Central nervous system manifestations of rheumatologic diseases.

PURPOSE OF REVIEW: To summarize the current literature on central nervous system manifestations of vasculitides and connective tissue diseases. RECENT FINDINGS: There have been advances in understanding the mechanisms behind the initiation and perpetuation of inflammatory processes in vasculitic neuropathy. Clinically relevant data have been obtained on the predictive criteria for a positive biopsy result in giant cell arteritis, the imaging characteristics of primary angiitis of the central nervous system, and Behçet disease, and the clinical and radiologic features of neuro-Behçet disease.

Viral diseases of the central nervous system.

Viral diseases of the central nervous system encompass a wide range of different processes, mainly inflammation affecting the brain (encephalitis), the meninges (meningitis), or a combined meningoencephalitis. The spinal cord can be affected as well (myelitis). Another group of viral-related disorders, sometimes without a clear pathophysiological mechanism disclosed, include post-viral illnesses. All of these groups of diseases are discussed in this article, with an emphasis on their imaging presentation, using magnetic resonance imaging.

[Various infective diseases of the nervous system].

A variety of viral, bacterial, fungal and spirochete infections may invade both the central and peripheral nervous system. The neurological hallmarks of these infectious diseases are very diverse and resulting in meningitis, meningoradiculitis, or as encephalomyelitis from the very start. Accordingly, patients may present with the cardinal signs of meningitis alone, or in association with focal neurological deficit, or with encephalopathy. This variability is related to the physiopathological mechanisms depending on the nature of the infectious agent. Thus the disease processes are due to the

[Central nervous system involvement in rheumatic diseases].

Among various rheumatic diseases, systemic lupus erythematosus and Behcet's disease are frequently complicated with serious neurological involvement, called CNS lupus and neuro-Behcet's syndrome (NB), respectively. CNS lupus includes lupus psychosis, such as organic brain syndrome and non-organic psychosis, as well as non-psychotic CNS lupus, such as epilepsy and focal lesions. Anti-ribosomal P antibody is closely associated with lupus psychosis, whereas anti-phospholipid antibodies appear to be involved in some manifestations of non-psychotic CNS lupus. NB includes acute type and chronic prog

[Morphological diagnosis of peripheral nervous system diseases].

Peripheral neuropathies are among the most common neurological diseases. Various tissues are available for morphological investigation, depending on the purpose of diagnosis. The sural nerve is most frequently used for nerve biopsy. The nomenclature of neuropathies is described together with prerequisites and techniques for nerve biopsy. Morphologically, a distinction can be made between parenchymatous and interstitial lesions. An account is given of the most important morphological patterns, such as axonal and neuronal degeneration and regeneration, including Waller's degeneration, segmental

Inherited metabolic diseases of the nervous system.

This overview was designed primarily to provide examples of hereditary metabolic disorders that result in nervous system dysfunction. Some of the more frequently encountered pathological conditions were selected in order to illustrate the mechanisms and the consequences of the metabolic derangements. Therapeutic approaches for the correction of such disorders are discussed where it appears appropriate. In time the precise etiology for those eponymous genetic conditions with stereotyped pathologic and clinical manifestations such as Huntington's chorea (79) and Friedreich's ataxia (80) will be

CRISPR/Cas9 system and its applications in nervous system diseases.

The clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated protein 9 (Cas9) system is an acquired immune system of many bacteria and archaea, comprising CRISPR loci, Cas genes, and its associated proteins. This system can recognize exogenous DNA and utilize the Cas9 protein's nuclease activity to break DNA double-strand and to achieve base insertion or deletion by subsequent DNA repair. In recent years, multiple laboratory and clinical studies have revealed the therapeutic role of the CRISPR/Cas9 system in neurological diseases. This article reviews the CRISPR/Cas

Neurosurgical gene therapy for central nervous system diseases.

Viral vector mediated gene therapies for neurodegenerative and neurodevelopmental conditions that require neurosurgical administration continue to expand. We systematically reviewed the National Institutes of Health (NIH) ClinicalTrials.gov database to identify all clinical trials studying in-vivo viral vector mediated gene therapies targeted to the CNS for neurodegenerative and neurodevelopmental diseases. We isolated studies which delivered therapies using neurosurgical approaches: intracisternal, intraventricular, and/or intraparenchymal. Clinical trials primarily registered in internationa

The Role of Mfsd2a in Nervous System Diseases.

Major facilitator superfamily (MFS) is the maximum and most diversified membrane transporter, acting as uniporters, symporters and antiporte … ntially in brain growth and integrity. Mfsd2a has attracted more and more attention in the study of nervous system diseases because of its critical role in maintaining the integrity of the blood-brain barrier (BBB) and transporting DHA, including inhibiting cell transport in central nervous system endothelial cells, alleviating BBB injury, avoiding BBB injury in cerebral …

Research Progress on circRNA in Nervous System Diseases.

Circular RNAs (circRNAs) are a kind of non-coding RNA molecule with highly stable circular structures. CircRNAs are primarily composed of exons and/or introns. Recently, a lot of exciting studies showed that circRNA played an essential role in the development of nervous system diseases. Here, classification, characteristics, biogenesis, and the association of circRNA dysregulation with nervous system diseases, such as Alzheimer's disease, are summarized. The review not only contributes to a better understanding of circRNAs, but also provides new research directions toward the diagnosis, treatm…

Venous endothelial injury in central nervous system diseases.

The role of the venous system in the pathogenesis of inflammatory neurological/neurodegenerative diseases remains largely unknown and underinvestigated. Aside from cerebral venous infarcts, thromboembolic events, and cerebrovascular bleeding, several inflammatory central nervous system (CNS) diseases, such as multiple sclerosis (MS), acute disseminated encephalomyelitis (ADEM), and optic neuritis, appear to be associated with venous vascular dysfunction, and the neuropathologic hallmark of these diseases is a perivenous, rather than arterial, lesion. Such findings raise fundamental questions a

[Central nervous system involvement in connective tissue diseases].

Neuropsychiatric manifestations are relatively common and serious complications in systemic lupus erythematosus (CNS lupus). Overall, in patients with CNS lupus, CSF IgM, IgA, IgG indexes (indicators of intrathecal Ig synthesis) as well as CSF IL-6 activities were significantly elevated. Of note, especially in patients lupus psychosis, but not in those with focal CNS lesions, anti-ribosomal P antibody (anti-P) in the sera as well as anti-neuronal antibody (anti-N) in the CSF was significantly elevated in relation to their CNS disease activities. These data indicate that the immune system activ
