Nephron function of the transplanted rat kidney.

Tubular function in the early phase (one to three hours) after transplantation of rat kidneys was analyzed with respect to glomerular filtration, vascular and tubular pressures, and excretory variables. Kidneys exposed to a short period of cold ischemia (two hours) functioned almost normally, except for a polyuria. After 12 and 16 hr of cold ischemia, nephron heterogeneity appeared with 1) "normal" tubules, 2) dilated tubules, and 3) collapsed tubules. In the "normal" tubules, the pressure was increased to 20 mm Hg, and the filtration was reduced in proportion to the mean net driving force. Th

Low Nephron Number and Kidney Disease.

Reduced nephron endowment and acquired nephron loss increase susceptibility to hypertension and chronic kidney disease (CKD). Low birth weight, prematurity, genetic factors, and adverse prenatal exposures can limit nephron development, whereas aging and kidney injury reduce the number of functioning nephrons after birth. Compensatory glomerular hypertrophy and increased single-nephron filtration can initially preserve overall glomerular filtration rate, but sustained glomerular stress may promote podocyte injury, proteinuria, and glomerulosclerosis. These changes can accelerate nephron loss an…

From Nephron Number to Global Health.

In 1988, in a seminal paper, Brenner and colleagues proposed that the numbers of nephrons acquired in utero may be a factor that determines an individual's lifelong risk of kidney disease. The hypothesis posited that a kidney with fewer nephrons would have a reduced filtration surface, a reduced capacity to excrete sodium, and a limited capacity to compensate for additional kidney injury and nephron loss. These factors increase the risk of high blood pressure and kidney dysfunction. Potential clinical markers for such risk include preterm birth, small-for-gestational-age birth and low birth we…

Prorenin receptor is critical for nephron progenitors.

Deficient nephrogenesis is the major factor contributing to renal hypoplasia defined as abnormally small kidneys. Nephron induction during k … ablation of PRR in Six2(+) nephron progenitors caused a marked decrease in the number of developing nephrons, small cystic kidneys and podocyte foot process effacement at birth, and early postnatal death. Reduced congenital nephron endowment resulted from premature depletion of nephron progenitor cell population due to impaired progenitor cell proliferatio …

Obesity-related glomerulopathy and the nephron complement.

Obesity-related glomerulopathy (ORG) is a secondary form of glomerular disease that can occur in individuals with obesity. However, the absolute risk for an obese individual to develop progressive renal deterioration is low. Therefore, obesity alone appears to be insufficient to develop such severe renal injury, and there are likely other factors that contribute to the development of this entity. The glomerular hyperfiltration found in patients with ORG has been postulated to lead to structural abnormalities in glomeruli, such as glomerulomegaly and focal segmental glomerular sclerosis, in a m

Diuretics and salt transport along the nephron.

The clinical use of diuretics almost uniformly predated the localization of their site of action. The consequence of diuretic specificity predicts clinical application and side effect, and the proximity of the sodium transporters, one to the next, often dictates potency or diuretic efficiency. All diuretics function by inhibiting the normal transport of sodium from the filtrate into the renal tubular cells. This movement of sodium into the renal epithelial cells on the apical side is facilitated by a series of transporters whose function is, in turn, dependent on the adenosine triphosphate (AT

Nephron-sparing surgery for renal angiomyolipoma.

OBJECTIVES: Angiomyolipoma (AML) is a benign renal tumor that may require treatment because of associated local complications. The present study was undertaken to evaluate the efficacy of nephron-sparing surgery (NSS) in the management of renal AML, with respect to the long-term preservation of renal function and absence of tumor recurrence. METHODS: From 1980 to 1997, 27 patients underwent NSS for treatment of renal AML. The clinical presentation, surgical approach, and outcome in these patients were analyzed. Surgical treatment for renal AML was indicated because of associated symptoms, siz

Hormonal control of distal nephron function.

The distal tubule and collecting tubules are important control sites of fluid and electrolyte excretion. In our presentation we consider the cell mechanisms of transport of sodium and potassium ions and the effects of several hormones. Aldosterone and antidiuretics stimulate potassium secretion directly, and the available evidence strongly suggests that this effect involves the principal cell population. Epinephrine inhibits potassium secretion at sites beyond the distal tubule. In addition to such direct effects, secondary factors such as hormone-induced changes in flow rate along the distal

Nephron functional heterogeneity in the postobstructive kidney.

The purpose of this study was to determine the distribution of nephron filtration rates between superficial and juxtamedullary nephrons (S/J ratio) in kidneys studied immediately after relief of 24 hr total obstruction (acute) and after relief of prolonged partial obstruction (chronic). Injection of 14C-ferrocyanide and microdissection (modified Hanssen's technique) was used to provide an index of superficial and deep nephron glomerular filtration rate (GFR), and standard clearance determinations were done. In normal kidneys S/J ratio of 14C content (nephron GFR) was 0.73 plus or minus 0.03, a…

NaCl and Ca delivery at the bend of rat deep nephrons decreases during antidiuresis.

The effects of 1-desamino-8-D-arginine vasopressin (dDAVP) on superficial and juxtamedullary nephrons were investigated by micropuncture in diabetes insipidus (DI) Brattleboro rats chronically treated with the peptide. The rats, acutely deprived of endogenous calcitonin, parathyroid hormone (PTH), and glucagon [hormone-deprived (HD) rats], were examined either 4 days after cessation of dDAVP treatment (HDT, control diuretic rats) or when the treatment was continued until the micropuncture experiment, during which dDAVP was also given intravenously (HDT + dDAVP, experimental nondiuretic rats). …

The problem of sampling homologous groups of nephrons during development of the chick mesonephros.

With the aim of defining homologous groups of nephrons in the chick mesonephros at different stages of development, we opened the abdominal wall of 295 White Leghorn embryos at stage HH 26 and marked off 6 segments of equal length at different levels of the organ by Seichert's plastic linear marking method (Seichert 1965), using marks previously introduced into the vertebral column at the same levels as a guide. The embryos were removed after 2-6 days, weighed and classified according to weight. The primordia of the marked vertebrae were identified. The length of the artificially defined segme…

Heterogeneity of osteopontin expression among nephrons in mouse kidneys and enhanced expression in sclerotic glomeruli.

BACKGROUND: Osteopontin (OPN) is a secreted Ca(2+)-binding phosphoprotein able to mediate cell attachment to bone via an RGD sequence and th … ial fibroblasts. In contrast to the localization of Tamm-Horsfall protein expression, in all distal nephrons, expression of OPN was detected by both methods of analysis in only some nephrons. OPN expression (relative to male mice) was somewhat increased in female, pregnant and lactating mice and markedly increased in the parietal epithelium of glomeruli un …

Transcription factors ETV4 and ETV5 are required for nephron progenitor cell maintenance, distal nephron development and connection to the collecting system.

The functional unit of the kidney, the nephron, is composed of the renal corpuscle and the renal tubule that includes proximal tubule, loop of Henle, distal and connecting tubule. Each of these segments perform distinct and complementary roles to ensure body homeostasis and urine excretion into the renal collecting system. During nephrogenesis, nephron progenitor cells (NPCs) either self-renew to maintain the progenitor pool or condense and epithelialize into nascent nephrons. Two transcription factors, ETV4 and ETV5 are downstream targets of receptor tyrosine kinase signaling and are known to…

Paracellular transport along the nephron in physiology and pathophysiology.

PURPOSE OF REVIEW: Tight junctions formed by claudins regulate paracellular permeability to sodium, chloride, calcium, magnesium, and water along the nephron, thereby enabling energy-efficient reabsorption of glomerular filtrate that limits tubular oxygen consumption. RECENT FINDINGS: We review evidence from knockout models, cell lines, and single-cell analyses demonstrating highly segment-specific claudin functions along the nephron. In the proximal tubule, independent claudin-2 and claudin-10a pores mediate cation/water and anion permeability, respectively, and inducible claudin-2 deletion

Computed tomography after nephron-sparing surgery.

The increased use of abdominal cross-sectional imaging has contributed to a greater detection of incidental small renal masses. Treatment options for localized disease renal cell carcinoma include radical nephrectomy or partial nephrectomy (PN), the former being preferred for treatment of early-stage tumors. The most adopted technique for follow-up imaging is contrast-enhanced computed tomography (CT), whose findings can cause uncertainty and unnecessary repetition of examinations. Our purpose is to describe CT findings after PN and to describe evolution in time of such images.
