An optimality score for the neurologic examination of the term newborn.

We describe the application of a revised version of the Dubowitz neurologic examination of the newborn in 224 low-risk, term newborn infants. The method has been updated by eliminating less useful items and including new items evaluating general movements and patterns of distribution of tone. An optimality score is included to make the evaluation more quantitative and for comparison with sequential examinations with neurophysiologic and imaging findings. The score is based on the distribution of the scores for each item in the population of low-risk term infants. We defined not only the most c…

Neurological examination of the infant: A Comprehensive Review.

Clinicians are required to perform neurological examinations on infants to ensure they meet developmental milestones. A full neurological examination includes evaluating motor and sensory function, assessing the status of the cranial nerves, testing primitive reflexes, and atypical responses to further evaluate any developmental pathologies. The difficulty in maintaining infants' cooperation requires resourcefulness on the clinician's part to understand what is being tested and in what way to complete the examination. This literature will provide clinicians with guidance on the way to conduct

Neurological examination of cortical function deficits.

Human cortical functions have been elucidated by studies of deficits in traumatic and vascular brain damage, outcomes after elective neurosurgical procedures, studies in primates and in more recent years by imaging techniques. Cortical functions are well-defined for primary cortical areas like motor, sensory and visual functions. More complex cortical functions like language and to some degree memory are also well clarified. The associative cortical areas are more difficult to study as functions are integrated to and modulate primary cortical functions. Nevertheless, the structural basis for s

[Neuropsychological evaluation as a part of the neurological examination].

A systematic evaluation of cognitive function is an integral part of the neurological examination. Behavioral measures could detect functional impairment that is too subtle to be detected by current neurological procedures. Neuropsychological examination should cover both general and specific cognitive processes. The level of general attention or emotional change may affect variety of cognitive processes. Impairment of a functional system due to a focal cerebral damage results in a specific cognitive dysfunction such as aphasia, apraxia, agnosia, etc. Simplified but systematic methods to asses

Neurological examination in the healthy term newborn.

We carried out a cross-sectional study with a sample of 106 normal full-term newborns examined within 24 to 72 hours of birth. The following findings were evaluated: head and chest measurements, muscle strength, tone, tendon reflexes, superficial reflexes, primitive reflexes, and cranial nerves. All 106 newborns were considered neurologically normal. We found no differences in the neurological examination findings for newborns with different gestational ages. Primitive reflexes and appendicular tone in newborns examined at earlier postnatal ages tended to be less intense. We were able to deter

Neurological examination for cervical radiculopathy: a scoping review.

BACKGROUND: To diagnose cervical radiculopathy according to the International Association for the Study of Pain definition, signs of neurological deficits must be examined with the neurological examination. However, the diagnostic accuracy of the standard neurological examination remains unclear, and no clear recommendations exist about standard components. Therefore, the objectives of this review are to map the research about the diagnostic accuracy, components, and performance of the neurological examination for cervical radiculopathy. METHOD: PubMed, Embase, Scopus, Cinhal, DiTA databases

Neurological examinations for psychiatric patients: A systematic review.

The neurological examination is a key part of the diagnostic evaluation of psychiatric patients. There is general agreement that the canonical neurological examination should be modified for psychiatric patients, however there is a lack of consensus around what this should entail. We reviewed the literature on proposed neurological examinations for a psychiatric clinical setting. A systematic review, following PRISMA guidelines, was conducted to identify all published neurological examinations aimed at adult psychiatric patients. Information on items, length and target population was extracted

Collection of the digital data from the neurological examination.

This review presents the status quo of how far the digitalization of elements of the neurological examination has progressed. Our focus was on studies that assessed the examination conducted in person, rather than through telemedicine platforms. Five hundred and twenty studies were included in this systematic review. The digital tools covered ten elements of the neurological examination: gait (173, 33%), motor system (149, 29%), eyes (85, 16%), cognitive functions (53, 10%), sensory system (47, 9%), balance (35, 7%), other movements (24, 5%), other cranial nerves (24, 5%), coordination (10, 2%

Neurological Examination: An Evaluation of Video-Based Learning.

Objective This study aimed to systematically review and assess educational YouTube videos on neurological examination. Methods YouTube was screened for educational videos on neurological examination. A scoring system (involving five major and six minor criteria) was used to assess videos. Educationally useful videos were defined as those satisfying all major criteria and at least three minor criteria; 2 points were allocated for each major criterion and 1 point for each minor criterion, thereby using a score of 13 as a threshold. Results A total of 500 videos were screened, and 128 videos were

[Examination of patients with acute dizziness in a neurological department].

BACKGROUND: Acute dizziness may have a number of causes, including cerebrovascular stroke which can present as isolated acute vestibular syndrome. It is recommended that acute episodic dizziness be assessed using positioning tests, and acute persistent dizziness with a focus on the HINTS (Head Impulse, Nystagmus, Test of Skew) battery of tests, which can distinguish cerebrovascular stroke from vestibular neuritis. We wished to identify the prevalence, diagnostic spectrum and approach to acute dizziness in a neurological department. MATERIAL AND METHOD: We undertook a retrospective review of t

Applying the extensor digitorum reflex to neurological examination.

AIM: To determine the value of the extensor digitorum reflex in neurologic examination. METHODS: The extensor digitorum, biceps, and brachioradialis reflexes were elicited in 65 patients with hemiplegia and upper-limb paralysis and in a control group of 120 apparently healthy people. Reflexes were elicited by both conventional means and a new method for the extensor digitorum reflex. The sensitivity and specificity of the extensor digitorum reflex were compared with that of the conventional biceps and brachioradialis reflexes to evaluate the value of the extensor digitorum reflex for neurolog…
