PURPOSE: To evaluate transdiverticular percutaneous nephrolithotomy (TDPN) with creation of a neoinfundibulum in the treatment of caliceal diverticular stones.
PATIENTS AND METHODS: Between 1990 and 1998, 18 patients with symptomatic calculi in caliceal diverticula underwent TDPN. Transdiverticular puncture into the renal collecting system with creation of a neoinfundibulum was used, eliminating the need for prolonged probing with a wire for the neck of the diverticulum. Eight diverticula were upper polar, six were interpolar, and four were in the lower pole. Stones were endoscopically treate

We herein propose a nomenclature for renal calices and anatomical classification of the renal pelvis for endoscopic surgery. We prospectively collected data on a total of 270 patients (540 kidneys) who underwent CT-urography. We then constructed three-dimensional images of the renal pelvis and examined their morphologic classification. Forty-eight kidneys were excluded from this study because of severe hydronephrosis or large renal cysts. The remaining 492 kidneys were included. We systematically categorized each minor calix into five levels: top, upper, middle, lower, and bottom. The upper, m

PURPOSE: We conducted a prospective study to define normal renal pelvic size in children.
MATERIALS AND METHODS: Institutional Review Board approved consent was obtained to perform renal ultrasound during excretory urography (IVP) scheduled for medical management. Mean patient age (17 females, 11 males) was 5.2 years. Renal ultrasound was conducted concurrent with 10-minute IVP.
RESULTS: Fifty kidneys were imaged with 51 collecting systems. IVP defined 44 collecting systems as normal. Mean anteroposterior pelvic diameter on sonography for these 44 systems was 3.3 mm. One normal collecting sy

UNLABELLED: A FREQUENT PROBLEM: Lithiases in the urinary tract form first in the calices, raising complex problems of lithogenesis. As described by Randall, in males caliceal lithiases are often composed of calcium oxalate, generally as a monohydrate. In females, calcium phosphate stones are common, stones related to urease-producing germs are less frequent, and the bi-hydrated form of calcium oxalate predominates.
UNPREDICTABLE NATURAL HISTORY: Caliceal lithiases can remain asymptomatic and are often discovered fortuitously. Symptoms include chronic lower back pain or urinary tract infection

Of 11,500 children who underwent excretory urography during a 17-year period, three were found to have the rare renal malformation infundibulopelvic stenosis, characterized by caliceal dilatation, infundibular stenosis, and hypoplasia or stenosis of the renal pelvis. The contralateral kidney was absent in two cases and normal in the other. Voiding cystourethrograms were normal in all three. Renal sonography showed a variable degree of caliceal dilatation without associated pelvic dilatation. The diagnosis was confirmed by retrograde ureteropyelography in one case. Two patients were followed fo

One thousand and ninety-four normal human kidneys and 18 abnormal (with duplication of the ureter) were studied by the corrosion method and pyelography followed by topometric and mathematics analyses. It was found that the renal pelvis is a calicopelvic complex built up of renal calices, urine ducts and renal pelvis. Before opening into the renal pelvis, renal calices join together forming urine ducts (superior and inferior; or superior, middle and inferior; or superior, middle anterior, middle posterior and inferior) which transport urine to the container, the renal pelvis. It can be seen tha

Hemangiomas of the kidney are uncommon. They are usually single, and are seldom multiple or bilateral. A 60-year-old male with multiple cavernous hemangioma of left kidney is reported. Macrohematuria and small pyelographic filling defects in lower calix were the principal manifestations and left nephrectomy was performed. Several tumors were found in upper and lower calices. The histologic diagnosis was cavernous hemangioma. Microscopic hemangioma was also observed in pericaliceal tissue. Related reports were also reviewed.

PURPOSE: We assess the efficacy of extracorporeal shock wave lithotripsy monotherapy for isolated lower pole nephrolithiasis, and compare it to that for isolated middle and upper caliceal calculi.
MATERIALS AND METHODS: We treated 714 renal units in 687 patients with isolated caliceal stones using a Lithostar lithotriptor (Siemens Medical Systems, Erlangen, Germany). The stones were localized in the lower, mid and upper calices in 455, 104 and 128 patients, respectively. Stone load was recorded in cm.2. Patients were stratified into 3 groups based on stone burden. The energy and shock waves,

Introduction: Our comprehension of the actual diversity of kidney pyelocaliceal system (PCS) is lacking and many crucial features have been overlooked in the existing literature on this subject. The purpose of this review is to provide a concise summary of the current understanding of the structure of the pelvicalyceal system, considering both anatomical and surgical perspectives, as well as to highlight any limitations or inconsistencies in these approaches. Material and Methods: A full review of all the literature on the anatomical and surgical methods used to classify pelvicalyceal system w

The authors present 15 cases of calicial ectopias. They then review some of the names frequently given to this illness and discuss certain theories which link this anomaly with other kidney cystoses, taking special care to point out the difficulties involved in the correct performing of a differential diagnosis with calculous hollows, particularly those situated in lower calices.

A clinical case is described featuring bilateral lithiasis of the renal pelvis and calices in conjunction with cystic kidney in a female patient. Progression was shown by tumor enhancement in the lower abdomen, a distension of the concretions in pyelogram, and an increment of creatine in the serum. In this case there was no indication for surgery.

Antegrade pyeloureterography performed via fine puncture needle yields detailed imaging of the anatomy of the renal pelvis, calices, and ureter of the transplanted kidney. It is a simple and safe examination method providing information which, as a rule, can not be obtained by urography, radioisotope gammagraphy or ultrasonography. Puncture of the collecting system of the transplanted kidney with subsequent pyeloureterography is indicated for determining the site and nature of the obstruction in the ureter in urinary stasis in the transplanted kidney, for diagnosing urinary fistulas, for funct

Extracorporeal Shock Wave Lithotripsy (ESWL) represents the first choice therapy for renoureteral stone disease. Clinical controversy exists concerning the efficacy of ESWL for lower pole kidney stones. Nowadays, the factors that hinder the spontaneous passage of stone debris that results from ESWL of lower caliceal stone are the gravity-dependent position of the lower pole calices and particular features of the inferior-pole collecting system anatomy. We studied the influence of the lower infundibulo-pelvic in the success of ESWL of lower caliceal stones 10-20 millimeters in size in 50 patien

OBJECTIVE: Megacalycosis or Puigvert's disease is a congenital anomaly of renal development characterized by caliceal dilatation, an increased number of calyces, associated with hypoplasia of the pyramids of Malpighi, and a normal renal pelvis. Renal function is always normal and there is no evidence of obstruction to urinary flow. The foregoing are important in distinguishing megacalycosis from congenital hydronephrosis. We have studied two pediatric patients with this renal anomaly by means of non invasive techniques, in order to demonstrate they had no urinary flow obstruction despite the c

9 cases of "thorotrast kidney" after retrograde pyelography with reflux are presented. In 6 patients carcinoma of the renal pelvis developed 47--31 years after thorotrast pyelography. Because of this high rate of carcinoma induction, thorotrast kidney must be classified as a precancerous condition with high-rate tumor manifestation. Thorotrast kidney is diagnosed on X-ray by very opaque linear densities following the calices and pyelon. Early ureteronephrectomy is the treatment of choice in unilateral thorotrast kidneys. If uretero-nephrectomy is not possible, the patient should be followed up

OBJECTIVES: To show that hydronephrosis (HN) can be tracked by the quantitative reproducible hydronephrosis index (HI) and that HI is useful for serial ultrasound (US) studies to determine whether HN is improving or deteriorating.
METHODS: We found 60 hydronephrotic kidneys in 46 study patients. The other 32 kidneys were normal or absent. Serial US studies were performed more than 1 month apart over a 3-year period. Hydration was maximized with oral fluids. Cases ranged in age from 2 days to 13 years. We determined HI as follows: Operators outlined the perimeters of the kidney and dilated ren

Polyethylene ureteral stents suspended from the kidney by a pigtail memory in the proximal end were placed bilaterally in a patient with ureteral obstruction secondary to retroperitoneal sarcoma. Both stents migrated upward and 1 migrated into the renal parenchyma. We recommend that these stents be inserted into the renal pelvis, as opposed to the calices, and that sufficient length be allowed to prohibit migration above the bladder.

We report for the first time a patient with a caliceal diverticulum that was detected in early childhood, who in adolescence developed xanthogranulomatous pyelonephritis to the diverticulum and surrounding kidney and infiltrated to the liver. The condition was treated by nonradical organ-sparing surgery and prolonged antibiotic therapy.

A 63-year-old man visited our hospital because of painless macrohematuria. Drip infusion pyelography and retrograde pyelography revealed a space-occupying lesion with extravasation of contrast medium to upper caliceal system. A computed tomographic study revealed an intrarenal solid moiety and further more, arteriography showed the arterial encasement and fine neovascularity of the lesion. On gross section of the extirpated kidney, a cystic cavity measuring 3 by 3 cm existed at the upper pole and apparently channeled to the upper caliceal system. Small stones were found in the cystic cavity. H
