Angioimmunoblastic lymphadenopathy (AILD) is a lymphoproliferative disorder with well established clinical and histological features, one of the clinical manifestations being a peripheral polyarthritis. Medical records of 370 patients with sarcoidosis were reviewed. Of these, 32 had a computerized tomographic (CT) and/or ultrasound (US) exam … oidosis. Although lymphoma and metastatic disease are far more common causes of extensive abdominal lymphadenopathy, sarcoidosis should be considered in the appropriate clinical setting. In many cases, correlation with conventional chest radiographs may be confirmatory.

A systemic disorder characterized by fever and reactive lymphadenopathy developed in a patient with scabies infestation. Regression of all clinical symptoms and signs occurred after antiscabies therapy. Two months later, the whole clinical picture reappeared concomitantly with scabies reinfestation. A 71-year-old man had status epilepticus and was treated with phenytoin (Dilantin) sodium. Chest radiographs were reviewed in 30 cases of biopsy proven sarcoidosis; posterior mediastinal or para-aortic lymph node enlargement was found in 6 patients (20%) who also had symmetrical hilar and/or paratracheal lymph node enlargement. The radiographic behavior of the hilar, mid- and posterior mediastinal lymph nodes was the same. Posterior mediastinal lymph node enlargement in otherwise typical sarcoidosis is not unusual.

A 6-year-old girl presented at the emergency department with increasing swellings of multiple lymph nodes over the past 3 days, without other complaints. At physical examination an impressive general lymphadenopathy was seen without any other abnormalities. The peripheral blood smear showed 86% lymphoblasts. This chapter provides an approach to the evaluation of lymphadenopathy in children. Malignant etiologies, including HIV-associated malignancies are reviewed. In discussing infectious agents or causes of lymphadenopathy, particular emphasis is placed on chronic regional lymphadenopathy with a focus on new advances in cat-scratch disease and tuberculous and non-tuberculous mycobacterial lymphadenitis. After a suggested preliminary set of laboratory tests, biopsy is often necessary both for histologic and cultural investigation. Therapy must be specific and must follow an accurate diagnosis.

Immunoblastic lymphadenopathy presents with fever, malaise, cough, dyspnea, lymphadenopathy, hepatosplenomegaly, hypergammaglobulinemia, interstitial pulmonary infiltrates, mediastinal adenopathy, and effusions.
