The nuts and bolts of hemisection treatment: managing congenitally missing mandibular second premolars.

When you treat the congenital absence of mandibular second premolars by extracting the residual deciduous second molar and closing the space, you run the risk of flattening facial fullness, especially in patients with nonextraction features--diminished facial fullness, absence of crowding, and reduced tooth size. This challenge is heightened by the increased likelihood that these features will occur in patients who have agenesis. This article describes a new approach that has been shown to minimize such a loss of anchorage. The technique is called hemisection and involves buccolingual sectioni

Induced ankylosis of a primary molar for skeletal anchorage in the mandible as alternative to mini-implants.

BACKGROUND: Mesial protraction of mandibular posterior teeth requires increased anchorage to avoid undesired tooth movements. Orthodontic mini-implants have become a popular and successful way to increase skeletal anchorage in such cases. However, mini-implants may cause injury to adjacent teeth or anatomical structures and may lead to tissue inflammation. Induced ankylosed primary teeth have been used in the past as abutments for the protraction of the maxilla in cases of maxillary retrognathism. However, this technique has not been described in the literature for the protraction of mandibula

Mesiodistal tip expression of mandibular anterior teeth in patients with mandibular incisor extraction treated with Invisalign aligners.

INTRODUCTION: Extraction of a single mandibular incisor may be prescribed in conjunction with clear aligner orthodontic treatment. The accuracy of therapeutic mesiodistal tooth positioning for the remaining mandibular anterior teeth for such patients has not been comprehensively evaluated. This retrospective study compared the predicted and achieved mesiodistal tooth movements in the mandibular anterior region for patients treated with Invisalign (Align Technology, Santa Clara, Calif), along with the influence of aligner attachments and the frequency of aligner change. METHODS: A sample of 83

Controlled slicing in the management of congenitally missing second premolars.

This report describes a simple method of allowing permanent first molars to drift mesially in patients with congenitally missing second premolars, thus facilitating future orthodontic treatment. Controlled slicing of the deciduous second molar between the ages of 8 and 9 years produced a bodily controlled mesial movement of the permanent first molar in less than 1 year with no or minor rotations or inclination. These results are compared with controlled slicing in 10- to 11-year-olds and with treatment involving extraction alone.

Asymmetric molars' mesial rotation and mesialization in unilateral functional posterior crossbite and implications for interceptive treatment in the mixed dentition.

INTRODUCTION: Symmetric transverse expansion is the main outcome of the early treatment in subjects with unilateral functional posterior crossbite. The aim of this study was to analyse mesial rotation and mesialization of upper first molars as sagittal parameters to be corrected in the treatment of these patients during the mixed dentition. METHODS: Digital dental cast measurements (rotation and mesialization) were performed in a sample of 48 subjects with unilateral posterior crossbite (UPXB; 19 males and 29 females, mean age 10.2 ± 1.2 years) and in a control group of 35 subjects with norma

Root resorption related to hypofunctional periodontium in experimental tooth movement.

Orthodontic movement of non-occluding teeth may result in undesirable apical root resorption. These teeth present with a histologically altered periodontium and are considered to be hypofunctional. The purpose of this study was to compare the amount of root resorption associated with a normal and a hypofunctional periodontium in rats during experimental tooth movement caused by heavy continuous force. The mandibular first molar was induced into a non-occluding condition in the hypofunctional periodontium group. Mesial orthodontic force was applied by means of 50-gram-force closed-coil springs

Identical unerupted maxillary incisors in monozygotic twins.

Mesiodens is the most common type of supernumerary tooth found in the premaxilla. It might be discovered by the orthodontist by chance on a radiograph or as the cause of an unerupted maxillary central incisor. The genetic transmission of supernumerary and impacted teeth is poorly understood. The occurrence of identical unerupted maxillary central incisors and mesiodentes in monozygotic twins suggests that genetic factors might influence the etiology of this problem. In this case report, we discuss the treatment of unerupted maxillary permanent incisors caused by mesiodentes in monozygotic twin

On tooth movements and associated tissue alterations related to edentulous areas and bone defects.

The aim of the thesis was to study orthodontic tooth movement in relation to edentulous areas and infrabony pockets as well as the physiological movement of teeth facing an edentulous area. A dog model was used in Studies I and II. Teeth were orthodontically moved into and out from inflamed, infrabony periodontal pockets (Study I) and into areas of reduced bone height (Study II). Clinical, radiographic and histometric analyses were made with respect to changes in tooth-supporting tissues. Study III involved clinical, radiographic and 3D model assessments of changes in periodontal conditions an

Three-dimensional finite element study of mandibular first molar distalization with clear aligner.

OBJECTIVES: This study aimed to construct the finite element model of the mandibular first molar with the invisible appliance and explore the dentition movement characteristics of the mandibular first molar when using micro-implant anchorage and different initial positions of the first molar. METHODS: Models of the mandible, tooth, periodontal membrane, and invisible appliance were constructed using cone beam computed tomography (CBCT) data. The two groups were divided into the non-anchorage group and the micro-implant group (between the roots of the first molar and the second molar) based on

Rate of mesial translation of mandibular molars using implant-anchored mechanics.

Retromolar dental implants served as anchorage to close first molar extraction sites in five adult patients. Rates of unidirectional space closure for mandibular second molars were assessed with periapical radiographs superimposed on anatomical landmarks and retromolar anchorage implants. Regression analysis revealed that mesial displacement of the midroot area of the leading root was less variable (r = 0.97) than for other landmarks on the same teeth: crown (r = 0.83), alveolar crest (r = 0.82) or apex (r = 0.90). When mesial root movement (uprighting) was the principal feature of the initial

[Early deciduous tooth loss--the mature or immature eruption of their permanent successors].

On the basis of 147 panoramic radiographs of 49 patients, this study investigated the influence of the premature loss of deciduous teeth on the formation of their permanent successors at the time of their eruption. Furthermore the study investigated the eruptive movement of the successor teeth at the time of eruption. In addition to detecting the accelerated emergence of still immature successor premolars, the study also determined that the extraction ot the deciduous molars before the age of eight years delayed the eruption of the permanent successors given the absence of an infected deciduou

[Histomorphometrical study of physiological tooth movements in humans].

Quantitative analysis of remodeling activity was done using microradiographs of horizontal ground sections of human alveolar bone obtained from autopsies. The results demonstrated no significant age- or sex-dependent variations. The local distribution of remodeling activity in younger individuals pointed to a tipping mesial movement of the teeth. In older individuals this distribution showed a more random pattern.
