Ketoconazole therapy for endemic blastomycosis.

Amphotericin B is effective in therapy for blastomycosis but causes a number of serious adverse reactions. Because ketoconazole has in-vitro activity against Blastomyces dermatitidis, we administered this agent in a dosage of 400 mg/d to 46 patients with blastomycosis, with 43 patients receiving at least 1 month of therapy. Thirty-five patients had cure without relapse over a mean follow-up of 17 months. Six had a relapse of infection but 4 of these had been noncompliant with therapy. Two patients improved initially but ultimately had progression of disease despite maintenance of adequate seru…

Baboon syndrome induced by ketoconazole.

A 27-year-old male patient presented with a maculopapular eruption on the flexural areas and buttocks after using oral ketoconazole. The patient was diagnosed with drug-induced baboon syndrome based on his history, which included prior sensitivity to topical ketoconazole, a physical examination, and histopathological findings. Baboon syndrome is a drug- or contact allergen-related maculopapular eruption that typically involves the flexural and gluteal areas. To the best of our knowledge, this is the first reported case of ketoconazole-induced baboon syndrome in the English literature.

Ketoconazole Therapy in Pityriasis Versicolor.

Twenty six cases having pityriasis versicolor, were treated with 10 and 14 days courses of oral ketoconazole. Cure rate was higher with the 14 day schedule than with the 10-day schedule. Recurrence was observed in one patient. No significant side effect was noticed except in one patient who developed intolerable nausea after the first dose.

Ketoconazole treatment in superficial mycoses.

Ketoconazole is a new antifungal drug, used for oral treatment, with a broad spectrum of activity. It is a member of the imidazole series and is active in superficial and deep mycotic diseases, in candidosis and pityriasis versicolor. This new imidazole derivative has been successfully administered in several cases of dermatophytosis, yeasts and pityriasis versicolor in the authors' clinic, as well as in many other research centres. Treatment has been given to patients with tinea corporis, tinea cruris, tinea pedis, with or without location in interdigital spaces, while the cases of tinea capi…

Disseminated blastomycosis treated with ketoconazole.

A patient with disseminated blastomycosis gave birth to an infant who died three weeks later of diffuse pulmonary blastomycosis. The infant most likely acquired the disease through intrauterine transmission. The patient's extensive skin lesions responded dramatically to ketoconazole.

Toxic hepatitis following ketoconazole treatment.

Ketoconazole, a new broad-spectrum oral antifungal agent, has proved most useful in the treatment of superficial and systemic fungal infections. Chronic conditions like mucocutaneous candidiasis can be treated with but few side effects. An increasing number of cases have, however, been noted of severe toxic hepatitis, following ketoconazole treatment. A new case (a 61-yr-old woman treated for onychomycosis) is reported here.

Cutaneous leishmaniasis: treatment with ketoconazole.

A 17-year-old boy with an eight-month history of cutaneous leishmaniasis on the left earlobe was treated with ketoconazole for a period of four weeks. The patient showed excellent improvement both clinically and protozoologically. Ketoconazole, a new imidazole bound covalently to piperazine, has been developed as a broad spectrum antimycotic agent and appears to be a good antileishmanial drug.

Oral ketoconazole therapy for keratomycosis.

Two patients, a 48-year-old woman and a 44-year-old man, with fungal keratitis were treated with orally administered ketoconazole, a new antifungal agent. In both cases healing and regression of the corneal ulcer began four or five days after initiation of ketoconazole treatment (300 mg/day). The clinical signs of corneal infection disappeared after eight (Case 1) and three (Case 2) weeks and visual acuities improved markedly (from counting fingers at 5 cm to 20/30 in Case 1 and from 20/30 to 20/16 in Case 2). The levels of serum glutamic oxaloacetic transaminase and serum glutamic pyruvic tra…

[Ketoconazole-induced hepatitis. Case report].

Since Oct. 1981 a new systemic antifungal drug Ketoconazole is available in the Federal Republic of Germany that has proven effective even in severe cases with fungal infections. This case-study will call attention on a rare but important side effect, namely Ketoconazole induced hepatitis. As an acute icteric viral hepatitis, type Non-A-Non-B-hepatitis possibly misdiagnosed only a carefully compiled history of the recent intake of drugs points at the real cause of hepatitis. In our case-report we observed a considerable increase in serum enzymes, especially GOT, GPT and GLDH after a drug-chall…

Ketoconazole blocks adrenal steroid synthesis.

Ketoconazole, a broad-spectrum, antifungal drug that is administered orally, has been shown to inhibit sterol synthesis in fungi. When gynecomastia developed in some patients taking this drug, we investigated the effects of ketoconazole on steroid synthesis in humans and in isolated adrenal cells from rats. In healthy humans, the cortisol response to adrenocorticotropic hormone was significantly blunted 4 hours after a 400-mg or 600-mg dose. The inhibition persisted for up to 8 hours and was absent by 16 hours. This finding indicated that adrenal androgen response was reduced. Easily achieved …

The Rise and Fall of Oral Ketoconazole.

BACKGROUND: Ketoconazole was the first broad-spectrum oral antifungal agent available to treat systemic and superficial mycoses. Evidence of hepatotoxicity associated with its use emerged within the first few years of its approval. Growing evidence of serious side effects including endocrine dysregulation, several drug interactions, and death led to the review of oral ketoconazole in 2011. OBJECTIVE: This article chronicles the use of oral ketoconazole from its introduction to its near replacement in medicine. CONCLUSION: Due to its hepatotoxic side effects, oral ketoconazole was withdrawn f…

The role of ketoconazole in seborrheic dermatitis.

Although the prominent broad-spectrum activity of ketoconazole was reported in the early 1980s, its effect against Malassezia species was most pronounced; thus, it was developed for the treatment of various skin infections in which a link with these fungal species was proposed. Later, a number of ancillary properties were described for ketoconazole that comprised its anti-inflammatory, antiseborrheic, and antiproliferative profile. The incorporation of ketoconazole in an adapted vehicle could further promote its efficacy. Recently, a new formulation-an anhydrous gel containing ketoconazole 2%-…

Effects of ketoconazole in hirsute women.

To determine the efficacy of ketoconazole in the treatment of hirsutism, clinical and hormonal effects of this agent were evaluated with a randomized, placebo-controlled, double-blind cross-over study design. Nine hirsute women were given ketoconazole (600 mg/day) or placebo for 6 months and then crossed over. The severity of hirsutism was assessed according to the scale of Ferriman & Gallwey. Baseline serum testosterone, dehydroepiandrosterone sulphate, progesterone, estradiol, basal and stimulated cortisol and 17-alpha hydroxyprogesterone were measured. Blood was also drawn for FSH and LH le…

Treatment of pulmonary sporotrichosis with ketoconazole.

Antifungal therapy that included ketoconazole failed in a 44-year-old woman with pulmonary sporotrichosis progressing slowly over a seven-year period. On the basis of this case and the modest amount of experience reported in the literature, ketoconazole does not appear to be effective in the treatment of pulmonary sporotrichosis.
