LYMPHOSCORE (Lymphoscintigraphy Scoring Reliability) Reliability of a New Qualitative and Semiquantitative Lymphoscintigraphy Scoring System for Primary Lymphedema.

PURPOSE OF THE REPORT: Primary lymphedema (PLE) is a rare chronic disorder. Extremity lymphoscintigraphy offers access for dynamic and functional information on peripheral lymphatics and lymph nodes. We aimed to assess intraobserver and interobserver reliability of a new lymphoscintigraphy quantitative and qualitative scoring system in a homogeneous population of adult patients followed for PLE of the lower limb(s). PATIENTS AND METHODS: This is a monocentric retrospective study. Clinical files of patients who underwent a lymphoscintigraphy were reviewed for inclusion. Lymphoscintigraphies we

Lymphoscintigraphy for Imaging of the Lymphatic Flow Disorders.

Lymphoscintigraphy has introduced with the great advantage for diagnostic imaging of the lymphatic flow disorders. Lymphoscintigraphy can be performed in patients of any age, including neonates, and even in patient in critical conditions. The procedure is quite simple, and it needs only subcutaneous injection of small amounts of radiotracers. From subcutaneous tissue the radiotracer is taken by the lymphatic vessels and gives off energy in the form of gamma radiation detected by a gamma camera. Radiotracers rarely cause the allergic reaction and can be administered to the patients with allergy

Types of lymphoscintigraphy and indications for lymphaticovenous anastomosis.

Several authors have reported the usefulness and benefits of lymphoscintigraphy. However, it is insufficient to indicate microvascular treatment based on lymphedema. Here, we present the relationships between lymphoscintigraphic types and indications for lymphatic microsurgery. Preoperative lymphoscintigraphy was performed in 142 limbs with secondary lymphedema of the lower extremity. The images obtained were classified into five types. Type I: Visible inguinal lymph nodes, lymphatics along the saphenous vein and/or collateral lymphatics. Type II: Dermal backflow in the thigh and stasis of an

Role of lymphoscintigraphy for selective sentinel lymphadenectomy.

An essential prerequisite for a successful sentinel node biopsy (SNB) procedure is an accurate map of the pattern of lymphatic drainage from the primary tumor site. The role of lymphoscintigraphy (LS) in SNB is to provide such a map in each patient. This map should indicate not only the location of all sentinel nodes but also the number of SNs at each location. Such mapping can be achieved using 99mTc-labeled small particle radiocolloids, high-resolution collimators with minimal septal penetration, and imaging protocols that detect all SNs in every patient regardless of their location. This is

[Contribution of lymphoscintigraphy in the exploration of lymphedema in children].

OBJECTIVES: The objectives of this work were a) to assess the feasibility of lymphoscintigraphy in very young children, and b) to assess its usefulness in understanding the underlying pathophysiology. PATIENTS AND METHODS: Forty-one children (age range 3 months-16 years) with primary lymphedema of the lower and/or upper limbs underwent lymphosinctigraphy using 99mTC-labeled colloid (Nanocis or Nanocol) injected subcutaneous at the foot and/or hand level. Recording time, which depended on the child's age, included at least static images during the thirty minutes following injection and four ho

Dermal versus intraparenchymal lymphoscintigraphy of the breast.

BACKGROUND AND OBJECTIVE: Dermal and intraparenchymal (IP) injections of radiocolloid have been used for lymphoscintigraphic identification of the sentinel node (SN) in breast cancer. Because of our institute's extensive experience with dermal and IP lymphoscintigraphy for melanoma and breast cancer, we compared patterns of lymphatic migration after both types of injections to identify any differences in drainage patterns or SN identification. METHODS: Lymphoscintigrams (n = 31) after dermal injections in 30 patients with primary cutaneous melanoma on the breast were compared with lymphoscint

[Intraoperative lymphoscintigraphy in radical surgery of cancer].

The use of a gamma-camera operating table and the intraoperative lymphoscintigraphy using 99mTc-Antimony sulfide in radical surgery of carcinoma of the breast allows a better control of axillary lymphadenectomy. Remained lymph nodes are localized by an indicator filled with technetium and removed secondarily. Due to this technique it is possible to control and increase the extent of the axillary lymphadenectomy. Since the introduction of intraoperative axillary lymphoscintigraphy 83 patients were treated by mastectomy and axillary lymphadenectomy. In 48 cases complete lymphadenectomy could be

Lymphoscintigraphy in staging and management of breast cancer.

Internal mammary lymphoscintigraphy (IML) is a simple non-invasive and reproducible technique to determine the extent of the parasternal node invasion in patients suffering from breast cancer. A total forty four patients--37 before surgery or any sort of treatment and seven patients after having undergone surgery, chemotherapy and/or radiotherapy were included in this study to assess the potential role of lymphoscintigraphy in the context of the present 'state of the art' of staging and management of breast cancer patients in Bangladesh. The scintigraphic test was done using Technetium 99m (Tc

Lymphoscintigraphy in a case of Merkel cell tumor.

The authors report a rare case of Merkel cell tumor with the findings of lymphoscintigraphy. The study was performed to delineate anatomic lymphatic drainage from the primary skin lesion to help surgeons to decide which lymphatic basin(s) should have elective lymph node dissection. The lesion was situated in the midline on the back and at the border zone in the lumbar area from which it could drain superiorly to the axillary lymph nodes(s), inferiorly to the inguinal node(s), or to both directions. Because of this location, prediction of the lymphatic drainage by applying conventional anatomic

Lymphoscintigraphy to confirm the clinical diagnosis of lymphedema.

Confirmation of the diagnosis of lymphedema often requires lymphangiography, a procedure that is painful for the patient and technically demanding. Radioisotope lymphoscintigraphy is a relatively new technique that uses technetium 99 m antimony trisulfide colloid to produce a diagnostic image similar to a lymphangiogram. The procedure requires a single subcutaneous injection in the involved extremity, and images are obtained 3 hours later. It is technically easy to perform, produces minimal discomfort for the patient, and has no adverse effects. We have recently used radioisotope lymphoscintig

Lymphoscintigraphy as a guide to treatment in malignant melanoma.

Regional node dissection is practiced as a measure of prophylaxis in patients with stage I and II malignant melanoma. Although the drainage pattern of the extremities is obvious, in the head and neck and trunk it may be ambiguous. We have used lymphoscintigraphy to assist in delineating the lymphatic drainage in 22 patients with primary malignant melanoma. Fourteen patients had melanoma in the head and neck region, and eight had melanoma in the trunk region. Based on Clark's classification there were ten level III melanomas, eight level IV melanomas, and two level V melanomas; the levels of th

Nuclear Medicine Lymphoscintigraphy: Applications and Technical Overview.

Although lymphoscintigraphy is most commonly used for sentinel lymph node mapping, it can also be helpful for evaluating lymphedema, lymphatic malformations, lymphatic leaks, chylous ascites, and chylothorax. Assessment for lymphedema is important because this condition poses significant clinical challenges, and early diagnosis is essential for timely intervention and prevention of complications. Lymphoscintigraphy is a valuable radiologic tool for distinguishing lymphatic causes of edema from nonlymphatic ones, with findings of lymphedema including absent, asymmetric, or delayed drainage from

Potential utilization of lymphoscintigraphy in patients with kaposiform hemangioendothelioma.

BACKGROUND: Kaposiform hemangioendothelioma (KHE) is characterized by the dysregulation of both angiogenic and lymphangiogenic pathways. The application of lymphoscintigraphy in the management of KHE remains poorly documented. This study aims to delineate the lymphoscintigraphic imaging features of KHE and assess the potential utility of this modality in clinical management. METHODS: In this retrospective study, we evaluated lymphoscintigraphic features in patients with KHE. The assessed parameters included the extent of dermal backflow, flow patterns, lymphatic stasis, regional lymph node vi

Quantitative lymphoscintigraphy of the lower limbs for the diagnosis of phlebolymphoedema.

INTRODUCTION: Phlebolymphoedema is caused by the interaction of the venous and lymphatic systems in a state of chronic venous insufficiency in which increased microvascular filtration causes an increased rate of lymph production. Lymphatic drainage rate increases in response, but this is unsustainable and can cause lymphatic failure and oedema. We hypothesise that in phlebolymphoedema we could measure unusually high lymphatic drainage while the lymph system is still fully functional. METHOD: Patients referred for lymphoscintigraphic investigation of swollen legs between April 2021 and Decembe

Rest/stress intradermal lymphoscintigraphy in diagnosis of lipedema.

Lipedema is a chronic and progressive disease characterized by a symmetrical and bilateral swelling of the lower extremities. In general, the feet are not involved. Lipedema is believed to affect nearly 1 in 9 adult women worldwide. Despite this relatively common disease, lipedema is often confused with primary lymphedema or obesity. In clinically advanced lipedema stages, fat continues to build up and may block the lymphatic vessels causing a secondary lymphedema (Lipo-Lymphedema). We consecutively evaluated 54 women with a clinical diagnosis of lower limbs lipedema. Two doses of 99mTc-nanoco

Inadvertent Injection of a Breast Implant During Lymphoscintigraphy.

A 34-year-old woman with history of bilateral saline implants, invasive ductal carcinoma of the right breast, and suspicious left breast calcifications was referred for left breast lymphoscintigraphy before bilateral mastectomy. A total of 3.2 mCi of Tc-filtered sulfur colloid was administered with 3 subdermal injections in the superior, lateral, and inferior periareolar left breast in the late afternoon preceding the day of surgery. Imaging identified radiotracer uptake within a dense, circular area deep to the injection sites within the left breast, compatible with implant puncture and radio

Transmission Imaging in Lymphoscintigraphy with a 153Gd Flood Source.

UNLABELLED: Lymphoscintigraphy uses intradermal or interstitial injections of (99m)Tc-labeled tracers to produce images of focal lymph nodes. Because there is little or no anatomic information in the (99m)Tc images, a (57)Co flood source is sometimes used to provide transmission data along with the emission data. The anatomic shadow from the transmission scan generally improves interpretation and surgical planning. However, the (57)Co transmission photons contribute to background on the (99m)Tc images, reducing contrast and signal-to-noise ratio (SNR). SNR is related to lesion detection, and s

Lymphoscintigraphy in plastic bronchitis, a pediatric case report.

Plastic bronchitis (PB) is an uncommon, potentially fatal disease, marked by endobronchial cast formation causing variable degrees of respiratory distress. Primary and secondary pulmonary lymphatic abnormalities have been identified among the underlying mechanisms of cast formation. We present a case of PB where lymphoscintigraphy demonstrated the underlying lymphatic defect. A 6-year-old Hispanic male with congenital heart disease (CHD; post-Fontan) presented with recurrent pneumonia, respiratory distress. Bronchoscopy showed inflamed hypervascular mucosa and thick mucus plugs; no casts were
