Nerve Injuries in Total Knee Arthroplasty.

Nerve injury is one of the potential complications of total knee arthroplasty. The extent of the injury includes motor and sensory dysfunction, either temporary or permanent. Although the consequences of nerve injury may be dramatic, the probability of occurrence during the course of primary knee arthroplasty is low, around 0.12% to 0.4%. Local dressing removal and knee flexion are imperative, and the initial investigations include ultrasound or MRI and nerve conduction studies. The extent of recovery depends on the type and severity of the initial nerve palsy; however, most patients are expec

[Ségond fracture in acute knee injuries].

Two patients with a lateral tibia avulsion fracture (Ségond fracture) are presented. The correlation between the Ségond racture and rupture of the anterior cruciate ligament is pointed out, as well as the fact that patients with Ségond fracture and rupture of the anterior cruciate ligament often have other injuries such as meniscus lesions and lesion of the medial collateral ligament.

Acute knee injuries: when to refer.

Primary care physicians are being called on more frequently to treat patients who have musculoskeletal injuries. This article discusses mechanisms of injury, diagnosis, and treatment, with emphasis on determining when to refer patients to an orthopedist. Discussed here are several types of knee injuries that require referral, including extensor mechanism injuries, most subluxations and dislocations, ligament injuries, and meniscus injuries.

Overuse Knee Injuries in Young Athletes.

In brief Because of their skeletal immaturity, children and adolescents are subject to a somewhat different set of overuse knee injuries than are adults. Described here are the diagnosis and treatment for the most common growth-related causes of knee pain in active young people: Osgood-Schlatter disease, Sinding-Larsen-Johansson disease, osteochondritis dissecans, bipartite patella, medial plica syndrome, and discoid meniscus. In most cases these conditions respond to conservative treatment.

Ipsilateral knee ligament injuries and open tibial diaphyseal fractures: incidence and nature of knee ligament injuries sustained.

Fifty patients with isolated open tibial shaft fractures were reviewed to determine the incidence and type of knee ligament injuries sustained. Eighteen patients (36 per cent) had at least one ligament injury in the ipsilateral knee; eight had multiple ligament injuries. Only four patients (22 per cent) were diagnosed as having a ligament injury at the time of initial management and the remaining 14 patients were diagnosed at the time of review for this study. There is a high incidence of ipsilateral knee ligament injuries in open tibial shaft fractures; the knee should be thoroughly examined

Arterial injuries below the knee: fifty-one patients with 82 injuries.

Clinical features of tibial and peroneal artery injuries are characterized in a review of 51 patients with 82 injured arteries. Injuries were penetrating in 34 (67%) patients and blunt in 17 (33%). Physical findings suggested arterial injuries in 42 (82%) patients, but nine had no signs of vascular trauma. Nineteen patients with no palpable distal pulses had average preoperative delays of nearly 5 hours, suggesting a lack of appreciation for the morbid potential of these injuries. Operations included ligations of single-vessel injuries and arterial reconstructions in the 21 (41%) patients with

Intra-articular knee injuries in patients with knee extensor mechanism ruptures.

The knee extensor mechanism is composed of the quadriceps tendon, patella and patellar tendon. Rupture of either the quadriceps tendon or patella tendon is a rare but significant injury. The purpose of our study is to determine if there are any associated injuries with these ruptures necessitating the need for further evaluation such as MRI or arthroscopy. We retrospectively reviewed all patients with ruptures of the knee extensor mechanism who required operative repair at our institution over the last 10 years. We reviewed the chart for any documented associated injury. The type and incidence

Non-operative Management of Acute Knee Injuries.

PURPOSE OF REVIEW: Acute knee injuries are commonly encountered in both the clinical and sideline setting and may be treated operatively or non-operatively. This article describes an evidence-based approach to non-operative acute knee injury. This includes history, physical exam, imaging, and initial management. In addition, the non-operative management of three such injuries-ligament injury, meniscus injury, and patellar dislocation injury-will be discussed via a case-based practical approach. RECENT FINDINGS: Aside from grade III ACL tears, most acute knee ligament injuries, especially in t…

Posterolateral corner knee injuries: a narrative review.

Limited knowledge of the anatomy and biomechanics of the posterolateral corner (PLC) of the knee, coupled with poor patient outcomes with non-operative management, resulted in the PLC often being labelled as the 'dark side' of the knee. In the last two decades, extensive research has resulted in a better understanding of the anatomy and function of the PLC, and has led to the development of anatomic reconstructions that have resulted in improved patient outcomes.Despite considerable attention in the clinical orthopaedic literature (nearly 400 articles published in the last decade), a standardi

Floating knee injuries: postoperative complications and outcome.

OBJECTIVE: To evaluatepost-operative complications and functional outcome of floating knee injuries using Karlstrom\'s criteria. METHODS: The prospective randomised study was conducted at Allied Hospital, Faisalabad, Pakistan, from November 2013 to October2014, and comprised patients of either gender. The injuries were classified using Fraser classification. Femur fractures were treated with intramedullary nails, dynamic hip screws, dynamic condylar screws, dynamic compression plates, buttress plates or external fixators. Tibia fractures were treated with buttress plates, dynamic compression …

Osseous and myotendinous injuries about the knee.

Osseous injuries may be articular, extra-articular, or physeal, and may be related to direct trauma, avulsion forces, or chronic microtrauma. Muscle and tendon injuries about the knee may occur alone or in association with more significant osseous and ligament injuries. Radiographs or computed radiography images remain an important screening technique for patients who have suspected knee injuries. After initial radiographs, MR imaging has become the most common modality for detection and complete evaluation of osseous and soft-tissue injuries about the knee when those radiographs and other tec…

[Treatment of children's and adolescent's knee injuries].

INTRODUCTION: Authors made 162 knee arthroscopic operations due to hemarthrosis of the knee, acute knee blocking or acute developed instability on patients under 18 years after knee injury during a 12 years period. The retrospective analysis of patient reports shows, that in 61.7% of the cases they found chondral- or osteochondral fracture in the knee. MATERIAL AND METHODS: On three dimensional routine X-ray pictures they were performed after the injury, in 85% of the cases fractures were not seen. Fortunately the most part of the hyalin surface injuries was smaller than 0.5 cm or were situat

"Dem bones": osteochondral injuries of the knee.

MR imaging plays a valuable role in the diagnosis and staging of osteochondral injuries of the femorotibial joint. Bone contusions may be the source of a patient's pain, and MR imaging characteristics of certain types may help to predict which contusions might progress to more serious osteochondral lesions. MR imaging also is vital in the diagnosis of occult osteochondral fractures and in accurately classifying displaced intra-articular fractures. Although osteochondral dissecans usually is diagnosed radiographically, MR imaging is the best noninvasive test for determining if an osteochondral

Management of penetrating injuries of the knee.

One hundred and two patients with 121 known or suspected penetrating wounds of the knee demonstrated that successful results are dependent upon early diagnosis and surgical treatment. The willingness to act immediately on the basis of clinical judgment alone is important in order to avoid irreparable damage damage to the joint. The knee should be debrided and thoroughly irrigated. The menisci, articular cartilage and bony stock should be preserved as much as possible. The joint and skin are left open. If joint surfaces are damaged or infection is grossly purulent, the synovium is also left ope

Modern Treatment Principles for Multiligament Knee Injuries.

Multiligament knee injuries are complex injuries that must be addressed with a comprehensive diagnostic workup and treatment plan. Multiligament injuries are commonly observed with concomitant meniscal, chondral, and neurovascular injuries, requiring a thorough clinical assessment and radiographic evaluation. Due to the higher failure rates associated with knee ligament repair following multiligament knee injury, the current literature favors single-stage anatomic knee reconstructions. Recent studies have also optimized graft sequencing and reconstruction tunnel orientation to prevent graft el…
