Cycloplegic and mydriatic agents for routine ophthalmologic examination: a survey of pediatric ophthalmologists.

INTRODUCTION: We have used a mixture of tropicamide 1% and cyclopentolate 2% for routine cycloplegia in children. Because our hospital pharmacy expressed reluctance to prepare this mixture, we were interested in the current standard of care among pediatric ophthalmologists. METHODS: A survey was mailed to all members and associate members of the American Association for Pediatric Ophthalmology and Strabismus. Responses were tabulated from 522 respondents (62%). RESULTS: Mixtures were used by 22% of respondents for neonates and by 40% for older children. The most popular mixtures were cyclope

The follow-up of patients screened for glaucoma with non-mydriatic fundus photography.

In order to evaluate the value of photographic screening in predicting progressive glaucomatous damage, we re-examined 26 subjects 5 years after the initial screening. Of the 26 patients 16 had typical glaucomatous optic disc and visual field abnormalities (n = 7), retinal nerve layer damage (n = 6), or other risk factors of glaucoma (n = 3). In 10 of 26 patients suspected of having glaucoma, no abnormalities were initially confirmed. Of the 16 eyes with initially abnormal findings, 10 (63%) showed progressive changes during the 5-year follow-up period. The 10 initially suspected cases have re

Anticholinergic syndrome due to mydriatic drops intoxication in a child: A case report.

Anticholinergic syndrome consists of neurological and systemic symptoms, including agitation, mydriasis and dryness that can be an adverse reaction to a number of medications, some of them as seemly harmless as cycloplegic mydriatic drops. We present the case of a 7-year-old previously healthy female child who presented to the emergency room with impaired neurological status, inability to recognize family members and incoherent speech, as well as facial flushing, mucosal dryness and bilateral mydriasis after having mistakenly received a high dose of mydriatic drops. She made a complete recover

Predictive Value of Pupillary Response to Mydriatic Agents for Diagnosis of Retinopathy of Prematurity.

PURPOSE: To evaluate the feasibility of screening for retinopathy of prematurity (ROP) by assessing the pupillary response to mydriatics. METHODS: This observational case series included 134 eyes of 67 premature infants with birth weight less than 2,000 grams and gestational age less than 33 weeks. A composite eye drop composed of phenylephrine 1%, tetracaine and tropicamide 0.5% was applied 3 times within 5-minute intervals and pupil diameters were measured. The eyes were examined by experienced ROP specialists using an indirect ophthalmoscope. Zone and stage of ROP, presence of plus disease…

The local vasoconstriction of infant's skin following instillation of mydriatic eye drops.

BACKGROUND: Systemic absorption of eye drops is known to occur via the nasal mucosa, cornea, and conjunctiva. Diffusion of eye drops through the skin is previously unrecognized. Here, two cases are presented in which we observed skin pallor around the eyes after instillation of phenylephrine 2.5% drops. CASE 1: A 32-week gestational age premature infant had mydriatic eye drops instilled as part of retinopathy of prematurity screening. CASE 2: A term newborn dysmorphic infant underwent fundus examination to rule out ocular pathology. In both cases, discoloration of periorbital skin was observed

Impact of an eye-stroke protocol with non-mydriatic ocular imaging in an emergency department.

INTRODUCTION: The diagnosis of acute central retinal artery occlusion (CRAO) and branch retinal artery occlusion (BRAO) is often delayed or missed in emergency departments (EDs) because of limited ocular funduscopic skills and lack of immediate ophthalmology access. We evaluated the clinical impact of our Eye-Stroke protocol using non-mydriatic ocular imaging (non-mydriatic fundus photography and optical coherence tomography [NMFP-OCT]) in our general ED with remote interpretation by ophthalmology on the time to diagnosis and emergent management of acute CRAO/BRAO. METHODS: Prospective consec

Efficacy of topical rocuronium bromide as a mydriatic agent in domestic pigeons (Columba livia).

This study was conducted to investigate the efficacy of rocuronium bromide as mydriatic agent in domestic pigeons (Columba livia). This study was done in two phases. In the first phase, rocuronium bromide (0.20 mg/20 µl) was topically instilled to the right eye (OD) of eight domestic pigeons. Pupil diameter was measured before instillation (T0), and at 5 (T05) and 10 (T10) min after instillation, and every 10 min thereafter until 160 (T160) min. Pupillary light reflex (PLR) was assessed using a scoring system at the same time points. In the second phase, the same dosage was instilled twice in

75-degree non-mydriatic single-volume optical coherence tomographic angiography.

Optical coherence tomography (OCT) and OCT angiography (OCTA) enable three-dimensional, high-resolution imaging of the eye. Yet, while they provide unprecedented structural and angiographic imaging detail, both have only limited fields of view in comparison to other imaging modalities like fundus photography. In this paper, we present a high-speed, high-sensitivity, swept source laser-based system that can acquire non-mydriatic 75-degree field of view OCT and OCTA images in a single complete scan without resorting to montaging techniques. The system uses an optimized scanning protocol and achi

[Periorbital pallor post application of mydriatic in infants with hydrocephalus associated to systemic effects].

UNLABELLED: Adequate pupil dilation is needed to evaluate some neonates at risk of developing illness during this stage. However, this procedure is not free of adverse effects, either local or systemic. One of these complications is the local vasoconstriction of the preterm baby’s skin following the application of mydriatic eye drops. OBJECTIVE: To describe secondary local and systemic complications of pharmacological pupil dilation in 2 newborns. Clinical case 1: Full term baby with diagnosis of low-birth weight and hydrocephalia. An ophtalmological evaluation was performed at 5 days of age

Non-Mydriatic Ocular Fundus Imaging on Consecutive Neurologic and Neurosurgical Patients in an Emergency Department.

BACKGROUND AND PURPOSE: Examination of the ocular fundus is part of the neurologic examination. However, bedside ophthalmoscopy is rarely performed, especially in emergency departments (EDs), with risk of delayed diagnoses and inappropriate triage. Our goal was to determine how often funduscopic examination using non-mydriatic ocular fundus photography combined with optical coherence tomography (NMPF-OCT) is possible and useful in a consecutive cohort of patients with neurologic/neurosurgical disorders evaluated in a general ED. METHODS: This was a quality improvement project, prospective ove

Effect of Mydriatic Administration on Retinal Hemodynamics in Glaucoma: An Optical Coherence Tomography Angiography Study.

PRCIS: Pupillary dilation with 0.5% tropicamide and 2.5% phenylephrine resulted in a statistically significant decrease in foveal avascular zone (FAZ) metrics in glaucoma eyes. This observation emphasizes the critical role of pupillary status in interpreting glaucomatous vascular alterations detected by optical coherence tomography angiography (OCTA). PURPOSE: The purpose of this study was to evaluate the influence of mydriatic eye drops on OCTA parameters in patients with primary open angle glaucoma (POAG). MATERIALS AND METHODS: A total of 20 POAG eyes and 20 control eyes were recruited in
