Two cases of bilateral adrenal neuroblastoma in infants are reported. The first patient presented with abdominal distension due to metastatic hepatomegaly and the second patient was found because of elevated urinary VMA by VMA mass-screening system. Both were under one year of age and were classified as stage IVs neuroblastoma. In each case unilateral adrenalectomy was performed and the contra-lateral adrenal tumor was observed. Report of clinical features, diagnostic and surgical aspects observed in a case of adult peripheral neuroblastoma infiltrative of the lumbosacral plexuses. The patient, a 39-year-old male, presented with sciatalgia and progressive crural monoplegia. Characteristic histological features of this rare tumor are described. Surgical resection was attempted via a posterior transgluteal approach to the retroperitoneum through the greater sciatic foramen.

Bilateral adrenal neuroblastoma is extremely rare, with isolated case reports considered to represent metastatic spread rather than simultaneous occurrence. This patient was found to have synchronous, separate, and equal size neoplasms that morphologically and ultrastructurally are neuroblastoma. We believe that these findings represent the unusual occurrence of simultaneous primary adrenal neuroblastoma reflecting multicentric origin of this neoplasm.

Two cases of intraventricular neuroblastomas were compared with cases of intraventricular and hemispheric neuroblastomas that have been reported in the published literature. The diversity of neuroblastoma and its clinical course depends on histology, biology and clinical features. We report a male presenting at 4 months of age with an abdominal mass and multiple subcutaneous nodules. The diagnosis was made by histological examination of a subcutaneous nodule and elevated urinary markers. The patient remained well during the subsequent 9 years. During that time no cytostatic treatment was given. Attempt to treat with cis-retinoic acid 10 years later did not result in any significant change of the clinical course. Primary congenital cervical neuroblastomas are very rare. A history of upper aerodigestive compromise with Horner syndrome can be of value for the early diagnosis of this lesion. Congenital neuroblastomas usually have a favorable outcome. Like all the other clinically relevant groups of neuroblastomas, management should take into consideration the biological findings of each tumor, which predicts outcome than other clinical findings.

A case of cerebral neuroblastoma originating in the cavernous sinus is reported. The patient presented with a left cavernous sinus syndrome and the pathological diagnosis came as a histological surprise. Subtotal removal of the tumour and radiotherapy was followed by recurrence and led to re-operation. A 3-month-old female with bilateral adrenal neuroblastoma had a unilateral adrenalectomy followed by cyclophosphamide treatment for 2 years. At 7, she has no evidence of disease. The favorable course of this patient and that of another young child reported with bilateral neuroblastoma as well as wide-spread metastases suggests that bilateral adrenalectomy may not be necessary for a child under 1 year of age with bilateral disease.

This is a review of the findings in 10 cases of stage IVS neuroblastoma, that have been observed in the First Surgical Department of Hokkaido University Hospital. The patients under the age of 1 yr in stage IVS neuroblastoma have a favorable prognosis; the survival rate was 70%. The regressions were histologically confirmed as maturation of the neuroblastoma cells to ganglioma cells.
