Nasotracheal intubation using the Airtraq versus Macintosh laryngoscope: a manikin study.

The Airtraq laryngoscope is a new intubation device that provides a non-line-of-sight view of the glottis. We evaluated this device by comparing the ease of nasotracheal intubation on a manikin with the use of Airtraq versus the Macintosh laryngoscope with and without Magill forceps. Nasotracheal intubation on a manikin was performed by 20 anesthesiologists and 20 residents with the Airtraq or Macintosh laryngoscope. The mean (+/- SD) time required for nasotracheal intubation by the residents was significantly shorter with the Airtraq laryngoscope than with the Macintosh laryngoscope (16 +/- 7

A comparison of direct and indirect laryngoscopes and the ILMA in novice users: a manikin study.

Direct laryngoscopic tracheal intubation using the Macintosh laryngoscope is taught to many healthcare professionals as it is a potentially life-saving procedure. However, it is a difficult skill to acquire and maintain. Several alternative intubation devices exist that may provide a better view of the glottis and require less skill to use. We conducted a prospective, randomised trial of four different laryngoscopes and the ILMA in 30 medical students who had no prior airway management experience. The devices were tested in both normal and cervical immobilisation laryngoscopy scenarios. Follow

Effect of chest compressions on the time taken to insert airway devices in a manikin.

BACKGROUND: Resuscitation guidelines recommend that chest compressions should continue throughout attempts to place airway devices. Few data support the use of the tracheal tube over supraglottic airway devices (SADs) during cardiopulmonary arrest. This study was designed to evaluate the speed with which different airway devices could be placed with and without interrupting chest compressions. METHODS: Forty volunteer doctors regularly involved in cardiopulmonary resuscitation (CPR) were timed inserting four different airway devices [tracheal tube (TT), LMA Classic (cLMA), LMA ProSeal (PLMA),

Three Airway Management Techniques for Airway Decontamination in Massive Emesis: A Manikin Study.

INTRODUCTION: Emesis occurs during airway management and results in pulmonary aspiration at rates of 0.01% - 0.11% in fasted patients undergoing general anesthesia and 0% - 22% in non-fasted emergency department patients. Suction-assisted laryngoscopy and airway decontamination (SALAD) involves maneuvering a suction catheter into the hypopharynx, while performing laryngoscopy and endotracheal intubation. Intentional esophageal intubation (IEI) involves blindly intubating the esophagus to control emesis before endotracheal intubation. Both are previously described techniques for endotracheal in

Manikin Laryngoscopy Motion as a Predictor of Patient Intubation Outcomes: A Prospective Observational Study.

BACKGROUND: The goal of this study was to determine whether motion parameters during laryngoscopy in a manikin differed with experienced ope … ttempts decreased by a factor of 0.982 (0.969-0.996, P = .045). DISCUSSION: Laryngoscopy motion in manikins may predict which trainees can complete a patient intubation successfully in a few attempts. The assessment could help determine readiness for intubating patients with indirect supervision.

Bougies as an aid for endotracheal intubation with the Airway Scope: bench and manikin comparison studies.

BACKGROUND: When encountering a difficult airway with an Airway Scope (AWS) a bougie can be inserted into the endotracheal tube in the AWS channel. The angulated tip of the bougie can be guided toward the glottis by rotating it. We tested the ease of rotating bougies (Venn reusable, Boussignac, Portex single-use, and Frova) in an endotracheal tube when placed in the AWS channel. METHODS: Bench study: Seven anesthesiologists inserted each of the four types of bougies into a 7.0 mm endotracheal tube in an AWS channel and rotated the bougie end (side of bougie operated by hand) clockwise or coun

Novel Visual Nasogastric Tube Insertion System: A Feasibility and Efficiency Study in a Manikin.

Background. Conventional nasogastric tube placement is an essential clinical procedure; however, complications may arise from blind manipulation. We tested the feasibility and efficiency of a visual nasogastric tube insertion system (VNGS) using a manikin. Methods. A microimaging fiber (0.8 mm) was integrated into the nasogastric tube to create the VNGS. Twenty inexperienced physicians were enrolled and assigned to the visual or conventional group. Each physician performed 10 repeated nasogastric tube insertions with visual guidance or the conventional method; another 20 inexperienced medical

Evaluation of Bag-Valve-Mask Ventilation in Manikin Studies: What Are the Current Limitations?

Introduction. Manikin-based studies for evaluation of ventilation performance show high heterogeneity in the analysis and experimental methods used as we pointed out in previous studies. In this work, we aim to evaluate these potential limitations and propose a new analysis methodology to reliably assess ventilation performance. Methods. One hundred forty healthcare providers were selected to ventilate a manikin with two adult self-inflating bags in random order. Ventilation parameters were analysed using different published analysis methods compared to ours. Results. Using different methods i

Patient-reported joint count in juvenile idiopathic arthritis: the reliability of a manikin format.

OBJECTIVE: To evaluate the reliability of a manikin format, patient-reported joint count in juvenile idiopathic arthritis (JIA), and to detect changes in agreement at a second visit. METHODS: Patients with JIA aged 12-21 were asked to mark joints with active arthritis on a manikin before their regular clinic visit. The physician then performed a joint count without having seen the patient's assessment. Agreement between scores of physician-reported and patient-reported joint counts was assessed using ICC. Kappa statistics were used to assess reliability of scoring individual joints. RESULTS:

Effectiveness and safety of in-water resuscitation performed by lifeguards and laypersons: a crossover manikin study.

OBJECTIVE: Drowning is associated with a high mortality and morbidity and a common cause of death. In-water resuscitation (IWR) in the case of drowning accidents has been recommended by certain resuscitation guidelines in the last several years. IWR has been discussed controversially in the past, especially with regard to the delay of chest compressions, effectiveness of ventilation, and hazard to the rescuer. The aim of the present study was to assess the effectiveness and safety of IWR. METHODS: In this crossover manikin study, 21 lifeguards and 21 laypersons performed two rescue procedures

Evaluation of the Pentax-AWS(®) and the Macintosh laryngoscope in difficult intubation: a manikin study.

BACKGROUND: The Pentax-AWS (AWS(®)), a new video laryngoscope, has been shown to be useful in cases of difficult intubation. We hypothesized that the AWS(®) would be more useful in the settings of a narrow upper airway than the Macintosh laryngoscope. We compared each device in simulated scenarios of representative difficulty of tracheal intubation using a manikin. The primary endpoint was the rate of successful intubation. METHODS: With each device, 23 anesthesiologists performed tracheal intubation in a SimMan(®) manikin in the following scenarios: (1) normal airway, (2) tongue edema, (3) c

Metronome improves compression and ventilation rates during CPR on a manikin in a randomized trial.

AIM: We hypothesized that a unique tock and voice metronome could prevent both suboptimal chest compression rates and hyperventilation. METHODS: A prospective, randomized, parallel design study involving 34 pairs of paid firefighter/emergency medical technicians (EMTs) performing two-rescuer CPR using a Laerdal SkillReporter Resusci Anne manikin with and without metronome guidance was performed. Each CPR session consisted of 2 min of 30:2 CPR with an unsecured airway, then 4 min of CPR with a secured airway (continuous compressions at 100 min(-1) with 8-10 ventilations/min), repeated after th

Manikin-based performance evaluation of N95 filtering-facepiece respirators challenged with nanoparticles.

Protection of the human respiratory system from exposure to nanoparticles is becoming an emerging issue in occupational hygiene. The potential adverse health effects associated with particles of approximately 1-100 nm are probably greater than submicron or micron-sized particles. The performance of two models of N95 half-facepiece-filtering respirators against nano-sized particles was evaluated at two inhalation flow rates, 30 and 85 l min(-1), following a manikin-based protocol. The aerosol concentration was measured outside and inside the facepiece using the Wide-Range Particle Spectrometer.

Effectiveness of subgingival instrumentation with power-driven instruments in the hands of experienced and inexperienced operators. A study on manikins.

Power instrumentation of periodontally-diseased root surfaces is gaining in significance as an alternative to conventional curette methods. In an experimental study employing manikins with simulated bone loss, we investigated whether inexperienced and experienced operators were able to achieve greater therapeutic success with power-driven devices than with hand instruments in subgingival scaling. 10 dentists experienced in periodontal treatment and 10 inexperienced dentists instrumented 7 teeth in the upper jaw, which had been covered with artificial deposits. Hand instruments, the Perioplaner…

Device capable of reducing gastric inflation during artificial ventilation in a manikin model.

Gastric inflation is a significant issue when ventilation is performed in cases of unprotected airway. The objective of this study was to compare the amounts of gastric insufflation and tidal volumes produced by a hose-extended bag-valve-mask (BVM) device supplemented by an interposed reservoir bag with a similar BVM without the reservoir in a simulated human model. Fourteen academic dental staff members performed 10 ventilations on a manikin using the reservoir-supplemented device in comparison to the control BVM in a randomized order. Lung compliance was adjusted to 45 (high) and 4.5 mL/mbar
