal intussusception: An interesting radiological findings.

A jejunal intussusception as a presentation of retroperitoneal liposarcoma (RLPS) is a rare occurrence. The majority of RLPS are presented as an abdominal mass, however, having a jejunal obstruction is an interesting case. The aim is to describe the management of jejunal intussusception secondary to atypical lipomatous tumours with concurrent RLPS. A 61-year-old lady presented with a sudden onset of intestinal obstruction with 1 month of constitutional symptoms and an enlarging right lumbar mass. Computed tomography showed a small bowel intussusception with diffuse peritoneal and retroperitone

CT features of abdominal plasma cell neoplasms.

The aim of this study was to describe the CT features of abdominal plasma cell neoplasms. We reviewed CT imaging findings in 11 patients (seven men, four women; mean age 62 years) with plasma cell neoplasms and abdominal involvement. Helical CT of the entire abdomen and pelvis was performed following intravenous administration of contrast material. Images were analyzed in consensus by two radiologists. Diagnoses were made from biopsy, surgery and/or clinical follow-up findings. Multiple myeloma was found in seven patients and extramedullary plasmacytoma in four patients. All patients with mult

[Pleomorphic liposarcoma arising in an unusual site. A case report].

Liposarcomas are the most common group of malignant mesenchymal neoplasms. They usually occur in the extremities and the retroperitoneum and only rarely in the intestine. We report the case of a 75-year-old man presenting with severe anaemia. A CAT scan revealed a 5cm thickening of the jejunal loop, arousing the suspicion of a neoplasm. A partial resection of the small intestine was performed. Macroscopy showed a 12×6cm ulcerated, polypoid mass. Microscopically, a well circumscribed, ulcerated tumour was seen, which had well-defined margins with the surrounding normal mucosa. It consisted of p

Jejunal Gastrointestinal Stromal Tumor Presenting as Life-Threatening Melena and Profound Anemia: A Case Report.

Gastrointestinal stromal tumors (GISTs) are rare mesenchymal neoplasms and the most common sarcomas of the gastrointestinal tract. Small bowel GISTs, particularly jejunal tumors, frequently present with occult or overt bleeding leading to anemia, but are uncommon causes of melena when routine endoscopy has not been performed. We report a 67-year-old man who presented with one week of intermittent melena and profound anemia (hemoglobin 5.3 g/dL). Computed tomography (CT) angiography revealed a 5 cm jejunal mass without active bleeding. The patient underwent exploratory laparotomy with segmental
