Blindness as a complication of Le Fort I osteotomy for maxillary distraction.

High Le Fort I osteotomy and maxillary distraction has become an accepted method for the treatment of maxillary retrusion in children and teenagers with cleft lip and palate or craniofacial anomalies. This procedure effectively corrects the dentofacial deformity in these patients. No major surgical morbidity has been reported. During the past 4 years, 94 cleft patients with maxillary hypoplasia received Le Fort I osteotomy and distraction osteogenesis at the authors' center. Two of them developed blindness after this operation. The first case was a girl with bilateral cleft lip and palate with

Implant-osseous osteotomy for correction of the misaligned anterior maxillary implant.

The trend has been to use angled abutments to compensate for implants that have poor emergence profiles. This trend has resulted in increasing unfavorable forces being placed on the implant, poor soft tissue emergence and contours, or creation of areas that the patient cannot maintain properly. The end results are an esthetic compromise and ultimately implant failure. This case illustrates that a misplaced or misaligned integrated implant can be repositioned successfully within the maxillary alveolus. Careful treatment planning, flap design, and soft tissue manipulation must be considered to m

Le Fort I Osteotomy Complicated by Presence of Maxillary Sinus Septa.

Presence of maxillary sinus septa, an anatomical variant, has never been documented as a possible complication of Le Fort I orthognathic surgery procedure. This case report describes the difficulty faced by an experienced surgeon during Le Fort I down-fracture procedure. The osteotomized maxilla remained firm despite confirmation that all osteotomy cuts have been performed adequately. The presence of a bilateral sagittally oriented maxillary sinus septa extending from the floor to the roof of the sinus in this case had prevented separation of the lower maxilla during down-fracture maneuver. [Maxillary osteotomy: an alternative treatment concept for obstructive sleep apnea syndrome].

This study contributes to the evaluation of the question whether patients with obstructive sleep apnea syndrome might benefit from maxillary surgery. Changes in nasopharyngeal and hypopharyngeal airway spaces of 13 non-apneic patients following maxillary advancement and/or impaction osteotomies were studied retrospectively by cephalometric analysis. The data suggest a long-term increase in both nasopharyngeal airway space and unexpected, in hypopharyngeal airway space which remained stable over time. The clinical implications are discussed in view of previous studies which suggest or recommend

Comparison of Maxillary Distraction Osteogenesis and Conventional Orthognathic Osteotomy: A Systematic Review.

Maxillary hypoplasia, affecting 0.3% of the US population and nearly 25% of patients with cleft lip and/or palate (CLP), often results in Class III malocclusion with significant functional and esthetic challenges. Treatment options include LeFort I distraction osteogenesis (DO) and conventional osteotomy (CO), but reported outcomes vary widely. A systematic review of PubMed, Embase, Scopus, and CINAHL identified 17 studies (6 randomized controlled trials, 11 retrospective cohort studies) from 5076 screened. Outcomes assessed included skeletal relapse, speech changes, velopharyngeal insufficien

Association Between Maxillary Segmentation and Perioperative Complications in Le Fort I Osteotomy.

The Le Fort I osteotomy is used to reposition the maxilla to correct numerous maxillofacial and occlusal deformities. The aim of this study was to delineate perioperative complication rates associated with Le Fort I osteotomy and determine whether the number of maxillary segments or bone grafting yielded increased complication rates. Patients undergoing Le Fort I osteotomy from 2012 to 2019 were identified from the multi-institution "National Surgical Quality Improvement Program" database using Current Procedure Terminology codes. The predictor variables of interest included maxillary segmenta

Posterior maxillary segmental osteotomy concomitant with sinus lift using a piezoelectric device.

Tooth loss evokes functional and esthetic problems, and adjacent teeth can move if they are not corrected in a timely manner, rendering the problems of restoring the dentition more complex. If the opposing tooth is consequently extruded, several treatment modalities may be adopted, such as occlusal adjustment, crown prosthesis, orthodontic treatment, and segmental osteotomy. Segmental osteotomy is the most aggressive and invasive treatment for restoring a problematic occlusal plane due to the complications, such as hemorrhage, loss of tooth vitality, or necrosis of the segment.
