Trigeminal Neuralgia (TN) is a well recognized syndrome characterized by lancinating attacks of severe facial pain. The diagnosis of TN is based on a history of characteristic pain attacks that are consistent with specific widely accepted criteria for the diagnosis. TN pain attacks may result from physiologic changes induced by a chronic partial injury to the brainstem trigeminal nerve root from a variety of causes. An early and accurate diagnosis of TN is important, because therapeutic interventions can reduce or eliminate pain attacks in the large majority of TN patients.

Following chicken pox infection, varicella-zoster virus stays as a latent infection in sensory root ganglia. After many years, the reactivation of this latent virus in sensory ganglia causes "herpes zoster". Herpes zoster (shingles) is an unilateral, dermatomal, localised, painful, vesicular, and contagious skin infection. In elderly and immunocompromised patients, shingles can cause complications such as postherpetic neuralgia (PHN) and direct central nervous system invasion. Early intervention with antiviral treatment, analgesic therapy and antidepressant therapy may reduce the risk of these…

The lesion of the sensorial nerves of the face is expressed by painful phenomena called neuralgias of the brainstem or its branches. These may be idiopathic or essential and secondary or symptomatic and their clinical evidence and course are different from the former. Essential neuralgia (E.N.), or tic douloureux, is the most frequent facial pain neuralgia. The aim of this paper is to review this subject with evidence of secondary trigeminal neuralgia from oral causes (dental, traumatic, post herpes zoster, etc). The author establishes the differential diagnoses with other etiologies of facial…

Review of current opinions concerning clinical presentation, etiology, differential diagnosis and management of vagoglossopharyngeal neuralgia. Three cases are reported, treated by intracranial section of n. IX (alone in one case) and upper rootlets of n. X. In two patients no vascular compression of the nerves was observed. In one case an atheromatous elongated basilar artery was observed compressing and deforming the medulla oblongata at the entry zone of nerves IX-X. In this case, pain paroxysms recurred some weeks after surgery, and the patient underwent re-operation (microvascular decompr…

We have encountered six zoster patients whose pain preceded rash by 7 to more than 100 days. Pain was severe, burning, and radicular, and located both in dermatomes different from, as well as in, the area of eventual rash. Two patients ultimately developed disseminated zoster with neurologic complications, one of zoster paresis, and the other, a fatal zoster encephalitis; both had been taking long-term, low-dose steroids. A third case of preherpetic neuralgia developed in a patient with prior metastatic carcinoma, and another case in a patient with an earlier episode of brachial neuritis. The …

Glossopharyngeal neuralgia, such less common than trigeminal neuralgia, is characterized by paroxysm of unilateral pain with sudden onset radiating from the oropharynx and base of the tongue, to the ear. Often precipitated by swallowing the glossopharyngeal neuralgia is occasionally accompanied by bradycardia or syncope. Conservative treatment consists of anticonvulsant drugs. Surgical treatment consists of section of the rootlets of the ninth and, in part, tenth cranial nerves. In some selected patients microvascular decompression without section of rootlets is also been made.

Postherpetic pain persisting 1 month or longer occurs in only a small percentage of all patients with herpes zoster. In most patients, PHN tends to diminish with time. The incidence is, however, directly related to age. Any therapeutic claim for prophylaxis or treatment of PHN has to be evaluated with these observations in mind. There is some information about the pathologic features and a concept of the pathogenesis can be suggested. There is evidence for an imbalance in fiber input (reduced large, inhibitory fibers, and intact or increased small, excitatory fibers) to an abnormal dorsal horn

Nine cases of glossopharyngeal neuralgia have been seen in the last 5 years and are reported. They fall into 2 groups, namely otitic and oropharyngeal, according to the initial location of pain. The condition is probably more common than is recognised, largely because there are diagnostic difficulties with the otitic form where the pain radiates widely into the distribution of the trigeminal nerve and also into upper cervical dermatomes. The pain is more variable than that seen in trigeminal neuralgia but usually responds initially to carbamazepine. The results from surgical section of the nin…

We report 2 cases of microvascular decompression of the nervus intermedius. The current views of aetiology of neuralgia and spasm of the cranial nerves are discussed based on intraoperative observations and electro-optical investigation reported in the literature. The complex anatomy within the sensory system of the facial nerve, and the intersection of its area of supply with that of the glossopharyngeal and vagus nerves are discussed. Surgical treatment is proposed with emphasis on the current preoperative difficulties of interpretation and the intraoperative constant variations in the cours…
