Changes in neurotrophic factors of adult rat laryngeal muscles during nerve regeneration.

Injury to the recurrent laryngeal nerve (RLN) leads to the loss of ipsilateral laryngeal fold movement, with dysphonia, and occasionally dysphagia. Functional movement of the vocal folds is never restored due to misrouting of regenerating axons to agonist and antagonist laryngeal muscles. Changes of neurotrophic factor expression within denervated muscles occur after nerve injury and may influence nerve regeneration, axon guidance and muscle reinnervation. This study investigates the expression of certain neurotrophic factors in the laryngeal muscles during the course of axonal regeneration us…

Misdirected reinnervation in the feline intrinsic laryngeal muscles after long-term denervation.

Nerve terminals which resembled the autonomic nerve varicosities were found at all neuromuscular junctions of the feline intrinsic laryngeal muscles after 21 months of unilateral laryngeal paralysis. The structure of muscle fibers was preserved throughout this period, although there was no evidence of regeneration of transected recurrent laryngeal nerve. In the cricothyroid muscle, misdirected reinnervation was recognized in the different pathways from the other intrinsic laryngeal muscles.

[Respiration rhythm-controlled electric stimulation of paralyzed laryngeal muscles--animal experiment findings].

Patients with bilateral paresis of the recurrent laryngeal nerves have primarily breathing difficulties. But on the other hand the common operations for widening the glottis produce voice problems. To overcome these difficulties a muscle stimulation device was developed, which is triggered by the inspiratory motion of the thorax. With this device impulses are transmitted by radiofrequency from a transmitter located outside the body to a receiver implanted into subcutaneous tissue of the neck and thence by electrode wires to the posterior cricoarytenoid muscle (PCA). In the dogs with one cut re

Muscle-nerve pedicle laryngeal reinnervation.

A muscle-nerve pedicle implantation to the lateral cricoarytenoid muscle as described by Tucker is an alternative to Teflon injection for treating dysphonia due to vocal cord palsy. Improvement in voice was noted in 19 of the 20 (95%) selected patients who were treated by muscle-nerve pedicle reinnervation. These patients have been followed for 6 months to 10 years. Changes in the voice were documented through assessment of high-quality audio tape recordings by three speech pathologists, results of a patient questionnaire, and evaluation by the surgeon. The improvement in voice quality was att

Dynamics of Intrinsic Laryngeal Muscle Contraction.

OBJECTIVES: Laryngeal function requires neuromuscular activation of the intrinsic laryngeal muscles (ILMs). Rapid activation of the ILMs occurs in cough, laughter, and voice-unvoiced-voiced segments in speech and singing. Abnormal activation is observed in hyperfunctional disorders such as vocal tremor and dystonia. In this study, we evaluate the dynamics of ILM contraction. STUDY/DESIGN: Basic science study in an in vivo canine model. METHODS: The following ILMs were stimulated: thyroarytenoid (TA), lateral cricoarytenoid/interarytenoid (LCA/IA), cricothyroid (CT), all laryngeal adductors (…

Three-dimensional posture changes of the vocal fold from paired intrinsic laryngeal muscles.

OBJECTIVES/HYPOTHESIS: Although the geometry of the vocal fold medial surface affects voice quality and is critical in the treatment of glottic insufficiency, the prephonatory shape of the vocal fold medial surface is not well understood. In this study, we activated intrinsic laryngeal muscles individually and in combinations, and recorded the temporal sequence and precise three-dimensional configurational changes of the vocal fold medial surface. STUDY DESIGN: In vivo canine hemilarynx model. METHODS: A hemilaryngectomy was performed in an in vivo canine model and ink was used to mark the m…

Differential expression of glial-derived neurotrophic factor in rat laryngeal muscles during reinnervation.

OBJECTIVES/HYPOTHESIS: Nonspecific, synkinetic reinnervation is one of the causes of poor functional recovery after a peripheral nerve lesion. Knowledge of the differential expression of neurotrophic factors that subserve axon guidance, as well as neuromuscular junction formation and maintenance in the denervated muscles, may allow appropriate interventions that will improve the functional nonsynkinetic reinnervation. STUDY DESIGN: Laboratory experiment. METHODS: The expression of glial-derived neurotrophic factor (GDNF) was studied in the abductor and adductor muscles of the larynx in the r

Functional electrical stimulation of intrinsic laryngeal muscles under varying loads in exercising horses.

Bilateral vocal fold paralysis (BVCP) is a life threatening condition and appears to be a good candidate for therapy using functional electrical stimulation (FES). Developing a working FES system has been technically difficult due to the inaccessible location and small size of the sole arytenoid abductor, the posterior cricoarytenoid (PCA) muscle. A naturally-occurring disease in horses shares many functional and etiological features with BVCP. In this study, the feasibility of FES for equine vocal fold paralysis was explored by testing arytenoid abduction evoked by electrical stimulation of t

Dystrophic pathology in the intrinsic and extrinsic laryngeal muscles in the mdx mouse.

OBJECTIVE: This study aimed to characterize the laryngeal muscles in the mdx mouse model of Duchenne muscular dystrophy (DMD). DESIGN: Pathology was analyzed in the intrinsic and extrinsic laryngeal muscles from mdx mice and compared with other mdx muscle groups and with muscles from age-matched normal control mice. SETTING: In DMD, a dystrophin protein deficiency causes skeletal muscle breakdown. However, some muscle groups, such as the extraocular muscles, are spared from damage. Characterizing other unaffected muscles may form the basis for developing new therapies to alleviate muscle bre…

Three-dimensional changes in the upper airway during neuromuscular stimulation of laryngeal muscles.

During swallowing, airway protection depends upon adequate glottal closure and laryngeal elevation to prevent the entry of substances into the airway. Three-dimensional changes in the upper airway during laryngeal muscle stimulation in a canine model were quantified in animals implanted with Peterson type stimulating electrodes in the inferior and superior portions of the thyroarytenoid muscle, together with a reference electrode. Computer tomography scanning was performed on an IMATRON scanner with a 3 mm slice thickness advanced at overlapping 1 mm increments. Stimulation of the thyroaryteno

Laryngeal pacemaker. Part I. Electronic pacing of reinnervated strap muscles in the dog.

The best approach to management of bilateral vocal cord paralysis, with its resultant airway compromise, has not yet been universally agreed upon. Recently proposed methods have included direct stimulation of the laryngeal dilators by the modulation of rhythmic information from the chest wall, diaphragm, phrenic nerve, or accessory muscles of respiration. In such an approach, the information obtained is not necessarily related to inspiration only, and direct implantation of a stimulating electrode into the laryngeal musculature may not be fully efficient and tolerated over time. To obviate suc

Laryngeal pacemaker. II. Electronic pacing of reinnervated posterior cricoarytenoid muscles in the canine.

A fully reliable means of rehabilitating patients with bilateral vocal cord paralysis has not yet been developed. In order to improve upon existing solutions to this problem, the authors have recently described a laryngeal pacemaker, initially tested through stimulation of a cross-over nerve-muscle pedicle from one sternohyoid muscle to the other in the canine. Afferent stimuli, initiated through elongation of the airway during inspiration, were detected by a linear strain gauge sutured to the tracheal rings and appropriately amplified. The current report deals with the application of this con
