A 76-year-old woman with a chronic leg ulcer for the last thirty-seven years was hospitalized in our institution for chronic diarrhea and terminal kidney failure with proteinuria. The diagnosis of secondary amyloidosis due to persistent skin inflammation was confirmed by aspiration of subcutaneous abdominal fat and by kidney biopsy which showed AA type systemic amyloidosis. This appears to be a rare complication of chronic leg ulcers as there have been only eight publications covering eleven cases in the literature.

Based on different experimental findings, the following sequence of pathogenetically important factors for the development of venous ulcers is outlined: Valvular incompetence, reflux----chronic venous ambulatory hypertension----capillary dilatation and deformation----hyperemia----increased transendothelial passage (protein), microlymphangiopathy----protein deposition in the tissue. Deficient fibrinolysis----pericapillary fibrin cuffs----diffusion block----local hypoxia, tissue death. Although other mechanisms and complex factors may also play important additional roles, for the first time a re

BACKGROUND: Leg ulcers are common, costly, and significantly impair quality of life, but their management is variable and associated with considerable delays in healing. The aim of this study was to design an accelerated leg ulcer care pathway in a community and hospital setting to improve patient outcomes.
METHODS: A new referral pathway was developed using a series of healthcare professional and patient interviews, focus groups, and stakeholder workshops. The referral pathway, investigation and treatment protocols were further informed by clinical guidelines to develop the Leg Ulcer Pathway…

Twenty-five leg ulcer exudate samples from 17 patients with chronic non-healing venous leg ulcer were analyzed for proteolytic activity using radial caseinolysis procedures and zymographic analysis, and for fibronectin fragmentation using immunoblotting technology. Caseinolytic activity was detected in 21 of the 25 samples. A minority of them were inhibited (3 were totally, 6 partially inhibited) by aprotinin, a serine proteinase inhibitor, suggesting that proteinase(s) other than plasmin were also responsible for the caseinolysis. In zymographic analysis, 23 of the 25 samples showed positive …

Seventeen cases of chronic leg ulcers were examined, 12 males and five females, with a mean age of 45 years. The average duration of ulcers was 21 years. Eleven of the cancers arose from ulcers that recurrently healed and broke down. Three arose from ulcers that never healed and three from stable scars. Treatment was by various levels of amputation in nine patients and by excision and skin grafting in one. Histological findings showed 16 cases to be squamous cell carcinoma while one case was sarcomatous in appearance. Most of the cases were poorly differentiated and findings of pseudoepithelio…

Ulcerations of the lower extremities are a frequently encountered problem in clinical practice and are of significant interest in public health due to the high prevalence of underlying pathologies, including chronic venous disease, diabetes and peripheral arterial occlusive disease. However, leg ulcers can also present as signs and symptoms of various rare diseases and even as an adverse reaction to drugs. In such cases, correct diagnosis ultimately relies on histopathological examination. Apart from the macroscopic presentation, patient history and anatomic location, which are sometimes indic…

This review discusses current therapeutic options for leg ulcers. Compression therapy remains the first-line treatment for venous ulcers, with the use of an external pressure of 30 to 40 mmHg at the ankle (0.8 < ABPI < 1.3). Superficial vein surgery does not improve healing rates of venous ulcers; however it has been shown to reduce ulcer reoccurrence in the context of a competent deep venous system. In cases of severe arterial insufficiency, lower limb revascularization should be considered if possible. Wounds cleansing does not require antiseptics. Wound debridement is an accepted practice b…

Leg ulcers represent the most common chronic wound in our population. This devastating problem often cripples patients, many of whom are in the prime of their working years. The resulting morbidity and financial cost are extraordinary. Fortunately, with the right approach, the vast majority heal with simple ambulatory outpatient therapy. The essential requirements for treating leg ulcers include a thorough understanding of lower extremity anatomy, vascular hemodynamics, and the management of trophic and inflammatory skin changes.
