This case report describes a young female patient with a history of surgery to treat choledochal cyst since childhood who was admitted to our hospital with cholangitis. An imaging examination revealed giant stones that almost completely filled the intrahepatic biliary tract. The patient underwent percutaneous transhepatic lithotripsy using a holmium laser. After the lithotripsy, cholangiography showed no residual stones. The patient displayed clinical improvement and was discharged after 14 days in the hospital. This case serves as a reminder of gallstone complications that can occur subsequen

BACKGROUND AND OBJECTIVES: Holmium laser lithotripsy is a safe and effective method to disintegrate urinary stones of all compositions in an endoscopic procedure. However, handling and safety could be improved by a real-time feedback system permanently monitoring the position of the treatment fiber. The laser is fired only when the fiber is identified as being placed in front of stone. This work evaluates the potential of fluorescence detection with an excitation wavelength of 532 nm for this purpose.
MATERIALS AND METHODS: A fiber-based fluorescence measurement was set-up to acquire autofluo

BACKGROUND AND PURPOSE: Urethral calculi usually present as acute retention of urine. The issues in this rare clinical problem are its emergency management scenario, small urethral caliber, and long-term risk of stricture formation. In this retrospective study, the positive experience with the safety and efficacy of holmiumlaser lithotripsy for ureteral calculi was applied to the management of urethral calculi, and the initial results were analyzed.
PATIENTS AND METHODS: From October 1999 to June 2003, 42 patients presented with symptomatic urethral calculi. Under anesthesia, an attempt was m

INTRODUCTION: To understand benign aggressive lesions at protein expression level, several tumor biomarkers assist in diagnosis, early detection and nature of the lesion with prognostication. Cell proliferation is the foundation of tumor development. Cyclin D1 and Ki-67 are two proliferation markers. The p53 gene, located at the short arm of chromosome 17 is the best known tumor suppressor gene. In this study, immunohistochemical staining is done to assess the expression patterns of Ki-67, p53 and Cyclin D1 in the benign aggressive lesions of the maxillofacial region and relate it with the cli

OBJECTIVE: This paper reviews the current use of laser techniques for the treatment of biliary stones.
BACKGROUND DATA: Biliary stones may pose a special problem when access to them is limited, when previous attempts of removal have failed, or when a less invasive option than surgery is needed. The availability of various laser sources and the adaptation of the technology for safe use in the biliary system make the use of laser energy for stone fragmentation possible.
METHODS: Current literature is reviewed concerning the use of laser for biliary lithotripsy, including experimental data and

PURPOSE: Holmium laser lithotripsy is a common modality used to fragment urinary stones during ureteroscopy. Laser energy deposited during activation produces heat and potentially causes thermal bioeffects. We aimed to characterize laser-induced heating through a computational simulation.
MATERIALS AND METHODS: A finite-element model was developed and used to estimate temperature in the urinary tract. Axisymmetric models of laser lithotripsy in a renal calyx, the renal pelvis, and proximal ureter were created. Heat generation by laser and heat transfer were simulated under different laser pow

BACKGROUND AND OBJECTIVE: Two laser devices that are used today in endoscopic treatment of renal and ureteral calculi are the frequency-doubled double-pulse neodynium:YAG (FREDDY) and Holmium:YAG lasers. The mechanism of action of these lasers differs, thus conferring potentially different safety and efficacy profiles. The in vitro efficacy of these lasers in treating urinary stones has been explored, but to our knowledge no study compares the in vivo efficacy. The purpose of this study is to compare the stone-free and complication rates of the FREDDY and holmium lasers.

The present study has established that a flashlamp pumped coumarin dye laser is capable of fragmenting salivary calculi. The refinement of spectroscopic feed-back techniques will allow laser lithotripsy to complement ultrasonic lithotripsy in the non surgical management of salivary calculi.

BACKGROUND: Electrical intravascular lithotripsy (E-IVL) uses shock waves to fracture calcified plaque.
AIMS: We aimed to demonstrate the ability of laser IVL (L-IVL) to fracture calcified plaques in ex vivo human coronary arteries and to identify and evaluate the mechanisms for increased vessel compliance.
METHODS: Shock waves were generated by a Ho:YAG (Holmium: yttrium-aluminium-garnet) laser (2 J, 5 Hz) and recorded by a high-speed camera and pressure sensor. Tests were conducted on phantoms and 19 fresh human coronary arteries. Before and after L-IVL, arterial compliance and optical coh

Advances in interventional radiology have seen the adaptation of urological endoscopic laser techniques to treat biliary tract calculi. Percutaneous transhepatic biliary laser lithotripsy provides an effective alternative procedure for the management of intrahepatic or conventionally refractory choledocholithiasis which would otherwise require invasive and high-risk surgical intervention. Several small studies have validated the procedure for management in this subset of patients, with most achieving 100% calculi clearance with minimal complications. Most patients are suitable for percutaneous

Mirizzi syndrome is a serious disease that, even today, causes significant morbidity and mortality, and is generally considered a contraindication to laparoscopic surgery. Here, we describe a severe cholecystitis patient who was treated successfully with percutaneous transhepatic gallbladder scopic lithotripsy by laser. The patient initially underwent percutaneous transhepatic gallbladder drainage. Subsequent examination revealed a gallbladder stone impacted in the cystic duct. As the patient had a short and thick cystic ductal lumen, we decided to try to crush the stones through the fistula o

INTRODUCTION: Advances in laser technology have rendered laser lithotripsy an attractive option for lithotripsy during percutaneous nephrolithotomy (PCNL). However, limitations such as lower ablation rates and significant stone retropulsion have been noted, particularly in larger or harder stones. Trilogy is an emerging lithotrite that combines electromagnetic ballistic and ultrasonic energy with integrated suction in a single probe. This design offers theoretical advantages in stone fragmentation speed and debris clearance. This systematic review and meta-analysis aims to evaluate the safety

BACKGROUND: Ureteroscopy is the recommended surgical treatment for renal stones less than two centimeters in diameter but often leaves residual fragments, increasing the risk of stone recurrence. Autologous blood coagulum is a potential adjunct to ureteroscopy for aiding fragment retrieval. This study evaluates the efficacy and efficiency of autologous blood coagulum assisted ureteroscopy compared to ureteroscopy and basketing for achieving absolute stone-free Grade A status.
METHODS: Following Institutional Review Board approval, a retrospective review of our comprehensive UC Irvine Minimall

Moses technology was born with the aim of controlling the Moses effect present in every single Ho:YAG laser lithotripsy. The capacity to divide the energy pulse into two sub-pulses gained popularity due to the fact that most of the energy would be delivered in the second pulse. However, is this pulse modulation technique really better for endocorporeal laser lithoripsy? A review of the literature was performed and all relevant clinical trials of Moses 1.0 and 2.0, as well as the lab studies of Moses 2.0 carried out up to June 2022 were selected. The search came back with 11 clinical experience
