Impact of initiating oral anticancer agents for leukemia on adherence to medications for multiple chronic conditions.

INTRODUCTION: Increased use of oral anticancer agents (OAAs) has empowered adults with chronic lymphocytic leukemia (CLL) and chronic myelogenous leukemia (CML) to manage their therapy, but this shift may complicate medication use, particularly among adults with multiple chronic conditions (MCC). METHODS: This retrospective cohort study used 2013-2018 commercial and Medicare claims data to assess medication use in adults with CML or CLL. To be included, patients must have been at least 18 years old, diagnosed with and had 2+ claims for an OAA indicated for either CML or CLL, continuously enro

Prevalence of multiple long-term chronic conditions and associated disabilities among community-dwelling adults in Riyadh.

BACKGROUND/OBJECTIVES: Saudi Arabia is experiencing a rapid increase in chronic diseases and disabilities. However, there is a dearth of research on these topics in the Arab world. This study aimed to examine the prevalence of multiple long-term chronic conditions (MLTCs) and disabilities and their relationship. METHODS: The survey was conducted in Riyadh, Saudi Arabia, in 2023. Convenient sampling was used to select 324 participants aged 50 years and older, using data on disabilities status. The survey collected information on age, sex, body mass index (BMI), MLTCs or multi-morbidity, and ac

Novel Electronic Health Record Strategies to Identify Frailty Among Hospitalized Older Adults with Multiple Chronic Conditions.

A growing number of aging adults are living with multiple chronic conditions (MCC). Older adults living with MCC are predisposed to developing frailty, a state of decreased physiologic reserve that increases risk for geriatric syndromes and associated morbidity and mortality. The electronic frailty index (eFI) is computed passively using structured EHR data and can aid in prospective screening. Unfortunately, certain diagnoses, such as functional status assessments in unstructured documentation, are less likely captured by eFI, potentially underestimating the degree of frailty. Here, we discus

Focus and features of prescribing indications spanning multiple chronic conditions in older adults: A narrative review.

BACKGROUND: Inappropriate prescribing is frequent in older adults and associated with adverse outcomes. Prescribing indications aim to optimize prescribing, but little is known about the focus and features of prescribing indications for the most common chronic conditions in older adults. Understanding the conditions, medications, and issues addressed (e.g., patient perspective, drug-disease interaction, adverse drug event) in current prescribing indications may help to identify missing indications and develop standardized measures to improve prescribing quality. METHODS: We searched Ovid/MEDL

Medication understanding among patients living with multiple chronic conditions: Implications for patient-reported measures of adherence.

BACKGROUND: Low health literacy is associated with poor medication adherence and poor health outcomes. Limited understanding of prescribed medications may decrease validity of patient-reported adherence measures. OBJECTIVES: To assess knowledge of names and purposes of prescribed medications among patients with multiple chronic conditions. METHODS: Individual interviews were conducted with a convenience sample of patients from six U.S. primary care clinics. Participants (n = 57) were English and/or Spanish-speaking patients prescribed 3+ medications for chronic conditions, for which non-adhe

Comparative Effectiveness of Coalitions Versus Technical Assistance for Depression Quality Improvement in Persons with Multiple Chronic Conditions.

SIGNIFICANCE: Prior research suggests that Community Engagement and Planning (CEP) for coalition support compared with Resources for Services (RS) for program technical assistance to implement depression quality improvement programs improves 6- and 12-month client mental-health related quality of life (MHRQL); however, effects for clients with multiple chronic medical conditions (MCC) are unknown. OBJECTIVE: To explore effectiveness of CEP vs RS in MCC and non-MCC subgroups. DESIGN: Secondary analyses of a cluster-randomized trial. SETTING: 93 health care and community-based programs in two

The Effects of Multiple Chronic Conditions on Adult Patient Readmissions and Hospital Finances: A Management Case Study.

Medicare and other payers have launched initiatives to reduce hospital utilization, especially targeting readmissions within 30 days of discharge. Hospital managers have traditionally contended that hospitals would prosper better by ignoring the penalties for high readmission rates and keeping the beds more full. We aimed to test the financial effects of admissions and readmissions by persons with and without specified chronic conditions in one regional hospital. This is a management case study with a descriptive brief report. This study was conducted at Winchester Memorial Hospital, a general

Examining therapeutic inertia patterns in type 2 diabetes patients with multiple chronic conditions: Time-to-event analysis.

OBJECTIVE: Therapeutic inertia (TI) in type 2 diabetes (T2D)-delayed treatment adjustment despite elevated A1C-is often complicated by multiple chronic conditions (MCC). We examined how clinicians address TI in patients with T2D and MCC and evaluated association with patient, clinic, and neighborhood factors. METHODS: Using electronic health records from two Midwestern U.S. health systems, we identified a retrospective six-year cohort of adults with T2D, MCC, and above-target A1C (>7% [53 mmol/mol] for age < 65; >8% [64 mmol/mol] for age ≥ 65). Time-to-event analyses assessed factors associa

How general practitioners perceive and assess self-care in patients with multiple chronic conditions: a qualitative study.

BACKGROUND: It is not known how general practitioners (GPs) perceive the concept of self-care and how they assess self-care ability in patients with multiple chronic conditions. As a part of the strategy to improve the care of people living with chronic conditions, disease management programs in Denmark require GPs and other health care workers to assess and support patients' self-care ability. The aim of the present study was to explore GPs' perceptions and assessment of self-care ability in patients with multiple chronic conditions who have difficulty following a given treatment. METHODS: A

Health care expenditures for adults with multiple treated chronic conditions: estimates from the Medical Expenditure Panel Survey, 2009.

The objective of this article is to illustrate the usefulness of Medical Expenditure Panel Survey (MEPS) data for examining variations in medical expenditures for people with multiple chronic conditions (MCC). We analyzed 2009 MEPS data to produce estimates of treated prevalence for MCC and associated medical expenditures for adults in the US civilian noninstitutionalized population (sample = 24,870). We also identified the most common dyad and triad combinations of treated conditions. Approximately one-quarter of civilian US adults were treated for MCCs in 2009; 18.3% were treated for 2 to 3
