The records of 100 consecutive patients undergoing transantral ligation of the internal maxillary artery (IMA) for epistaxis, including 15 patients who developed postoperative bleeding, were reviewed. Preoperative parameters predictive of surgical failure were advanced age, anemia, and a history of hypertension. The causes of postoperative epistaxis, as determined by surgical reexploration or angiography in 12 cases, included failure to identify the IMA in the pterygomaxillary space (6 cases), blood flow through partially closed clips on the IMA (2 cases), bleeding from posterior ethmoid arter…

PURPOSE: Maxillary sinus carcinomas usually present as a locally advanced disease at the time of diagnosis and it is extremely unusual to have a second primary maxillary carcinoma on the contralateral side after many years of completion of treatment of the first malignancy. We present here a case report of a sphenopalatine artery (SPA) pseudoaneurysm mimicking the second primary maxillary carcinoma.
METHODS: We reviewed the literature for SPA pseudoaneurysm.
RESULTS/CASE REPORT: This report describes the case of a 90-year-old man with a background of adenoid cystic carcinoma of the right max

BACKGROUND: The maxillary artery can be injured during procedures in the subcondylar portion of the mandible. Thorough knowledge of this region is mandatory to avoid accidental puncture of the maxillary artery, which can lead to profuse bleeding that is hard to control.
METHODS: In 16 halves of eight embalmed cadaver heads, the maxillary artery was dissected from the branching point to the entrance point to the maxillary sinus. Its anatomical relationships with certain landmarks were recorded numerically.
RESULTS: The mean distance of the branching point of the maxillary artery to the tragal…

We report the endoscopic ligation of the maxillary and sphenopalatine arteries for the treatment of intractable epistaxis. From March 2003 t … ntraarterial embolization and less invasive than conventional surgical approach for the ligation of maxillary artery. This technique appears to be a safe and effective surgical treatment for patients with intractable epistaxis.

Selective embolization of the distal branches of the internal maxillary artery was performed to treat severe essential epistaxis in 11 patients. Elective indications for this technique are elderly patients with profuse epistaxis.

BACKGROUND: Intractable posterior epistaxis sometimes requires intensive treatment, such as surgery or embolization. Maxillary artery ligation has been widely used for the treatment of intractable posterior epistaxis. It is highly effective, but significant complications may occur, including an oroantral fistula and damage to the infraorbital nerve. Embolization is less invasive and can be performed in poor surgical candidates. However, it has more serious complications, such as facial nerve paralysis and hemiplegia. This investigation evaluates the effectiveness and complications of endoscopi…

Internal maxillary arterial pseudoaneurysm is a potential complication of maxillofacial surgery. A 34-year old woman with maxillary sinus cancer undergoing multiple endoscopic sinus surgery, and subsequently developed acute massive nasal hemorrhage. She was eventually successfully treated with intravascular embolization. The recurrence of nasal hemorrhage was not observed during a seven-month follow up.

AIM: To describe vascular anatomy of the maxillary sinus in dentate specimens dissected from human cadavers.
METHODS: Twenty dentate maxillary specimens were dissected, anatomically prepared, and injected with liquid latex for a better visualization of the maxillary sinus artery.
RESULTS: We found an intraosseous anastomosis in 100% and an extraosseous anastomosis in 90% of the cases. The anterior lateral wall of the maxillary sinus was transversed by two anastomoses between the posterior superior alveolar artery (PSAA) and the infraorbital artery (IOA). The PSAA was divided into a gingival

BACKGROUND: Embolization of the maxillary artery is a successful treatment, alternative in cases of recurrent severe nosebleeds when anterior and posterior nasal packing have failed to achieve permanent control.
PATIENTS AND METHOD: Two cases of male patients are presented who suffered from severe nosebleeds after a submucous resection of the septum, electrocautery of the inferior turbinate, and submucosal conchotomy. Anterior and posterior nasal packing proved to be unsuccessful. Angiography of the internal and external carotid artery was performed and selective embolization of the mayillary…

In our hands angiographic embolisation for epistaxis proved to be a good alternative to surgical occlusion of the maxillary artery. We used this approach in cases in which neither nasal packing nor other local treatment such as coagulation were successful and the bleeding site was within the region of the maxillary artery. The reported cases were all successfully controlled for several months. There were no side effects or complications as is sometimes reported. We can recommend embolization as an alternative procedure in such cases of epistaxis.

Following a review of 13 cases of epistaxis treated by internal maxillary artery with anterior ethmoidal artery ligation over a three year period at the Toronto General Hospital certain indications for ligation have evolved. These are: 1) hospital stay longer than five days, 2) total number of nasal packs more than three, 3) concomitant disease aggravating control, 4) blood transfusion in excess of five units, 5) massive epistaxis.

Epistaxis that originates from the posterior aspect of the nasal cavity is most often due to hemorrhage from one of the branches of the internal maxillary artery. There are multiple methods of treatment of this problem that vary in effectiveness, complexity, and stress on the patient. Most cases are controlled with either packing or surgical arterial ligation, but occasionally patients either cannot tolerate this therapy or continue to hemorrhage. We describe ten patients who were referred to the Neuroradiology Section of the Massachusetts General Hospital from the Massachusetts Eye and Ear In…

PURPOSE: This clinical study assessed the reverse facial artery-submental artery mandibular osteomuscular flap with titanium dental implants for the functional reconstruction of maxillary defects.
PATIENTS AND METHODS: Class 2a defects in 5 patients were repaired with a reverse facial artery-submental artery mandibular osteomuscular flap with titanium dental implants (n = 21). All patients received a fixed partial denture after a 3- to 6-month healing period.
RESULTS: All lesions were widely excised in an area extending to the maxilla. Of the implants, 20 (95.2%) were loaded and 1 was lost b

OBJECTIVE: The present study assessed the reliability of the reverse facial artery-submental artery deepithelialised submental island technique to reconstruct maxillary defects.
METHODS: The study included 13 patients (9 men and 4 women; 43-62 years) with maxillary defects resulting from cancer ablation. Ten patients presented with maxillary gingival squamous cell carcinoma and the remaining 3 cases were hard palate squamous cell carcinomas. The maxilla was resected and the remaining defects were classified as Class 2a. Reverse facial artery-submental artery deepithelialised submental island

The blood supply to both the lateral wall of the maxillary sinus and the overlying membrane originates from the posterosuperior alveolar artery (PSAA) and the infraorbital artery. The purpose of the present study was to evaluate the anatomic characteristics of the PSAA in a large number of subjects of the Lebanese population. Images of 696 sinuses were analyzed using cone beam computed tomography (CT). Coronal, axial, and sagittal CT images were evaluated for the presence of an osseous canal in the lateral wall of the sinus, and the prevalence, position, and location of the canal were studied
