INTRODUCTION: Uterine leiomyosarcomas are relatively uncommon soft tissue neoplasms and rarely metastases to small bowel. In the current case; a patient is suffering from intestinal perforation due to metastatic leiomyosarcoma of the small bowel.

Leiomyosarcomas of the gastrointestinal tract are rare mesenchymal neoplasms which grossly and histologically resemble gastrointestinal stromal tumors. A case of leiomyosarcoma of the scrotum is reported for its rarity. The tumor measured 6.2 x 4 x 4 cm and presented as a gradually increasing painless mass of 1 month duration. Light microscopy showed the typical interlacing fascicles of neoplastic cells with mitotic count of 1-2 / 10 HPF in different areas. Immunoperoxidase stains of vimentin & desmin and electron microscopy confirmed its muscle origin.

BACKGROUND: This is a single case report of a rare tumour, pancreatic leiomyosarcoma.
CASE OUTLINE: A 44-year-old woman presented with a painless epigastric mass, and CT scan showed a cystic lesion of the head of pancreas. An associated epigastric mass was found to be a lipoma. Pancreatoduodenectomy was successfully performed. Pathology revealed leiomyosarcoma of the pancreas. The patient is well and free of disease four years later.

Primary ureteral tumors occur relatively infrequently compared to tumors of the kidney and bladder. Smooth muscle tumors of the ureter are very rare and only 9 cases of leiomyosarcoma have been reported in the literature. The treatment of choice is total nephroureterectomy with en bloc excision of the tumor. Prognosis is unfavorable as indicated by the short survival of the reported cases.

A fourth case of leiomyosarcoma of the hypopharynx is reported. The histopathological diagnosis and treatment are discussed. Surgery is still the treatment of choice and, in combination with chemotherapy (especially Methotrexate) and radiotherapy, the recurrence rate can be reduced. However, randomized studies are required.

Uterine leiomyosarcoma distant metastasis is common, and lung metastasis has been reported. However, unique cases have been identified either with late onset of metastatic disease or with large size of lung metastasis. A typical approach to avoid metastasis would be a hysterectomy. Nonetheless, metastatic recurrence is common. We encountered a case at our hospital with leiomyosarcoma metastatic to the lungs. Lung metastasis was noted to be 17 cm in diameter. This size has not yet been reported in the literature to the best of our knowledge.

Primary testicular leiomyosarcoma is an extremely rare tumor, and, to the best of our knowledge, only 20 cases in adults have been reported in the literature to date.
