Self-supervised representation learning reveals explainable physiological structure in high-dimensional magnetocardiography.

Artificial intelligence (AI) has shown strong performance in cardiology, but most approaches rely on sensing modalities whose physical limitations constrain available information. Magnetocardiography (MCG) records the cardiac magnetic field with less tissue distortion than surface potentials and may preserve higher-dimensional spatiotemporal electrophysiological structure. Here, we investigated whether combining MCG with self-supervised learning enables physiologically meaningful cardiac representations. We developed MCG2Vec, a contrastive encoder trained directly on raw 64-channel MCG recordi…

Magnetocardiography-based coronary artery disease severity assessment and localization using spatiotemporal features.

Objective. This study aimed to develop an automatic and accurate method for severity assessment and localization of coronary artery disease (CAD) based on an optically pumped magnetometer magnetocardiography (MCG) system.Approach.We proposed spatiotemporal features based on the MCG one-dimensional signals, including amplitude, correlation, local binary pattern, and shape features. To estimate the severity of CAD, we classified the stenosis as absence or mild, moderate, or severe cases and extracted a subset of features suitable for assessment. To localize CAD, we classified CAD groups according…

Optical Pumped Magnetometer Magnetocardiography in Left Main Disease With Coronary Microvascular Dysfunction.

BACKGROUND: Despite successful revascularization, a significant number of patients with obstructive coronary artery disease continue to experience angina because of often-overlooked coronary microvascular dysfunction (CMD). CASE SUMMARY: A 46-year-old man with hypertension, hyperlipidemia, and alcohol use presented with angina and left main stenosis; despite revascularization, symptoms and abnormalities on optical pumped magnetometer magnetocardiography (OPM-MCG) persisted, and AccuIMR (angiography-derived index of microcirculatory resistance) supported a diagnosis of CMD. Nicorandil led to c…

Fetal diagnosis of KCNQ1-variant long QT syndrome using fetal echocardiography and magnetocardiography.

A pregnant woman with KCNQ1 variant long QT syndrome (LQTS) underwent fetal magnetocardiography (fMCG) after atrioventricular (AV) block was noted during fetal echocardiogram-atypical for LQTS type 1. Concern for fetal LQTS on fMCG prompted monitoring of maternal labs, change of maternal beta blocker therapy, and frequent fetal echocardiograms. Collaboration between obstetricians, neonatologists, and pediatric cardiologists ensured safe delivery. Beta blocker therapy was initiated after birth, and postnatal evaluation confirmed genotype and phenotype positive LQTS in the infant. Our experience…

Reversal of atrial remodeling after cardioversion of persistent atrial fibrillation measured with magnetocardiography.

BACKGROUND: Atrial fibrillation (AF) causes electrical, functional, and structural changes in the atria. We examined electrophysiologic remodeling caused by AF and its reversal noninvasively by applying a new atrial signal analysis based on magnetocardiography (MCG). METHODS: In 26 patients with persistent AF, MCG, signal-averaged electrocardiography (SAECG), and echocardiography were performed immediately after electrical cardioversion (CV), and repeated after 1 month in 15 patients who remained in sinus rhythm (SR). Twenty-four matched subjects without history of AF served as controls. P-wa…

Temporal Convolutional Neural Network Analysis of Magnetocardiography Signals for Detection of Pulmonary Hypertension.

Non-invasive methods used for PH detection in clinical practice have several limitations. The combination of high spatiotemporal sensitivity magnetocardiography (MCG) and artificial intelligence algorithms may offer an accurate approach for PH detection. In this study, we develop a convolutional neural network (CNN) model based on the 64-channel MCG time-series data. This exploratory study enrolled patients undergoing 64-channel MCG, including right-heart-catheterization confirmed PH patients and symptomatic controls with low echocardiographic probability of PH. After data preprocessing, a tem…

Cardiac time intervals derived by magnetocardiography in fetuses exposed to pregnancy hypertension syndromes.

OBJECTIVE: To test the hypothesis that fetuses exposed to maternal preeclampsia or chronic hypertension have deranged development of cardiac time intervals. STUDY DESIGN: Pregnancies were divided into three groups: Intrauterine Growth Restricted (IUGR), Hypertensive, and Normal. Each group's mean fetal cardiac time intervals (P, PR, QRS and RR) derived by magnetocardiography were calculated using an analysis of covariance model's regression-adjusted estimates for a gestational age of 35 weeks. RESULTS: We reviewed 141 recordings from 21 IUGR, 46 Hypertensive and 74 Normal patients. The IUGR,…

Influence of intrauterine growth restriction on cardiac time intervals evaluated by fetal magnetocardiography.

OBJECTIVE: Differences in the cardiac excitation cycle between normotrophic and intrauterine growth-restricted fetuses were to be investigated by fetal magnetocardiography (fMCG). STUDY DESIGN: In this study, the time intervals of the fMCG signal in dependence on gestational age were compared between a group of 30 growth-restricted fetuses and 60 normotrophic fetuses by using Spearman's correlation coefficient and two-way analyses of variance. RESULTS: A significantly increasing duration of the P wave and the QRS complex could be observed with advancing gestational age in the normotrophic co…

Enhancing the efficiency and cost-effectiveness of magnetocardiography by optimal channel selection for cardiac diagnosis.

Background. Magnetocardiography (MCG) is a non-invasive and non-contact technique that measures weak magnetic fields generated by the heart. It is highly effective in the diagnosis of heart abnormalities. Multichannel MCG provides detailed spatio-temporal information of the measured magnetic fields. While multichannel MCG systems are costly, usage of the optimal number of measurement channels to characterize cardiac magnetic fields without any appreciable loss of signal information would be economically beneficial and promote the widespread use of MCG technology.Methods. An optimization method…

Bayesian approach for localizing cardiac sources in Magnetocardiography using Vectorcardiography based total variational priors.

The human heart produces electrical signals to contract and relax its muscles that helps in its blood pumping activities. These electrical impulses give rise to electric potentials on the body surface and tiny magnetic field around the thorax. These functional activities can be investigated using Electro/Magnetocardiogram (E/MCG). The more challenging task in the E/MCG research is to image the cardiac dysfunctions in three dimensions not at the surface level but at the source level and this is called the inverse problem. To solve this, one has to model a generic structure of the discretised he

Characterization of fetal arrhythmias by means of fetal magnetocardiography in three cases of difficult ultrasonographic imaging.

Characterization of ultrasound detected fetal arrhythmias is generally performed by means of M-mode and pulsed Doppler echocardiography (fECHO), sonographic techniques that allow only indirect and approximate reconstruction of the true electrophysiological events that occur in the fetal heart. Several studies demonstrated the ability of fetal magnetocardiography (fMCG) to identify fetal arrhythmias. We report on three women, studied after the 32nd gestational week, who were referred for fMCG because of unsatisfying fetal cardiac visualization with fECHO due to maternal obesity, fetus in consta…

A case of fetal complete heart block recorded by magnetocardiography, ultrasonography and direct fetal electrocardiography.

Fetal magnetocardiograms (FMCGs) were recorded in a case of fetal complete heart block (CHB) from the 30th to the 37th week of gestation using the multichannel SQUID system (Hitachi, Japan). M-mode ultrasonography and direct fetal electrocardiography using needle electrodes revealed fetal CHB. We identified independent fetal P-waves and QRS complexes in the FMCG recorded in the 32nd week of gestation when the fetal atriums were close to the FMCG sensor. We also recorded FMCG P-waves in the 37th week of gestation when the fetal heart was larger. Fetal heart position and size are important for o
